Abstract: Aims: Health problems associated with indoor environments have been reported and discussed extensively during the past few decades, not least in Sweden. There is, however, great uncertainty concerning the background prevalence of the symptoms in question. The main objective of the present study was to investigate the prevalence of general, mucosal, and skin symptoms in the Swedish population. Methods: A survey comprising 3,000 randomly selected Swedes, age 18—64, was carried out. The survey addressed 25 symptoms, principally general, mucosal, and skin symptoms. A number of other areas were covered and individual data registered at Statistics Sweden were added. The response rate was 70% (2,154 cases). Results: The prevalence of symptoms in the Swedish population was found to accord with results in studies based on different kinds of samples. Abstract: Facial punch biopsies from 83 patients with skin complaints with or without skin lesions and supposedly associated with work at visual display units (VDUs) were compared to biopsies from 51 subjects with no VDU exposure with or without skin lesions. Coded slides were assessed by three independent observers regarding hyperplasia of sebaceous glands, occurrence of telangiectases, intensity of inflammatory infiltrate, degree of hydropic degeneration of basal cells, occurrence of demodex folliculorum, number of mast cells and degree of degenerative changes in elastic fibers. No parameter was significantly more common in exposed subjects than in non-exposed persons with equivalent skin signs. The controls without skin lesions also showed various degrees of histologic changes. Hyperplasia of sebaceous glands was more common in men and in young persons. Degenerative changes in elastic fibers increased with age, but were also obvious in some persons under 35 years of age. In this study no characteristic histologic changes in facial skin were found in individuals with skin symptoms claimed to be associated with exposure to VDUs.

Abstract: A questionnaire about facial skin problems was answered by 3745 office employees with the aim of discovering whether VDU work causes skin disease. From this group, 809 randomly selected persons were examined and interviewed by a dermatologist. There was a relatively good correspondence (87%) between the results from the interviews and the questionnaires, but the questionnaire results corresponded to current status in only 46% of the cases. No important difference was seen between those who were informed and those who were not informed about the purpose of the investigation. There was a relatively wide variation in the correlation between two responses to the same questions. In conclusion, the self-administered questionnaire seems to give reasonably adequate answers regarding the occurrence of more clear-cut symptoms over a period of time, rather than answers regarding the presence of signs at a particular point in time. However, less marked changes might not be picked up particularly well and the use of questionnaires alone cannot be recommended when studying mild skin problems due to a particular exposure in a population.

Abstract: Because of its long-term clinical use there is more information and research data available about dental amalgam than about any other dental restorative material. However, on and off the safety of dental amalgam has been called in question and during the 80's the mercury controversy came to the fore, not only within the profession but also among the general public. Sources of mercury vapor contamination within dentistry were identified and attempts made to evaluate the contributions to the daily mercury uptake which can be attributed to dental amalgam. Mercury can be released from dental amalgam by evaporation and electrochemical corrosion as well as from amalgam particles which have been swallowed. A major route for mercury uptake from amalgam restorations is through inhalation of mercury vapor. The present study focused on experimental and analytical difficulties associated with the measurement of mercury vapor released in the oral cavity. A careful methodological study of the kind of source of mercury vapor that is prevalent and on the methods for measuring the intra-oral release of mercury vapor was carried out. With this as a basis quantitative determinations of the release rate of mercury vapor from amalgam restorations were made on healthy human subjects not occupationally exposed to mercury. The daily uptake of mercury from inhaled mercury vapor was calculated and salivary and urinary mercury levels were determined. In addition the release rate of mercury vapor from different types of amalgam was studied in vitro and in vivo. The findings may be summarized as follows: The only relevant measurable quantity when determining the mercury vapor released from amalgam restorations is the amount released per time unit, i.e. the amount of mercury vapor collected during intra-oral sampling is proportional to the sampling time. The diffusion of mercury atoms inside an amalgam restoration results in the formation of a concentration gradient in the surface of the amalgam. This mercury diffusion is the rate-determining step for mercury vapor release in the long run. In the short run the mercury concentration gradient prevalent on the amalgam surface on the measuring occasion is the apparent rate-determining step. The daily uptake of mercury from inhaled mercury vapor released from dental amalgam seems to make a very small contribution to the total body burden of mercury, in comparison with what can be tolerated in the work environment. The in vitro results revealed obvious differences regarding the release rate of mercury vapor from dissimilar amalgam types.(ABSTRACT TRUNCATED AT 400 WORDS)

Abstract: Twenty patients with symptoms of "environmental illness" were subject to a controlled study of deep versus superficial acupuncture. The patients were evaluated by a detailed questionnaire concerning their occupational, environmental, and medical history. Blood samples were taken as well. Patients were randomized to deep or superficial acupuncture. Both groups improved significantly in key variables during and after treatment. There were no group differences. There were no changes in biological variables apart from a gradual and continuous increase in serum cortisol and a decrease in neuropeptide Y, which was somewhat more accentuated in those receiving deep acupuncture. This rise in cortisol may have contributed to decreased dermal symptoms among the participants. It is hypothesized that the positive treatment results observed are partly due to weakening of the conditioned response, linking bodily symptoms to environmental agents. To date, a number of different methods have been tried in the management of patients with environmental illness. However, only rarely have the treatments been evaluated in controlled studies.

Abstract: This study analyzed the experimental data about the relationship between exposure to 50/60-Hz electric and magnetic fields (EMF) and melatonin secretion by the pineal gland. The authors report their results and discuss possible health consequences, in the light of epidemiologic data suggesting that breast neoplasms and depressive disorders might be related to EMF. The nocturnal peak of melatonin has been reported to be diminished, or time-shifted, or both, in rodents exposed to electric and/or magnetic fields. Current experimental data from primates and humans are insufficient to show that this change occurs in them. Epidemiologic studies of associations between EMF exposure and breast cancer and between EMF exposure and depressive disorders suggest that modifications of melatonin secretion could be a biologic signal of these effects. Nonetheless, changes in melatonin secretion cannot yet be considered a verified biologic explanation of any such association.
