Humanización en Unidad de Terapia Intensiva para Adulto (UTI): comprensiones del equipo de enfermería

Abstract: Trata-se de estudo descritivo de abordagem qualitativa cujo objetivo foi compreender como os profissionais da enfermagem (enfermeiros e tecnicos) percebem a politica de humanizacao no cenario de uma UTI e sua importância nesse processo. Foi desenvolvido em uma instituicao privada no municipio de Porto Alegre/RS, com 18 participantes. A coleta dos dados ocorreu por meio de entrevista estruturada. Da analise emergiram aspectos referentes a elementos e caracteristicas que definem a humanizacao, bem como questoes facilitadoras e dificultadoras presentes no processo. Conclui-se que a empatia, o respeito e a valorizacao constituem elementos fundamentais e que o profissional de enfermagem acredita fazer a diferenca no processo de humanizar, com vistas a melhorar as praticas de cuidado baseadas na etica, no dialogo e na autonomia do paciente, de sua familia e da propria equipe.

Abstract: The study aimed to investigate the scientific literature on Public Humanization Policies, available in online periodicals, from 2009 to 2012, in the health field. This is an integrative literature review conducted in the Virtual Health Library databases: Latin-America and Caribbean Health Sciences (Lilacs) and the Scientific Electronic Library Online (SciELO) and Portal Capes. Data were collected in July 2013. To this end, the following Health Sciences Descriptors (DeCS) were used: "Humanization of Care," "Public Policies," "National Humanization Policy". The sample consisted of 27 articles about the investigated theme. From the publications selected for the research, three categories emerged according to their respective approaches: National Human-ization Policy: history and processes involved in its implementation; National Humanization Policy: health professionals contribution; Humanization and in the care process. The study showed that the National Humanization Policy is an important benchmark in the development of health practices. For this reason, there is a pressing multiplication of related reflections on ways to promote human-ization in health services.

Abstract: This article aims to understand the meaning of the process of dying with dignity in an intensive care unit from the perception of nurses. A qualitative study was performed with 12 nurses of the lato sensu nursing specialization course in adult, pediatric and neonatal ICUs of the Pontifical Catholic University of Minas Gerais, Brazil, through the discourse analysis approach proposed by Laurence Bardin. An interview was used as the research instrument. Following discourse analysis, five categories emerged as discussed throughout the article. It was concluded that, in the perception of the nurses, orthothanasia is a practice that respects human dignity through a dignified death. Additionally, it is surrounded by bioethical principles that allow the suspension of curative measures, providing space for palliative care as a mechanism for reducing the suffering and pain of patients in the terminal phase.

ABSTRACT This descriptive, exploratory and qualitative study was developed at a public Intensive Care Unit (ICU) in Goiânia (Goias, Brazil), with the objective to describe the concept and practice of care humanization developed by nurses of the referred ICU, who form a team undergoing a continuous process of development and scientific study. Data collection was performed in 2011, with seven nurses of the ICU, by means of taped semi-structured interviews, which were transcribed and categorized by content analysis, revealing the categories “The concept of humanization” and “Practice in the everyday intensive care routine”. Nurses are familiar with the concept and know how to practice humanized care, but still do not apply that knowledge in every situation, assigning this to external factors. It is necessary to identify the bottlenecks of this process to help plan the interventions needed to produce effective changes in the behavior of these workers. Abstract: Considerando-se a importância das estrategias de humanizacao e intervencao hospitalar no atendimento ao recem-nascido enfermo, esta pesquisa investigou a concepcao de humanizacao da equipe de profissionais de tres Unidades de Terapia Intensiva Neonatal da Regiao Metropolitana da Grande Vitoria, Espirito Santo. Participaram do estudo 29 profissionais de saude, entre medicos, enfermeiros, fonoaudiologos, fisioterapeutas e tecnicos de enfermagem, os quais responderam a roteiros de entrevistas semiestruturadas. Os dados das entrevistas foram processados pelo software Alceste e submetidos a Analise de Conteudo. Concluiu-se que os profissionais compreendem o cuidado humanizado a partir do resgate da perspectiva afetiva, em oposicao ao modelo medico-tecnicista de atencao a saude, ou seja, com enfase nos aspectos emocionais que envolvem sua relacao com o bebe e com o trabalho em Neonatologia. A participacao da familia foi o aspecto mais relevante para os profissionais, que expressaram a importância da permanencia dos pais na unidade de terapia e sua participacao nos cuidados ao recem-nascido.

Abstract: Objective: to identify what the national literature has addressed about humanization of care in Intensive Care Units in online databases from 2009 to 2013. Method: this is an integrative review conducted through access to databases: LILACS and BDENF, using the keywords "humanization of care" and "Intensive Care Units". Results: six studies were selected. The results showed that the humanization of care contributes significantly in the recovery of the patient in the Intensive Care Unit. However, there are difficulties to be overcome, especially related to the patient and their families, the nursing staff and health care institutions. Conclusion: there is need for training and sensitization of professionals, increasing investments in training, institutional management and care to improve care for critical patients. Objetivo: identificar o que a literatura cientifica nacional tem abordado acerca da humanizacao do cuidado nas Unidades de Terapia Intensiva em bases de dados online, no periodo de 2009 a 2013. Metodo: trata-se de uma revisao integrativa realizada atraves do acesso as bases LILACS e BDENF, utilizando os descritores “humanizacao da assistencia” e “Unidades de Terapia Intensiva”. Resultados: seis estudos foram selecionados. Os resultados evidenciaram que a humanizacao no cuidado contribui de maneira significativa na recuperacao do paciente em Unidade de Terapia Intensiva. Entretanto, ha dificuldades de efetivacao a serem superadas, sobretudo, relacionadas a questoes do paciente e seus familiares, da equipe de enfermagem e das instituicoes de saude. Conclusao: ha necessidade de capacitacao e sensibilizacao dos profissionais, incremento de investimentos na formacao, gestao institucional e do cuidado para melhorias na assistencia ao paciente critico. Abstract: A proposta deste estudo qualitativo e analisar a producao cientifica sobre "humanizacao em saude/enfermagem", compreendendo quais concepcoes sobre humanizacao vem se configurando. Realizou-se levantamento bibliografico, sendo analisados 42 artigos, do final da decada de 50 ate os dias atuais, nos periodicos Revista Brasileira de Enfermagem, Revista Paulista de Hospitais e Texto e Contexto. Efetuaram-se analise e sintese integrativa dos mesmos. A tematica vem se constituindo, desde uma perspectiva caritativa ate a preocupacao atual com a valorizacao da saude como direito do cidadao, sendo inserida em projeto politico de saude. Artigos de todas as decadas mostram a necessidade de investir no trabalhador, valorizando a dimensao subjetiva. Todavia, a tematica e pouco abordada na formacao. A humanizacao do atendimento supoe encontro entre sujeitos que compartilham saber, poder e experiencia vivida, implicando em transformacoes politicas, administrativas e subjetivas.

Abstract: Acompanhamos o debate em torno do tema da humanizacao no campo da saude,impulsionado recentemente pela construcao da Politica Nacional de Humanizacao daatencao e da gestao na saude (PNH), com a qual estivemos implicados em 2003 e 2004na posicao de integrantes da equipe da Secretaria Executiva (SE) do Ministerio da Saude(MS).Ainda que timidamente, este tema se anuncia desde a XI Conferencia Nacional deSaude, CNS (2000), que tinha como titulo “Acesso, qualidade e humanizacao na atencaoa saude com controle social”, procurando interferir nas agendas das politicas publicas desaude. De 2000 a 2002, o Programa Nacional de Humanizacao da Atencao Hospitalar(PNHAH) iniciou acoes em hospitais com o intuito de criar comites de humanizacaovoltados para a melhoria na qualidade da atencao ao usuario e, mais tarde, aotrabalhador. Tais iniciativas encontravam um cenario ambiguo em que a humanizacao erareivindicada pelos usuarios e alguns trabalhadores e, no minimo, secundarizada (quandonao banalizada) pela maioria dos gestores e dos profissionais. Os discursos apontavampara a urgencia de se encontrar outras respostas a crise da saude, identificada pormuitos como falencia do modelo SUS. A fala era de esgotamento. De fato, cada posicaoneste debate se sustenta com as suas razoes. Por um lado, os usuarios porreivindicarem o que e de direito: atencao com acolhimento e de modo resolutivo; osprofissionais, por lutarem por melhores condicoes de trabalho. Por outro lado, os criticosas propostas humanizantes no campo da saude denunciavam que as iniciativas emcurso se reduziam, grande parte das vezes, a alteracoes que nao chegavam efetivamentea colocar em questao os modelos de atencao e de gestao instituidos. Vale destacar queentre os anos 1999 e 2002, alem do PNHAH, algumas outras acoes e Programas forampropostos pelo Ministerio da Saude voltados para o que tambem ali ia se definindo como

Abstract: The purpose of this paper is to prompt discussions of the perspectives of the Evaluation of Brazil's National Health Humanization Policy being implemented by the Ministry of Health (PNH/HumanizaSUS). It strives to systemize some Health Evaluation benchmarks, distinguishing two key aspects of this evaluation: one examines methodologies for the definition of Humanization actions monitoring indicators; the other analyzes evaluatory surveys measuring the impacts of these actions. Both aspects were scored, highlighting specific characteristics that must be recognized in the approach and use of evaluation methodologies under the aegis of this Policy. In order to strengthen such specific characteristics and approach perspectives, it explores the extent and complexity of the tools and actions proposed in the field covered by this Policy, while also offering an brief review of the concepts and components encompassed by the Monitoring and Evaluation area.

Abstract: The interest on this theme emerged from a reflection on the role of health professionals in relation to the attendance carried out in hospitals. Thus, the aim is to transmit other pieces of information to these professionals, regarding health concerns, in a simpler and humanized way, thereby, leading to an atmosphere of well-being to the whole group or working team. Government adopts legal and ethics policies in relation to the heath in the country, holding a fundamental role in the humanization process. When aiming at improving the quality of hospital service, it was noticed that such activities require time and understanding on part of: professionals, government and also on part of all the people involved in the health system. Thus, the psychologist appears, with the purpose of rescuing the human being, beyond their physic-biological dimension, placing them into a bigger context of senses and meanings regarding their psychic and social dimensions.
