Hyper-alimentation - effects on health and well-being

Abstract: The general aim of this thesis was to prospectively examine the effects on health and well-being when healthy normal weight individuals increase their energy intake, mainly from fast food and simultaneously adopt a sedentary lifestyle.This thesis is based upon a prospective experimental study design where 18 healthy normal weight individuals, 12 men and 6 women, aged 26 (6.6) years, increased their energy intake with in average 70 % during four weeks. Simultaneously their physical activity was limited to a maximum of 5000 steps per day. An age and gender matched control group (n=18), was recruited and asked not to change their eatingand physical activity habits for four weeks. Long-term follow-up measurements were performed after 6 and 12 months and 2.5 years after the intervention.During the intervention body weight increased with 6.4 (2.8) kg and measurements of body composition showed an increase of both fat mass and fat free mass after the intervention. Lower physical and mental health scores on SF-36 as well as depressive symptoms were found compared to baseline. They were temporary and when followed up 6 and 12 months after the intervention, physical and mental health had returned to baseline values, despite a somewhat increased body weight. The main essence of adopting an obesity provoking behaviour was lack of energy emerging from five structures: influenced self-confidence, commitment to oneself and others, managing eating, feelings of tiredness and physical impact. Laboratory measurements showed an increase of ALT above reference limits in 14 of the 18 participants during the intervention and HTGC increased, although this was not related to the increase in ALT levels. Twelve months after the intervention an increase of body weight with 1.5 (2.4) kg was found compared to baseline (p=0.018), fat free mass was unchanged compared to baseline while fat mass had increased, + 1.4 (1.9) kg (p=0.01). Two and a half years after the intervention an increase of body weight with 3.1 (4.0) kg was found compared to baseline (p=0.01), while there was no change in controls compared to baseline, + 0.1 (2.5) kg (p=0.88).Hyper-alimentation and limited physical activity during a short-term period of 4 weeks is sufficient to temporarily induce worsened HRQoL, cause depressive symptoms and lack of energy in healthy normal weight individuals. There were also temporary but clear effects on biochemical markers, suggesting that hyperalimentation per se can induce profound ALT elevations in less than 4 weeks. During the intervention both fat mass and fat-free mass increased while after 12 months there was only an increase of fat mass which was greater than expected from epidemiological studies. The marked difference between the increase in body weight in the intervention- and control group at 2.5 years also raises the question whether there is a long-term effect of increasing fat mass after a short period of hyperalimentation.

Abstract: Few studies have investigated the associations of takeaway food consumption with overall diet quality and abdominal obesity. Young adults are high consumers of takeaway food so we aimed to examine these associations in a national study of young Australian adults. A national sample of 1,277 men and 1,585 women aged 26–36 completed a self-administered questionnaire on demographic and lifestyle factors, a 127 item food frequency questionnaire, usual daily frequency of fruit and vegetable consumption and usual weekly frequency of takeaway food consumption. Dietary intake was compared with the dietary recommendations from the Australian Guide to Healthy Eating. Waist circumference was measured for 1,065 men and 1,129 women. Moderate abdominal obesity was defined as ≥ 94 cm for men and ≥ 80 cm for women. Prevalence ratios (PR) were calculated using log binomial regression. Takeaway food consumption was dichotomised, with once a week or less as the reference group. Consumption of takeaway food twice a week or more was reported by more men (37.9%) than women (17.7%, P < 0.001). Compared with those eating takeaway once a week or less, men eating takeaway twice a week or more were significantly more likely to be single, younger, current smokers and spend more time watching TV and sitting, whereas women were more likely to be in the workforce and spend more time watching TV and sitting. Participants eating takeaway food at least twice a week were less likely (P < 0.05) to meet the dietary recommendation for vegetables, fruit, dairy, extra foods, breads and cereals (men only), lean meat and alternatives (women only) and overall met significantly fewer dietary recommendations (P < 0.001). After adjusting for confounding variables (age, leisure time physical activity, TV viewing and employment status), consuming takeaway food twice a week or more was associated with a 31% higher prevalence of moderate abdominal obesity in men (PR: 1.31; 95% CI: 1.07, 1.61) and a 25% higher prevalence in women (PR: 1.25; 95% CI: 1.04, 1.50). Eating takeaway food twice a week or more was associated with poorer diet quality and a higher prevalence of moderate abdominal obesity in young men and women.

Abstract: Although lipid profiles tend to worsen with age, it is not fully known if such age-related changes are influenced primarily by body composition and lifestyle or by other aspects of aging. We investigated the extent to which the fat and fat-free components of body mass index (BMI) and lifestyle factors influence patterns of change in lipids independent of age. Serial data were analyzed using sex-specific longitudinal models. These models use serial data from individuals to assume a general pattern of change over time, while allowing baseline age and the rate of change to vary among individuals. Serial data were obtained from 940 examinations of 269 healthy white participants (126 men, 143 women), aged 40–60 years, in the Fels Longitudinal Study. Measurements included age, the fat (FMI) and fat-free mass (FFMI) components of BMI, high-density lipoprotein (HDL-C), low-density lipoprotein (LDL-C), triglycerides (TG), total cholesterol (TC), fasting glucose and insulin, physical activity, alcohol use and smoking, and women's menopausal status and estrogen use. In both sexes, increased FMI was significantly associated with increased LDL-C, TG and TC, and decreased HDL-C. Increased FFMI was significantly related to decreased HDL-C and increased TG. Independent age effects remained significant only for LDL-C and TC in men and TC in women. Increased insulin was significantly related to increased TG in women. Moderate alcohol consumption was associated with higher HDL-C in men. Physical activity lowered male LDL-C and TC levels, and increased female HDL-C levels. Menopause was associated with increases in LDL-C. Premenopausal women not using estrogen had significantly lower HDL-C, TG, and TC than postmenopausal women taking estrogen. (1) Age is an important independent predictor for LDL-C and TC in men, and TC in women, but it is not as influential as body composition and lifestyle on HDL-C and TG in men and women, and LDL-C in women. (2) Increasing FMI is the major contributor to elevated TC, LDL-C and TG levels, and decreased HDL-C levels in men and women. (3) FFMI significantly influences HDL and TG levels in both sexes. (4) Maintaining a lower BMI via a reduced fat component may be more beneficial in lowering CVD risks than other factors.
