Hyper-alimentation - effects on health and well-being

Abstract: The general aim of this thesis was to prospectively examine the effects on health and well-being when healthy normal weight individuals increase their energy intake, mainly from fast food and simultaneously adopt a sedentary lifestyle.This thesis is based upon a prospective experimental study design where 18 healthy normal weight individuals, 12 men and 6 women, aged 26 (6.6) years, increased their energy intake with in average 70 % during four weeks. Simultaneously their physical activity was limited to a maximum of 5000 steps per day. An age and gender matched control group (n=18), was recruited and asked not to change their eatingand physical activity habits for four weeks. Long-term follow-up measurements were performed after 6 and 12 months and 2.5 years after the intervention.During the intervention body weight increased with 6.4 (2.8) kg and measurements of body composition showed an increase of both fat mass and fat free mass after the intervention. Lower physical and mental health scores on SF-36 as well as depressive symptoms were found compared to baseline. They were temporary and when followed up 6 and 12 months after the intervention, physical and mental health had returned to baseline values, despite a somewhat increased body weight. The main essence of adopting an obesity provoking behaviour was lack of energy emerging from five structures: influenced self-confidence, commitment to oneself and others, managing eating, feelings of tiredness and physical impact. Laboratory measurements showed an increase of ALT above reference limits in 14 of the 18 participants during the intervention and HTGC increased, although this was not related to the increase in ALT levels. Twelve months after the intervention an increase of body weight with 1.5 (2.4) kg was found compared to baseline (p=0.018), fat free mass was unchanged compared to baseline while fat mass had increased, + 1.4 (1.9) kg (p=0.01). Two and a half years after the intervention an increase of body weight with 3.1 (4.0) kg was found compared to baseline (p=0.01), while there was no change in controls compared to baseline, + 0.1 (2.5) kg (p=0.88).Hyper-alimentation and limited physical activity during a short-term period of 4 weeks is sufficient to temporarily induce worsened HRQoL, cause depressive symptoms and lack of energy in healthy normal weight individuals. There were also temporary but clear effects on biochemical markers, suggesting that hyperalimentation per se can induce profound ALT elevations in less than 4 weeks. During the intervention both fat mass and fat-free mass increased while after 12 months there was only an increase of fat mass which was greater than expected from epidemiological studies. The marked difference between the increase in body weight in the intervention- and control group at 2.5 years also raises the question whether there is a long-term effect of increasing fat mass after a short period of hyperalimentation.

Abstract: This paper describes an exploratory study aimed at understanding peoples perceptions and experiences of wellbeing. As health and wellbeing are considered by world and national health authorities to be closely related, occupational therapists along with other health workers need to know more about wellbeing, particularly from the perspective of ‘the people’.Using a questionnaire, seven cluster samples of 20 people, giving a total of 140 participants, were asked to define their concept of wellbeing, how it felt to them and how often they experienced the feeling. Data were also collected about nationality, marital status, employment, income, religion and levels of education because these have been identified as affecting wellbeing. In this study, wellbeing was described as happiness, peace, mind and body health and self-esteem, and was strongly associated with occupations as varied as work, relaxation and dressing up; relationships; and environments. Abstract: Purpose: Few epidemiological studies have been conducted among middle-aged women on long-term total and specific physical activity (PA) trends. We studied in a cross-sectional setting the relationship of self-reported total daily PA with age and calendar time. Methods: In a population-based cohort of 38,988 women aged 49-83 yr in central Sweden, information was collected on physical activity, such as work or occupation, household work, walking or bicycling, exercise, watching TV or reading, and other lifestyle factors through a self-administered questionnaire. Total and specific daily PA levels at ages 15, 30, and 50 yr were recalled retrospectively and measured as metabolic equivalents (MET[middle dot]h[middle dot]d-1). Results: Total PA level linearly decreased with calendar time in all three age groups (slope for 5-yr change in calendar time among those 15 yr of age = -0.82; 95% confidence interval (CI), -0.86 to -0.78; among those 30 yr of age=-0.42; 95% CI, -0.45 to -0.38; and among those 50 yr of age = -0.62; 95% CI, -0.66 to -0.58). High-intensity activities such as walking or bicycling decreased by 0.21 MET[middle dot]h[middle dot]d-1 (95% CI, -0.22 to -0.20) every 5-calendar-year change among adolescents between the 1930s and 1960s. Total activity level decreased in all age groups by an average of approximately 3MET[middle dot]h-1[middle dot]d-1, corresponding to approximately 45 min of brisk walking. Conclusions: Our results suggest that intervention efforts aimed at engaging in healthful amounts of physical activity are needed throughout the life cycle.

Abstract: From a total of 12 pairs of young male identical twins who were overfed by an estimated 84,000 kcal over a period of 100 days, several pairs (eight to 11, depending on variables) were remeasured for body weight, body composition with the underwater weighing technique, regional fat distribution from skinfolds, girths, computed tomography (CT) fat areas in the abdominal region, and fasting plasma glucose, insulin, total cholesterol, high-density lipoprotein (HDL) cholesterol, and triglycerides 4 months and 5 years after completion of the overfeeding protocol. At 4 months, the twins had lost approximately 7 of 8 kg that they had gained with overfeeding. However, 5 years later, body weight had increased by 5 kg over the preoverfeeding level. Fluctuations in fat mass were greater than those in fat-free mass. The younger twins gained approximately twice as much as the older twins in the late recovery period, a difference attributed to the late phase of growth in body mass in the former. Upper-body fat was reduced at 4 months of follow-up study, but was increased in the late recovery phase. All blood values were normalized in the postoverfeeding periods. A within-pair resemblance was generally observed for the changes noted in the recovery periods, but it was more striking when variations between preoverfeeding and 4-month or 5-year values were considered. We conclude from these observations that there were no persistent effects of exposure to the overfeeding protocol over the expected age-associated increases in body mass, body fat, upper-body fat, abdominal visceral fat (AVF), and metabolic variables predictive of risk for common diseases in individuals of normal body weight and with no family history of obesity. The intrapair resemblance suggests that the genotype contributes to the alterations observed in the recovery from overfeeding and in the age-associated changes.
