Hypoplasia of the aortic root The problem of aortic valve replacement

01 May 1970-Thorax (BMJ Group)-Vol. 25, Iss: 3, pp 339-346
TL;DR: No matter what type of valve is used for replacement of a diseased aortic valve, and no matter what improved designs of valvular prosthesis are ultimately developed, it will be necessary to enlarge the aorti ring to accommodate a size which will function correctly without causing left ventricular outflow obstruction.
Abstract: We report a technique for the enlargement of a hypoplastic aortic root by an operation whereby the hypoplastic aortic root has been so enlarged by the insertion of a Dacron fabric gusset that it will accommodate a size 9A or larger Starr-Edwards prosthesis. Our experience in five patients is described. No matter what type of valve is used for replacement of a diseased aortic valve, and no matter what improved designs of valvular prosthesis are ultimately developed, it will be necessary (in the particular group described) to enlarge the aortic ring to accommodate a size which will function correctly without causing left ventricular outflow obstruction.

TL;DR: This communication describes a new surgical procedure of enlarging the narrow aortic valve ring by extending theAortic incision through the fibrous origin of the aorti leaflet of the mitral valve into this leaflet, which permits the replacement of the anulus by a suitable prosthesis.

TL;DR: Risks for aortic valve replacement in octogenarians include female gender, unstable symptoms, poor ejection fraction, renal impairment, and bypass grafting, but the outlook for operative survivors is excellent, with good relief of symptoms and an expected survival normal for this particular age group.

TL;DR: This method was used in 4 children with tunnel-like subaortic stenosis, 3 of whom had had unsuccessful previous surgical attempts, and two patients died postoperatively, one from advanced myocardial damage and progressive failure and the other from cerebral ischemia caused by insufficient retrograde perfusion through an aortic coarctation that was not repaired earlier.
