Abstract: Autotransplantation of teeth, if carried out successfully, ensures that alveolar bone volume is maintained due to physiological stimulation of the periodontal ligament. Autotransplantation has been carried out for many years, but with varying success rates. As a result, it is seldom regarded as an appropriate treatment option for patients with missing teeth especially with the continued development of osseo-integrated implants. Since placement of osseo-integrated implants in growing alveolar bone is contra-indicated, transplantation of available teeth remains a suitable choice for replacing missing units in the young patient. The role of autotransplantation of teeth in several clinical situations, the clinical techniques involved and factors influencing success of the transplant are discussed. The aim of this paper is to underline the evidence based principles for successful autotransplantation and present the technique as a viable treatment option in present day orthodontic practice.

Abstract: Thermal injury to the periodontium via the root canal was produced in 12 squirrel monkeys in order to study the sequence of events leading to ankylosis Three days and one week following such injury an area of necrosis was found in the periodontal ligament which was circumferential, involved both periodontal ligament and alveolar hone, extended approximately l/3 the length of the root and did not involve the epithelial attachment Two and 3 weeks following thermal injury new fibroblasts and blood vessels were seen at the periphery of neerotic areas and progressively decreased the area of necrosis This was accompanied by frontal resorption of bone and cementum Rear resorption took place adjacent to areas where the periodontal ligament was still neerotic By one month, healing had progressed so that neerotic tissue was no longer present, bone apposition was more prominant than resorption and in some areas tooth and bone had fused Ankylosis persisted but a continuous remodeling process was taking place as evidenced by further resorption and bone formation until the end of the study (6 months)

Abstract: EXPERIMENTS by Charles B. Huggins and his associates, 1 and many others during the past thirty years, 2-11 demonstrate that whole toothbuds and parts of toothbuds possess the capacity to grow and develop in areas of the body other than the jawbone, even in the form of explants in tissue culture. The toothbud not only continues its characteristic development, but also exhibits the capacity to induce undifferentiated connective tissues to form bone. To explain these observations, Hoffman 12 postulates that the enamel organ and its derivative, Hertwig's epithelial root sheath, stimulates connective tissue cells to differentiate into bone cells, while Zussman 13 contends that odontoblasts which had previously participated in dentin formation can produce bone even when not in contact with oral tissues.
