The third universal definition of myocardial infarction The main feature of this new definition is the recognition of a rise and/or fall of cardiac biomarker values, with at least one of the values being raised. The favoured cardiac biomarker of myocardial injury is the highly sensitive and specific cardiac troponin (cTn). At least one of the following criteria should also be met: w Symptoms of ischaemia w New significant ST/T wave changes or left bundle-branch block w Appearance of pathological Q waves on an electrocardiogram (ECG) w Evidence of recent loss of viable myocardium or regional wall motion abnormality
