TLDR
Nine additional cases of primary tuberculosis and two positive reactors were identified among the total group of 38 day-care children and there is a need for a tuberculosis-control program in family day- care homes.
Abstract
Two children developed clinical primary tuberculosis in an unlicensed family day-care home. The husband of the proprietor of the day-care home was identified as an index patient. Contacts and associates of the index patient and the two children were tested with tuberculin skin test and chest x-ray films. The tuberculin-positivity rate was 60% in immediate family members of the index patient, and 28.9% in day-care children. Nine additional cases of primary tuberculosis and two positive reactors were identified among the total group of 38 day-care children. There is a need for a tuberculosis-control program in family day-care homes. ( JAMA 228:851-854, 1974)

Community-Based Outbreaks of Tuberculosis

TL;DR: It is concluded that community-based outbreaks of tuberculosis continue to occur and that well-established risks contribute to most outbreaks, and many outbreaks can be prevented or limited by attention to basic infection control principles.

Infection control challenges in child-care centers.

TL;DR: Children attending out-of-home care settings are at increased risk for a variety of infections and increased rates of infections result in increased morbidity of children attending child-care facilities and significant economic impact because of loss of work and cost of medical care.

Infectious Diseases and Day Care

TL;DR: Infectious diseases often occurring in epidemics, are to be expected in group day care because of the susceptibility of infants and toddlers to infection and the ready transmission of infection in the day care setting.

Tuberculosis transmission from healthcare workers to patients and co-workers: a systematic literature review and meta-analysis.

TL;DR: The risk of TB transmission from HCWs appears to be lower than that recorded in other settings or in the healthcare set when the index case is not a HCW, and more and better information is needed on the infectivity of the source cases, the actual exposure level of screened contacts, and the environmental characteristics of the healthcare setting.

TL;DR: Careful and persistent follow-up observation of tuberculin reactors is extremely important and may avert disaster and the drug susceptibility pattern of the source case should be reported to the chest clinic or health agency checking the contact cases.
