What is the rate of reccurence and progression in low-grade non-invasive papillary urothelial carcinoma?
4 answers
The rate of recurrence and progression in low-grade non-invasive papillary urothelial carcinoma (LG-UrCa) has been a subject of extensive study due to its implications for patient management and prognosis. According to a retrospective study conducted from January 2016 to December 2018, which included 46 patients initially diagnosed with LG-UrCa, the recurrence rate was found to be 30.4%. This study highlighted that patients with larger initial tumor sizes and those presenting with multiple lesions at diagnosis had a significantly higher rate of tumor recurrence. Specifically, the recurrence rate was significantly associated with the size of the initial tumor (p=0.009) and the presence of multiple lesions at initial diagnosis (p=0.02). Furthermore, the progression from LG-UrCa to a higher grade or stage was observed in a subset of patients. Out of the 46 patients, four (8.7%) experienced progression to a higher grade, and among these, two also showed TNM stage progression. These findings underscore the heterogeneity in the behavior of LG-UrCa, with a notable proportion of patients experiencing disease recurrence and a smaller fraction undergoing progression to more aggressive disease forms. The study by Dedy Suryadi et al. corroborates the high recurrence risk associated with LG-UrCa, noting that more than 50% of primary bladder neoplasms are low grade at diagnosis and have a significant risk of recurrence, although less than 15% of these cases progress to invasive disease. This aligns with the observed recurrence and progression rates in the previously mentioned study, emphasizing the clinical challenge in managing LG-UrCa due to its potential for recurrence and progression. In summary, LG-UrCa presents a considerable risk of recurrence, estimated at around 30.4%, with a smaller yet significant risk of progression to higher disease grades or stages. These findings highlight the importance of vigilant monitoring and management strategies for patients diagnosed with LG-UrCa to mitigate the risks of recurrence and progression.
