Abstract: Raised intraocular pressure is confirmed to be the principal complication following the use of sodium hyaluronate in cataract surgery and the value of the 'piping technique' limiting the use of sodium hyaluronate to the wound edges is demonstrated in reducing this hypertensive response. Difficulties with technique, differentiation between vitreous and sodium hyaluronate, damping effects and has dissipation fall into the category of problems rather than complications. A double cannula is described to remove as much sodium hyaluronate as possible at the end of surgery.

Abstract: Two clinical interferometers generated gratings directly on to the retina in young experienced observers and also in patients about to undergo cataract surgery. Eyes in the patients with no media opacities were used as controls. We agreed with the manufacturers' claims that gratings are seen on the retina independent of refractive state and that gratings can still be seen through most cataracts. However, we did not find that preoperative retinal visual acuity was of any value in predicting postoperative Snellen visual acuity. Reasons for this are discussed.

Abstract: • Although cataract surgery is feasible in children with unilateral cataracts, visual rehabilitation depends on optical correction and conscientiously maintained amblyopia therapy. Epikeratophakia for the correction of aphakia was performed in 47 children (50 grafts). Postoperative keratometry readings showed an average increase of 12.68 diopters in early patients. With new tissue-handling techniques, the last eight patients showed an average increase of +16.80 D, with −0.64-D overrefraction for emmetropia. Visual results indicate that epikeratophakia is an effective primary procedure for patients with unilateral traumatic cataracts and that it is superior to leaving vision uncorrected in children who have had unilateral congenital cataracts removed and are contact-lens intolerant. Further studies with younger patients will be necessary to define its role in neonates; however, individual results demonstrate that good vision is obtainable with epikeratophakia.

Abstract: The pupillary response to various doses of intraocular epinephrine (0.1 ml of 1:16,000, 1:32,000, 1:64,000, 1:80,000, or 1:96,000) was studied in 55 consecutive patients during extracapsular cataract surgery. The 1:16,000 epinephrine concentration provided a mean 0.74 mm increase in pupil diameter (range 0.0 to 1.7 mm) when administered to re-dilate the pupil after nucleus expression. The mean increase in pupil area with 1:16,000 epinephrine was 27% which greatly facilitated removal of lens cortex in most cases. However, 25% of all pupils failed to dilate with epinephrine 1:16,000. The other concentrations provided essentially the same mydriasis as the 1:16,000 concentration. Pupils smaller than 6 mm dilated more easily than pupils larger than 6 mm. Iris color, age, or sex had no significant effect on the mydriatic response. It is concluded that an extremely dilute concentration of epinephrine (i.e., 1:96,000 or less) may be effective in maintaining mydriasis during cataract surgery.

Abstract: In a study of 33 patients of heterochromic cyclitis, some unusual clinical features were observed. In many cases the iris was feltlike, month eaten, and associated with gelatinous nodules at the pupillary border or over the surface of iris stroma. An association of pseudoexfoliation of the lens capsule with bilateral heterochromic cyclitis was noted. Twenty-one eyes were subjected to cataract surgery. Hyphema (23.6%) and phacoanaphylactic uveitis (9.5%) were the main postoperative complications.

Abstract: This was the second in a series of studies intended to develop a method for predicting the degree of recovery and satisfaction of elderly patients following cataract surgery. Sixty-six patients (mean age, 68 years; 27 men and 39 women) completed an activity questionnaire and a series of walking, reaching, and hand-eye coordination tasks 1 day prior to cataract surgery. An average of 6 months later, the patients completed the activity questionnaire and some of the motor tasks again and then made a four-point assessment of satisfaction with their recovery. Postoperative reaching, walking, activity, and satisfaction were all positively correlated with scores on some of the preoperative tasks. Together with earlier published data, these data also show that patients report less postoperative activity after an eyeglass correction than after a lens-implant or contact-lens correction.

Abstract: The pathology of subacute granulomatous endophthalmitis centered on a posterior chamber lens implant placed at extracapsular cataract surgery in the eye of a 72 year-old diabetic with cystoid macular edema following intracapsular extraction and an anterior chamber implant in the other eye is described in detail 3 months after surgery. The features of the severe granulomatous inflammation do not fit the usual pattern of sympathetic or phacoanaphylactic uveitis. The possibility of a specific hypersensitivity to the substance of the implant is considered.

Abstract: The use of sodium hyaluronate in 383 cases of Cataract extraction with primary lens implantation has been to be effective, non-toxic and safe. Chief uses are to protect the corneal endothelium during nucleus extraction and to maintain the anterior chamber depth during lens implantation and during iris suturing. It is important not to use excessive amounts and to dilute with miochol or salt solution during wound closure. Using these techniques sodium hyaluronate was found to be free from side effects and when timolol maleate was used b.d. from the close of surgery the incidence of postoperative ocular hypertension was found to be no different from that in a control group. Endothelial cell studies showed a cell loss of 13 per cent to be within the range previously established for intracapsular cataract surgery without lens implantation, and support a widening conviction that lens implantation may now be regarded as a safe procedure.

Abstract: To determine the effect of retrobulbar lidocaine, with or without epinephrine, on intraocular pressure, 50 patients admitted to hospital for cataract surgery were selected at random. By applanation tonometry the tension of each eye was measured when the patient was placed on the operating table and 4 minutes after the retrobulbar injection. The decrease in intraocular pressure 4 minutes after the injection was not significant, clinically or statistically, and probably resulted from relaxation of the extraocular muscles.
