Abstract: Prosody assessment and treatment in dysarthria is clinically relevant, since prosodic impairment can have a negative impact on speech intelligibility and thus on participation in daily life conversation. We propose a speech-technology based software tool that provides automated numerical and visual feedback on two important aspects of prosody: speech rate and intonation. The tool includes speech rate and intonation algorithms, both specifically developed for the analysis of Dutch dysarthric speech. The tool enables speech-language pathologists to obtain objective measures of these prosodic aspects in a standardized and fast way, and enables dysarthric speakers to practise their prosodic skills intensively without the presence of a speech-language pathologist being required.

Abstract: The aim of this study is to report the case of a patient with Fahr's Disease in order to describe the main stomatognathic and vocal changes that can be found in individuals with this disease. In order to establish the diagnosis, an assessment of the conditions of orofacial motor system and speech production, as well the efficiency of swallowing, was realized. Based on these assessments, there were difficulties in coordinating and sustaining muscle during speech and presence of oropharyngeal dysphagia. Speech disorders found in Fahr's disease manifest themselves in complex and cover various aspects of phonological knowledge and the diseases that affect the basal ganglia have similar frames of speech-language disorders of the stomatognathic system, being able to present a picture of dysarthria.

Abstract: The Four-Level Framework of speech sensorimotor control (Van der Merwe, 1997) complicates the traditional view of dysarthria as a purely motor execution disorder. According to this framework, hypokinetic, hyperkinetic and ataxic dysarthria are programming-execution dysarthrias, while flaccid dysarthria is the only execution dysarthria. This preliminary study aimed to differentiate programming-execution dysarthria from execution dysarthria by examining variability of the temporal control of speech. Six participants and five control participants repeated 15 stimulus words ten times. Voice onset time, vowel duration, vowel steady state duration and vowel formant transition duration were measured acoustically. The coefficient of variation of the temporal parameters, and the correlation coefficient between the durational parameters, were calculated and analysed using descriptive statistics. The coefficient of variation revealed that the speakers with dysarthria were more variable than the control speakers. All participants, except those with flaccid dysarthria, showed similar patterns of intra-subject variability. Those with flaccid dysarthria exhibited greater intra-subject variability of voice onset time. The correlation analysis did not reveal differences between dysarthria type, or between the dysarthric speakers and the controls. Differences found in the patterns of variability may support the hypothesis that individuals with programming-execution dysarthria resort to a different level of control than those with execution dysarthria. Further research in this field is necessary.

Abstract: Purpose Early studies of perceptual learning of dysarthric speech, those summarized in Borrie, McAuliffe, and Liss (2012), yielded preliminary evidence that listeners could learn to better understand the speech of a person with dysarthria, revealing a potentially promising avenue for future intelligibility interventions. Since then, a programmatic body of research grounded in models of perceptual processing has unfolded. The current review provides an updated account of the state of the evidence in this area and offers direction for moving this work toward clinical implementation. Method The studies that have investigated perceptual learning of dysarthric speech (N = 24) are summarized and synthesized first according to the proposed learning source and then by highlighting the parameters that appear to mediate learning, culminating with additional learning outcomes. Results The recent literature has established strong empirical evidence of intelligibility improvements following familiarization with dysarthric speech and a theoretical account of the mechanisms that facilitate improved processing of the neurologically degraded acoustic signal. Conclusions There are no existing intelligibility interventions for individuals with dysarthria who cannot behaviorally modify their speech. However, there is now robust support for the development of an approach that shifts the weight of behavioral change from speaker to listener, exploiting perceptual learning to ease the intelligibility burden of dysarthria. To move this work from bench to bedside, recommendations for translational studies that establish best practices and candidacy for listener-targeted dysarthria remediation, perceptual training, are provided.

Abstract: Dysarthria, a disordered speech production resulting from neuro-muscular impairment, is a common symptom after stroke. It causes significant problems for patients’ speech intelligibility, communication, psychological well-being, social engagement and stroke recovery. Rehabilitation for dysarthria is variable in quality, intensity and duration, which may be, in part, due to the lack of good quality evidence. An online therapy programme, ReaDySpeech, has the potential to improve quality, intensity and duration of speech rehabilitation and was considered in a proof-of-concept study to be acceptable to speech and language therapists and patients which warranted further evaluation. The present study aims to examine the feasibility of running a trial using the ReaDySpeech intervention. A feasibility, randomised controlled trial, will recruit a minimum of 36 people with post-stroke dysarthria who are more than 1 week post stroke. Participants will be externally randomised in a 2:1 ratio to receive either ReaDySpeech and usual care (24 participants) or usual care only (12 participants). This study is single blind with the researcher carrying out the baseline and outcome measures while blinded to treatment allocation. The primary objective is to assess the feasibility of conducting a larger Phase III trial. The specific objectives are to determine the following: recruitment rate and reasons for non-recruitment; loss of participants to follow-up; acceptability of randomisation; adherence to the intervention; delivery of ReaDySpeech and content; acceptability of outcome measures; success of blinding strategies; defining ‘usual’ care; and the implications of the intervention for the patient/family/carer. This study will involve a regional, multi-centre, randomised controlled feasibility trial of a complex intervention in order to evaluate whether a Phase III randomised controlled trial is feasible.
