Abstract: Introduction: Children with dysarthria due to cerebral palsy (CP) can experience problems manipulating intensity, fundamental frequency and duration to signal sentence stress in an utterance. Pauses have been identified as a potential additional cue for stress marking, which could compensate for this deficit. Objective: This study aimed to determine whether children use pauses to signal stress placement, and whether this differs between typically-developing children and those with CP. Methods: Six children with CP and eight typically developing children produced utterances with stress on target words in two different positions. Pauses before and after the stressed target words were analyzed in terms of number, location and duration. Results: Results showed that both groups inserted pauses into their utterances. However, neither group used pause location or duration in a systematic manner to signal the position of stressed words. Conclusions: The results suggest that pausing was not used strategically by either group to signal sentence stress. Further research is necessary to explore the value of pausing as a cue to stress marking in general and as a potential compensatory strategy for speakers with dysarthria.

Abstract: Parkinson Disease (PD) is a neuromotor illness affecting general movements of different muscles, those implied in speech production being among them. The relevance of speech in monitoring illness progression has been documented in these last two decades. Most of the studies have concentrated in dysarthria and dysphonia induced by the syndrome. The present work is devoted to explore how PD affects the dynamic behavior of the speech neuromotor biomechanics (neuromechanics) involved in deficient articulation (dysarthria), in contrast to classical measurements based on static features as extreme and central vowel triangle positions. A statistical distribution of the kinematic velocity of the lower jaw and tongue is introduced, which presents interesting properties regarding pattern recognition and classification. This function may be used to establish distances between different articulation profiles in terms of information theory. Results show that these distances are correlated with a set of tests currently used by neurologists in PD progress evaluation, and could be used in elaborating new speech testing protocols.

Abstract: Purpose: Amyotrophic lateral sclerosis (ALS) is a progressive neurologic disease that frequently affects speech and swallowing. The purpose of this article is to emphasize the role of the speech-language pathologist (SLP) in management of the progressive speech and swallowing symptoms. Methods: A review of the neurologic degeneration and resulting symptoms of ALS are presented, including the cognitive, sensory and speech-language changes that accompany the progressive motor impairment. The important role of the SLP on a multidisciplinary team is described. Approaches to management of both swallowing and speech are discussed. Results and Conclusions: ALS is a progressive, degenerative neurologic disease. SLPs play a vital role in helping patients maintain effective communication and manage dysphagia symptoms.

Abstract: We reported the two cases with bulbar-onset ALS showing isolated agraphia without overt dementia and aphasia. Patient 1 was a 69-year-old man and patient 2 was an 81-year-old woman, and both were right-handed. Each patient developed dysarthria as an initial symptom followed by dysphagia, and neurological examinations showed atrophy and fasciculation of the tongue with upper and lower motor-neuron involvement of the extremities. These characteristic features with electromyographic evidence including widespread acute and chronic denervation led to a diagnosis of bulbar-onset ALS. Around 1 year after the onset of ALS, dysarthria was mild enough to allow oral communication enabling the determination that aphasia was absent with well preserved confrontation naming, repetition, reading and comprehension. The patients were polite without abnormal behavior or character change, and their general intelligence was also well preserved with excellent scores on the Mini Mental State Examination (30 and 27 points for patients 1 and 2, respectively) and Frontal Assessment Battery (16 points for each patient). However, spontaneous writing and dictation revealed abundant writing errors characterized by omission of kana letters and paragraphia of kana and kanji letters in both patients. Some syntactic errors were also observed in writing but in spoken language. A letter-number effect on writing errors was observed in patient 1. Copying of letters or words was intact and structure and orientation of written letters was well preserved, indicating the absence of constructional, apraxic or spatial feature of agraphia. Single photon emission computed tomography demonstrated reduced uptake in the bilateral frontotemporal lobes, predominantly in the left hemisphere, with less evident alternation in magnetic resonance imaging. Our results suggest that patients with bulbar-onset ALS may develop isolated agraphia as a single-domain cognitive impairment, preceding the clinical manifestation of aphasia or dementia. We speculate that the main responsible region might be the posterior part of the middle and inferior frontal gyri including Exner's writing center and Broca's area beyond the primary motor cortex.
