Abstract: Some of the debates reminded me about themes I had been taking for granted. For example, Svab and van Weels’ negative position on ‘Family Medicine should refine its essential attributes’ reminded me that family medicine is an emancipated field of medicine and no longer needed to debate this position (been there, done that). Furthermore, family medicine should get on with improving its image and be rid of ‘the harmful benefits of extreme complexity, uniqueness, exclusivity, and threat’.

Abstract: The Diagnostic and Statistical Manual of Mental Disorders (DSM), published by the American Psychiatric Association (APA), is used for clinical, research, and educational purposes. The focus of the DSM is on diagnostic codes for psychological disorders and on medical record keeping. The DSM is also used for communication of information to governmental agencies, private insurers, and the World Health Organization (WHO). The DSM is now in its fourth edition. In 1952, the first edition of the Diagnostic and Statistical Manual of Mental Disorders (DSM-I) was recognized as the manual of mental health categorization. It developed as a variant of the “Mental Disorders” section of the sixth edition of the World Health Organization's International Classification of Diseases (ICD). Abstract: After 20 years of development and research, dual diagnosis services for clients with severe mental illness are emerging as an evidence-based practice. Effective dual diagnosis programs combine mental health and substance abuse interventions that are tailored for the complex needs of clients with comorbid disorders. The authors describe the critical components of effective programs, which include a comprehensive, long-term, staged approach to recovery; assertive outreach; motivational interventions; provision of help to clients in acquiring skills and supports to manage both illnesses and to pursue functional goals; and cultural sensitivity and competence. Many state mental health systems are implementing dual diagnosis services, but high-quality services are rare. The authors provide an overview of the numerous barriers to implementation and describe implementation strategies to overcome the barriers. Abstract: The relation between overly positive self-evaluations and psychological adjustment was examined. Three studies, two based on longitudinal data and another on laboratory data, contrasted self-descriptions of personality with observer ratings (trained examiners or friends) to index self-enhancement. In the longitudinal studies, self-enhancement was associated with poor social skills and psychological maladjustment 5 years before and 5 years after the assessment of self-enhancement. In the laboratory study, individuals who exhibited a tendency to self-enhance displayed behaviors, independently judged, that seemed detrimental to positive social interaction. These results indicate there are negative short-term and long-term consequences for individuals who self-enhance and, contrary to some prior formulations, imply that accurate appraisals of self and of the social environment may be essential elements of mental health.
