Abstract: Background: This study presents estimates of lifetime and 12-month prevalence of 14 DSM-III-R psychiatric disorders from the National Comorbidity Survey, the first survey to administer a structured psychiatric interview to a national probability sample in the United States. Methods: The DSM-III-R psychiatric disorders among persons aged 15 to 54 years in the noninstitutionalized civilian population of the United States were assessed with data collected by lay interviewers using a revised version of the Composite International Diagnostic Interview. Results: Nearly 50% of respondents reported at least one lifetime disorder, and close to 30% reported at least one 12-month disorder. The most common disorders were major depressive episode, alcohol dependence, social phobia, and simple phobia. More than half of all lifetime disorders occurred in the 14% of the population who had a history of three or more comorbid disorders. These highly comorbid people also included the vast majority of people with severe disorders. Less than 40% of those with a lifetime disorder had ever received professional treatment, and less than 20% of those with a recent disorder had been in treatment during the past 12 months. Consistent with previous risk factor research, it was found that women had elevated rates of affective disorders and anxiety disorders, that men had elevated rates of substance use disorders and antisocial personality disorder, and that most disorders declined with age and with higher socioeconomic status. Conclusions: The prevalence of psychiatric disorders is greater than previously thought to be the case. Furthermore, this morbidity is more highly concentrated than previously recognized in roughly one sixth of the population who have a history of three or more comorbid disorders. This suggests that the causes and consequences of high comorbidity should be the focus of research attention. The majority of people with psychiatric disorders fail to obtain professional treatment. Even among people with a lifetime history of three or more comorbid disorders, the proportion who ever obtain specialty sector mental health treatment is less than 50%. These results argue for the importance of more outreach and more research on barriers to professional help-seeking.

Abstract: In trying to solve multiobjective optimization problems, many traditional methods scalarize the objective vector into a single objective. In those cases, the obtained solution is highly sensitive to the weight vector used in the scalarization process and demands that the user have knowledge about the underlying problem. Moreover, in solving multiobjective problems, designers may be interested in a set of Pareto-optimal points, instead of a single point. Since genetic algorithms (GAs) work with a population of points, it seems natural to use GAs in multiobjective optimization problems to capture a number of solutions simultaneously. Although a vector evaluated GA (VEGA) has been implemented by Schaffer and has been tried to solve a number of multiobjective problems, the algorithm seems to have bias toward some regions. In this paper, we investigate Goldberg's notion of nondominated sorting in GAs along with a niche and speciation method to find multiple Pareto-optimal points simultaneously. The proof-of-principle results obtained on three problems used by Schaffer and others suggest that the proposed method can be extended to higher dimensional and more difficult multiobjective problems. A number of suggestions for extension and application of the algorithm are also discussed.

Abstract: The widespread use of standardized health surveys is predicated on the largely untested assumption that scales constructed from those surveys will satisfy minimum psychometric requirements across diverse population groups. Data from the Medical Outcomes Study (MOS) were used to evaluate data completeness and quality, test scaling assumptions, and estimate internal-consistency reliability for the eight scales constructed from the MOS SF-36 Health Survey. Analyses were conducted among 3,445 patients and were replicated across 24 subgroups differing in sociodemographic characteristics, diagnosis, and disease severity. For each scale, item-completion rates were high across all groups (88% to 95%), but tended to be somewhat lower among the elderly, those with less than a high school education, and those in poverty. On average, surveys were complete enough to compute scales scores for more than 96% of the sample. Across patient groups, all scales passed tests for item-internal consistency (97% passed) and item-discriminant validity (92% passed). Reliability coefficients ranged from a low of 0.65 to a high of 0.94 across scales (median = 0.85) and varied somewhat across patient subgroups. Floor effects were negligible except for the two role disability scales. Noteworthy ceiling effects were observed for both role disability scales and the social functioning scale. These findings support the use of the SF-36 survey across the diverse populations studied and identify population groups in which use of standardized health status measures may or may not be problematic.

Abstract: The collaboration between the Forensic Community and the scholars of human population genetics, among whom I place myself, has always been fruitful and of reciprocal benefit in Italy as much as in Europe and in North-America, in the latter with a more dialectical attitude as shown by recent rather hot debates. DNA analysis is today offering new possibilities of collaboration. Case work and search for reference populations complement in the daily activity of the forensic scholars. Substantial DNA databases have already been established for a number of population groups, but the development of new standards and new reference databases is likely in the immediate future following the implementation of PCR-based DNA typing systems, and a strong argument can be made for population geneticists to share protocols and markers with the forensic community in order to type well-defined reference populations and from them to contribute to the analysis of human genetic diversity. My talk, however, does not address to the future, but rather to the past: I have had the chance to analyze, over the past ten years, many genetic data from human populations and I am going to give a short review of our recent analyses. Our main interest lies in their interpretation in terms of prehistory and history of our species: a more comprehensive treatment will appear in a forthcoming book written in collaboration with Cavalli-Sforza and Menozzi [1].

Abstract: Many, if not most, optimization problems have multiple objectives. Historically, multiple objectives have been combined ad hoc to form a scalar objective function, usually through a linear combination (weighted sum) of the multiple attributes, or by turning objectives into constraints. The genetic algorithm (GA), however, is readily modified to deal with multiple objectives by incorporating the concept of Pareto domination in its selection operator, and applying a niching pressure to spread its population out along the Pareto optimal tradeoff surface. We introduce the Niched Pareto GA as an algorithm for finding the Pareto optimal set. We demonstrate its ability to find and maintain a diverse "Pareto optimal population" on two artificial problems and an open problem in hydrosystems. >

Abstract: In this paper we describe an efficient approach for multimodal function optimization using genetic algorithms (GAs). We recommend the use of adaptive probabilities of crossover and mutation to realize the twin goals of maintaining diversity in the population and sustaining the, convergence capacity of the GA. In the adaptive genetic algorithm (AGA), the probabilities of crossover and mutation, p/sub c/ and p/sub m/, are varied depending on the fitness values of the solutions. High-fitness solutions are 'protected', while solutions with subaverage fitnesses are totally disrupted. By using adaptively varying p/sub c/ and p/sub ,/ we also provide a solution to the problem of deciding the optimal values of p/sub c/ and p/sub m/, i.e., p/sub c/ and p/sub m/ need not be specified at all. The AGA is compared with previous approaches for adapting operator probabilities in genetic algorithms. The Schema theorem is derived for the AGA, and the working of the AGA is analyzed. We compare the performance of the AGA with that of the standard GA (SGA) in optimizing several nontrivial multimodal functions with varying degrees of complexity. >

Abstract: During the development of the mammalian brain, neuronal precursors migrate to their final destination from their site of birth in the ventricular and subventricular zones (VZ and SVZ, respectively). SVZ cells in the walls of the lateral ventricle continue to proliferate in the brain of adult mice and can generate neurons in vitro, but their fate in vivo is unknown. Here SVZ cells from adult mice that carry a neuronal-specific transgene were grafted into the brain of adult recipients. In addition, the fate of endogenous SVZ cells was examined by microinjection of tritiated thymidine or a vital dye that labeled a discrete population of SVZ cells. Grafted and endogenous SVZ cells in the lateral ventricle of adult mice migrate long distances and differentiate into neurons in the olfactory bulb.

Abstract: BACKGROUNDThe WHO MONICA Project is a 10-year study that monitors deaths due to coronary heart disease (CHD), acute myocardial infarction, coronary care, and risk factors in men and women aged 35 to 64 years in defined communities. This analysis of methods and results of coronary event registration in 1985 through 1987 provides data on the relation between CHD morbidity and mortality.METHODS AND RESULTSFatal and nonfatal coronary events were monitored through population-based registers. Hospital cases were found by pursuing admissions ("hot pursuit") or by retrospective analysis of discharges ("cold pursuit"). Availability of diagnostic data on identified nonfatal myocardial infarction was good. Information on fatal events (deaths occurring within 28 days) was limited and constrained in some populations by problems with access to sources such as death certificates. Abstract: Conservation biology has two threads: the small-population paradigm which deals with the erect of smallness on the persistence of a population, and the declining-population paradigm which deals with the cause of smallness and its cure. The processes relevant to the small-population paradigm are amenable to theoretical examination because they generalize across species and are subsumed by an inclusive higher category: stochasticity. In contrast, the processes relevant to the declining-population paradigm are essentially humdrum, being not one but many. So far they have defied tight generalization and hence are of scant theoretical interest. Abstract: Recent advances in the application of the polymerase chain reaction make it possible to score individuals at a large number of loci The RAPD (random amplified polymorphic DNA) method is one such technique that has attracted widespread interest The analysis of population structure with RAPD data is hampered by the lack of complete genotypic information resulting from dominance, since this enhances the sampling variance associated with single loci as well as induces bias in parameter estimation We present estimators for several population-genetic parameters (gene and genotype frequencies, within- and between-population heterozygosities, degree of inbreeding and population subdivision, and degree of individual relatedness) along with expressions for their sampling variances Although completely unbiased estimators do not appear to be possible with RAPDs, several steps are suggested that will insure that the bias in parameter estimates is negligible To achieve the same degree of statistical power, on the order of 2 to 10 times more individuals need to be sampled per locus when dominant markers are relied upon, as compared to codominant (RFLP, isozyme) markers Moreover, to avoid bias in parameter estimation, the marker alleles for most of these loci should be in relatively low frequency Due to the need for pruning loci with low-frequency null alleles, more loci also need to be sampled with RAPDs than with more conventional markers, and some problems of bias cannot be completely eliminated

Abstract: One of the ecological tenets justifying conservation of biodiversity is that diversity begets stability. Impacts of biodiversity on population dynamics and ecosystem functioning have long been debated1–7, however, with many theoretical explorations2–6,8–11 but few field studies12–15. Here we describe a long-term study of grasslands16,17 which shows that primary productivity in more diverse plant communities is more resistant to, and recovers more fully from, a major drought. The curvilinear relationship we observe suggests that each additional species lost from our grasslands had a progressively greater impact on drought resistance. Our results support the diversity—stability hypothesis5,6,18,19, but not the alternative hypothesis that most species are functionally redundant19–21. This study implies that the preservation of biodiversity is essential for the maintenance of stable productivity in ecosystems.

Abstract: A mismatch distribution is a tabulation of the number of pairwise differences among all DNA sequences in a sample. In a population that has been stationary for a long time these distributions from nonrecombinant DNA sequences become ragged and erratic, whereas a population that has been growing generates mismatch distributions that are smooth and have a peak. The position of the peak reflects the time of the population growth. The signature of an ancient population expansion is apparent even in the low-resolution mtDNA typings described by Merriwether et al. (1991). The smoothness of the mismatch distribution, an indicator of population expansion, is hardly affected by population structure, whereas mean sequence divergence increases in a pooled sample from highly isolated subpopulations.

Abstract: While ecologists involved in management or policy often are advised to learn to deal with uncertainty, there are a number of components of global environmental change of which we are certain–certain that they are going on, and certain that they are human—caused. Some of these are largely ecological changes, and all have important ecological consequences. Three of the well—documented global changes are: increasing concentrations of carbon dioxide in the atmosphere; alterations in the biogeochemistry of the global nitrogen cycle; and ongoing land use/land cover change. Human activity–now primarily fossil fuel combustion– has increased carbon dioxide concentrations from °280 to 355 mL/L since 1800; the increase is unique, at least in the past 160 000 yr, and several lines of evidence demonstrate unequivocally that it is human—caused. This increase is likely to have climatic consequences–and certainly it has direct effects on biota in all Earth's terrestrial ecosystems. The global nitrogen cycle has been altered by human activity to such an extent that more nitrogen is fixed annually by humanity (primarily for nitrogen fertilizer, also by legume crops and as a by product of fossil fuel combustion) than by all natural pathways combined. This added nitrogen alters the chemistry of the atmosphere and of aquatic ecosystems, contributes to eutrophiction of the biosphere, and has substantial regional effects on biological diversity in the most affected areas. Finally, human land use/land cover change has transformed one—their to one—half of Earth's ice—free surface. This in and of itself probably represents the most important component of global change now and will for some decades to come; it has profound effects on biological diversity on land and on ecosystems downwind and downstream of affected areas. Overall, any clear dichotomy between pristine ecosystems and human—altered areas that may have existed in the past has vanished, and ecological research should account for this reality. These three and other equally certain components of global environmental change are the primary causes of anticipated changes in climate, and of ongoing losses of biological diversity. They are caused in turn by the extraordinary growth in size and resource use of the human population. On a broad scale, there is little uncertainty about any of these components of change or their causes. However, much of the public believes the causes–even the existence–of global change to be uncertain and contentious topics. By speaking out effectively, we can help to shift the focus of public discussion towards what can and should be done about global environmental change.

Abstract: Objective. —To quantify the efficacy of BCG vaccine against tuberculosis (TB). Data Sources. —MEDLINE with index termsBCG vaccine, tuberculosis, andhuman. Experts from the Centers for Disease Control and Prevention and the World Health Organization, among others, provided lists of all known studies. Study Selection. —A total of 1264 articles or abstracts were reviewed for details on BCG vaccination, concurrent vaccinated and unvaccinated groups, and TB outcome; 70 articles were reviewed in depth for method of vaccine allocation used to create comparable groups, equal surveillance and follow-up for recipient and concurrent control groups, and outcome measures of TB cases and/or deaths. Fourteen prospective trials and 12 case-control studies were included in the analysis. Data Extraction. —We recorded study design, age range of study population, number of patients enrolled, efficacy of vaccine, and items to assess the potential for bias in study design and diagnosis. At least two readers independently extracted data and evaluated validity. Data Synthesis. —The relative risk (RR) or odds ratio (OR) of TB provided the measure of vaccine efficacy that we analyzed. The protective effect was then computed by 1 —RR or 1 —OR. A random-effects model estimated a weighted average RR or OR from those provided by the trials or case-control studies. In the trials, the RR of TB was 0.49 (95% confidence interval [Cl], 0.34 to 0.70) for vaccine recipients compared with nonrecipients (protective effect of 51%). In the case-control studies, the OR for TB was 0.50 (95% CI, 0.39 to 0.64), or a 50% protective effect. Seven trials reporting tuberculous deaths showed a protective effect from BCG vaccine of 71% (RR, 0.29; 95% Cl, 0.16 to 0.53), and five studies reporting on meningitis showed a protective effect from BCG vaccine of 64% (OR, 0.36; 95% Cl, 0.18 to 0.70). Geographic latitude of the study site and study validity score explained 66% of the heterogeneity among trials in a random-effects regression model. Conclusion. —On average, BCG vaccine significantly reduces the risk of TB by 50%. Protection is observed across many populations, study designs, and forms of TB. Age at vaccination did not enhance predictiveness of BCG efficacy. Protection against tuberculous death, meningitis, and disseminated disease is higher than for total TB cases, although this result may reflect reduced error in disease classification rather than greater BCG efficacy. Abstract: Systemic lupus erythematosus is a multisystem autoimmune disease in which the autoantibody response targets a variety of autoantigens of diverse subcellular location. We show here that these autoantigens are clustered in two distinct populations of blebs at the surface of apoptotic cells. The population of smaller blebs contains fragmented endoplasmic reticulum (ER) and ribosomes, as well as the ribonucleoprotein, Ro. The larger blebs (apoptotic bodies) contain nucleosomal DNA, Ro, La, and the small nuclear ribonucleoproteins. These autoantigen clusters have in common their proximity to the ER and nuclear membranes, sites of increased generation of reactive oxygen species in apoptotic cells. Oxidative modification at these sites may be a mechanism that unites this diverse group of molecules together as autoantigens.

Abstract: The emergence of multidrug-resistant bacteria is a phenomenon of concern to the clinician and the pharmaceutical industry, as it is the major cause of failure in the treatment of infectious diseases. The most common mechanism of resistance in pathogenic bacteria to antibiotics of the aminoglycoside, beta-lactam (penicillins and cephalosporins), and chloramphenicol types involves the enzymic inactivation of the antibiotic by hydrolysis or by formation of inactive derivatives. Such resistance determinants most probably were acquired by pathogenic bacteria from a pool of resistance genes in other microbial genera, including antibiotic-producing organisms. The resistance gene sequences were subsequently integrated by site-specific recombination into several classes of naturally occurring gene expression cassettes (typically "integrons") and disseminated within the microbial population by a variety of gene transfer mechanisms. Although bacterial conjugation once was believed to be restricted in host range, it now appears that this mechanism of transfer permits genetic exchange between many different bacterial genera in nature.

Abstract: Subarachnoid hemorrhage (SAH) is a common and frequently devastating condition, accounting for ≈5% of all strokes and affecting as many as 30 000 Americans each year.1,2 The American Heart Association (AHA) previously published “Guidelines for the Management of Aneurysmal Subarachnoid Hemorrhage.”3 Since then, considerable advances have been made in endovascular techniques, diagnostic methods, and surgical and perioperative management paradigms. Nevertheless, outcome for patients with SAH remains poor, with population-based mortality rates as high as 45% and significant morbidity among survivors.4–9 Several multicenter, prospective, randomized trials and prospective cohort analyses have influenced treatment protocols for SAH. However, rapid evolution of newer treatment modalities, as well as other practical and ethical considerations, has meant that rigorous clinical scientific assessment of the treatment protocols has not been feasible in several important areas. To address these issues, the Stroke Council of the AHA formed a writing group to reevaluate the recommendations for management of aneurysmal SAH. A consensus committee reviewed existing data in this field and prepared the recommendations in 1994.3 In an effort to update those recommendations, a systematic literature review was conducted based on a search of MEDLINE to identify all relevant randomized clinical trials published between June 30, 1994, and November 1, 2006 (search terms: subarachnoid hemorrhage , cerebral aneurysm , trial ; Table 1). Each identified article was reviewed by at least 2 members of the writing group. Selected articles had to meet one of the following criteria to be included: randomized trial or nonrandomized concurrent cohort study. Case series and nonrandomized historical cohort studies were reviewed if no studies with a higher level of evidence were available for a particular topic covered in the initial guidelines. Abstract: Natural evolution is a population-based optimization process. Simulating this process on a computer results in stochastic optimization techniques that can often outperform classical methods of optimization when applied to difficult real-world problems. There are currently three main avenues of research in simulated evolution: genetic algorithms, evolution strategies, and evolutionary programming. Each method emphasizes a different facet of natural evolution. Genetic algorithms stress chromosomal operators. Evolution strategies emphasize behavioral changes at the level of the individual. Evolutionary programming stresses behavioral change at the level of the species. The development of each of these procedures over the past 35 years is described. Some recent efforts in these areas are reviewed. >

Abstract: Mutational processes of simple sequence repeats (SSRs) in complex genomes are poorly understood. We examined these processes by introducing a two-phase mutation model. In this model, most mutations are single-step changes, but infrequent large jumps in repeat number also occur. We used computer simulations to determine expected values of statistics that reflect frequency distributions of allele size for the two-phase model and two alternatives, the one-step and geometric models. The theoretical expectations for each model were tested by comparison with observed values for 10 SSR loci genotyped in the Sardinian population, whose genetic and demographic histories have been previously reconstructed. The two-phase model provided the best fit to the data for most of these loci in this population. In the analysis we assumed that the loci were neutral and that this population had undergone rapid population growth. Recent observations made for unstable trinucleotide repeats support our suggestion that frequent small changes and rare large changes in repeat number represent two distinct classes of mutation at SSR loci. We genotyped the same 10 loci in Egyptian and sub-Saharan African samples to assess the utility of SSRs for studying the divergence of populations and found that estimates of interpopulation distances from SSRs were similar to those derived from analysis of mitochondrial DNA.

Abstract: Special credit institutions in Bangladesh have dramatically increased the credit available to poor rural women since the mid-1980s. Though this is intended to contribute to women's empowerment, few evaluations of loan use investigate whether women actually control this credit. Most often, women's continued high demand for loans and their manifestly high propensity to repay is taken as a proxy indicator for control and empowerment. This paper challenges this assumption by exploring variations in the degree to which women borrowers control their loans directly; reporting on recent research which finds a significant proportion of women's loans to be controlled by male relatives. The paper finds that a preoccupation with “credit performance” — measured primarily in terms of high repayment rates — affects the incentives of fieldworkers dispensing and recovering credit, in ways which may outweigh concerns to ensure that women develop meaningful control over their investment activities.

Abstract: In the adult male, a population of diploid stem-cell spermatogonia continuously undergoes self-renewal and produces progeny cells, which initiate the complex process of cellular differentiation that results in mature spermatozoa. We report here that stem cells isolated from testes of donor male mice will repopulate sterile testes when injected into seminiferous tubules. Donor cell spermatogenesis in recipient testes showed normal morpholigical characteristics and produced mature spermatozoa. This methodology, besides opening new avenues of basic research into spermatogenesis and stem-cell self-renewal, may prove useful as a tool for biomedical science and biotechnology.

Abstract: This paper analyzes the convergence properties of the canonical genetic algorithm (CGA) with mutation, crossover and proportional reproduction applied to static optimization problems. It is proved by means of homogeneous finite Markov chain analysis that a CGA will never converge to the global optimum regardless of the initialization, crossover, operator and objective function. But variants of CGA's that always maintain the best solution in the population, either before or after selection, are shown to converge to the global optimum due to the irreducibility property of the underlying original nonconvergent CGA. These results are discussed with respect to the schema theorem. >

Abstract: Most major population surveys used by social scientists are based on complex sampling designs where sampling units have different probabilities of being selected. Although sampling weights must generally be used to derive unbiased estimates of univariate population characteristics, the decision about their use in regression analysis is more complicated. Where sampling weights are solely a function of independent variables included in the model, unweighted OLS estimates are preferred because they are unbiased, consistent, and have smaller standard errors than weighted OLS estimates. Where sampling weights are a function of the dependent variable (and thus of the error term), we recommend first attempting to respecify the model so that they are solely a function of the independent variables. If this can be accomplished, then unweighted OLS is again preferred. If the model cannot be respecified, then estimation of the model using sampling weights may be appropriate. Abstract: Seismicity is modeled as a sequence of earthquake nucleation events in which the distribution of initial conditions over the population of nucleation sources and stress- ing history control the timing of earthquakes. The model is implemented using solutions for nucleation of unstable fault slip on faults with experimentally derived rate- and state- dependent fault properties. This yields a general state-variable constitutive formulation for rate of earthquake production resulting from an applied stressing history. To illustrate and test the model some characteristics of seismicity following a stress step have been explored. It is proposed that various features of earthquake clustering arise from sensi- tivity of nucleation times to the stress changes induced by prior earthquakes. The model gives the characteristic Omori aftershock decay law and interprets aftershock parameters in terms of stress change and stressing rate. Earthquake data appear to support a model prediction that aftershock duration, defined as the time for rates to return to the back- ground seismicity rate, is proportional to mainshock recurrence time. Observed spatial and temporal clustering of earthquake pairs arises as a consequence of the spatial depen- dence of stress changes of the first event of the pair and stress-sensitive time-dependent nucleation. Applications of the constitutive formulation are not restricted to the simple stress step models investigated here. It may be applied to stressing histories of arbitrary complexity. The apparent success at modeling clustering phenomena suggests the possibil- ity of using the formulation to estimate short- to intermediate-term earthquake probabil- ities following occurrence of other earthquakes and for inversion of temporal variations of earthquake rates for changes in driving stress.
