Abstract: Aim The aim of the present study was to investigate to what extent patients using prescription antiobesity drugs (orlistat, sibutramine and rimonabant) concomitantly or concurrently used psychotropic drugs and analgesics and the association between this drug use and the patients' gender and age. An additional aim was to investigate the sequence of drug therapy among users of both antiobesity drugs and antidepressants or antipsychotics, respectively. Methods Data were retrieved from the Norwegian Prescription Database (NorPD). All patients who had an antiobesity drug (ATC code A08A) dispensed from a Norwegian pharmacy between January 2004 and December 2007 were included in the study. Results One in four patients using antiobesity drugs had at least on one occasion used a psychotropic drug concomitantly. The most commonly used psychotropic drugs were anxiolytics/hypnotics/sedatives (17.7%) and antidepressants (14.7%). Analgesics were used by 36.2%. A significantly higher percentage of women used anxiolytics/hypnotics/sedatives (18.8% vs. 14.0%, p < 0.0005), antidepressants (16.1% vs. 9.5%, p < 0.0005), antipsychotics (4.0% vs. 2.9%, p < 0.0005) and analgesics (37.8% vs. 30.5%, p < 0.0005) concomitantly with antiobesity drugs when compared to men. One out of ten patients using sibutramine had at least on one occasion used an interacting drug concomitantly. Conclusion Use of psychotropic drugs and analgesics among patients using antiobesity drugs is extensive, especially among women. Clinicians prescribing sibutramine should be more aware of drug interactions with other prescribed drugs. There is still insufficient information on psychiatric disorders among these patients. Abstract: Aims and methodTo increase the proportion of patients with no psychotropic drug discrepancies at the community mental health team (CMHT)–general practice interface. Three CMHTs participated. Over a 14 month period, quality improvement methodologies were used: individual patient-level feedback to patient's prescribers, run charts and meetings with CMHTs.ResultsOne CMHT improved medicines reconciliation accuracy and demonstrated significant reductions in prescribing discrepancies. One in three (119/356) patients had ≥1 discrepancy involving 20% (166/847) of all prescribed psychotropics. Discrepancies were graded as: ‘fatal’ (0%), ‘serious’ (17%) and ‘negligible/minor harm’ (83%) but were associated with extra avoidable prescribing costs. For medicines routinely supplied by secondary care, 68% were not recorded in general practice electronic prescribing systems.Clinical implicationsImprovements in medicines reconciliation accuracy were achieved for one CMHT. This may have been partly owing to a multidisciplinary team approach to sharing and addressing prescribing discrepancies. Improving prescribing accuracy may help to reduce avoidable drug-related harms to patients.

Abstract: The purpose of this brief work is to study and make a general survey of the problem of obesity as related in whatever way, to the use of psychotropic drugs. It is a fact learned through clinical experience and knowledge by clinics and researchers working in the field of psychotropic drugs that patients subjected to extended therapy with such drugs show, in a considerable number of cases, an increase in body weight. We consider that this obesity, the pathogenetic cause of which is attributed to the action of psychotropic drugs, constitutes a serious disadvantageous side-effect of those drugs, since a large number of mental patients discontinue their drug therapy for the reasons that it is responsible for the increase in their body weight. It is a known fact discontinuation of the psychotropic drug treatment generally results in a relapse of their mental disorder.

Abstract: Objective: The association between abnormal pupil reactivity (abnormal) and the outcome among patients with psychotropic drug overdose (OD) was retrospectively investigated. Methods: The study included patients that had experienced an OD between January and December 2007. The subjects were divided into 2 groups, namely, abnormal and normal groups. Results: There were 12 subjects in the abnormal and 74 subjects in the normal group. Glasgow Coma Scale in the abnormal was significantly smaller that that in the normal group. An average quantity of ingested tranquilizer per subject in the abnormal was significantly larger that those in the normal group. However, the duration of admission and survival rates between the two groups were not significantly different. Conclusion: The patients that experienced an OD, who demonstrated abnormal pupil reactivity, tended to have ingested larger amounts of drugs while also demonstrating severe unconsciousness. However, the patients with abnormal pupil reactivity had a favorable outcome.
