Abstract: A carriage assembly for controlling a steering wire steering mechanism within a flexible shaft, for use in gastrointestinal tract surgery, comprises a plurality of selectably engageable motors, including two drive motors for selectably rotating drive shafts of a surgical attachement, two steering motors mechanically communicating with corresponding pulleys around which are coiled directional steering wires communicating with a flexible shaft to enable the selective direction of the flexible shaft within a spatial plane, wherein within the assembly housing the steering motors travel on a steering motor carriage and the pulleys travel on a pulley carriage which is biased by a spring means away from the interior wall of the housing, which biases the steering wires toward a taut state. The bias can be overcome by a carriage motor mechanically communicating with the pulley carriage to cause the flexible shaft to go limp. Also disclosed are a series of surgical attachments which may be coupled to and utilized in conjunction with the carriage assembly.

Abstract: In a manipulator, an actuator block contains motors and rotary shafts extending from the motors. A working unit of the manipulator contains a connecting portion, attachable to and detachable from the actuator block. The connecting portion has pulleys connectable to ends of the rotary shafts, and has a locking plate movable by alignment pins. The locking plate has slits, which are engaged with plate-shaped portions formed on the upper ends of the pulleys. A coil spring is placed between the locking plate and a top plate.

Abstract: A retraction mechanism allows the full distal retraction of the firing drive from various positions. The retraction mechanism has been developed for use with a hand held surgical apparatus. An embodiment of the retraction mechanism includes a drive member, a first pulley, a second pulley, and a firing drive. The drive mechanism is configured to be driven by a motor. The first pulley is configured to rotate when the motor drives the drive mechanism. The second pulley is movable relative to the first pulley between a proximal position and a distal position. The firing drive is movable relative to the first pulley between proximal and distal positions. In operation, the first pulley rotates in a first direction in response to a distal translation of the firing drive and wherein the firing drive moves proximally upon rotation of the first pulley in a second direction.

Abstract: An improved surgical stapling instrument is provided for applying surgical staples to human tissue. The instrument has an articulated stapling head assembly mounted by a rotatable and flexible support shaft on an actuator handle assembly with an improved actuator mechanism for closing and firing the stapling head assembly. The stapling head assembly comprises a fixed jaw with a staple cartridge and a movable jaw with an anvil for clamping the tissue therebetween. The actuator mechanism comprises a jaw closure lever for closing the jaws and a staple firing lever for actuating a staple driver to drive the staples from the staple cartridge into the tissue and against the anvil. The actuator mechanism also includes a cam pulley mounted for rotation by the staple firing lever and secured to a firing cable for actuating the staple driver. A set of drive gears rotatably positioned on opposite sides of the cam pulley mesh with gear sectors on the staple firing lever to rotate the cam pulley and apply tension to the firing cable when the staple firing lever is actuated. The cam pulley has a contoured cam lobe for actuating the firing cable with different mechanical advantages as the pulley is rotated. An anti-backup member engages the cam pulley to prevent the pay-out of the firing cable before and after the staple firing lever is actuated. The length of the support shaft can be adjusted to set the tension in the closure cable so that the jaws are completely closed by the jaw closure lever to produce staples of uniform height when the staple firing lever is actuated.

Abstract: A surgical stapling apparatus comprises a handle assembly, an actuation shaft having a connector, and a retraction mechanism positioned within the handle assembly. The retraction mechanism includes a motor coupled to a transmission shaft, a first gear rotatably mounted on the transmission shaft, the first gear configured to engage a second gear, a clutch operatively attached to the second gear, a main shaft rotatably connected to the clutch, and a pulley coupled to the main shaft, the pulley being operatively attached to the connector. The connector and the pulley may be attached via a cable. The retraction mechanism may include a spring motor for maintaining the cable in tension. The spring motor may have a spring element wrapped around and interconnecting at least two arbors.
