Abstract: Our observations of 14 women with vesico-uterine fistulas as a result of caesarean section complications were presented. The patients were treated between 1965-1999 in urology clinic in Lublin. The case were diagnosed on the basis of medical history radiological investigations and cystoscopy. In 3 cases hysterectomy was performed in the rest of cases uterus was preserved and after the first operation complete recovery was observed. (authors)

Abstract: CASE PRESENTATION 26 y.o woman 3 weeks after cesarean section complained of urine leakage from vagina. Urography revealed uretero-uterine fistula. Surgery was performed--3 dexon sutures attaching strictured ureter to the uterine were localized and then cut off. Distally obliterated ureter was separated out of adjacent tissues. The strictured part of ureter was then incised on the length of ca 8 cm, pig tail catheter was induced and human dura mater allograft was put on and sewed in into the strictured segment. In the long term follow-up the woman gave birth to two more children in the vaginal delivery without any complications. Urographies revealed no fistula, no stricture recurrency and normal urine passage.

Abstract: A 26-year-old woman, gravida 4, abortions 4 presented with greenish discharge per vaginum occurring 2 days after a spontaneous incomplete abortion at 14 weeks gestation. Speculum examination revealed severe vaginitis and bilious discharge from the external os. Digital rectal examination was normal. She had undergone termination of pregnancy in her first pregnancy, which was followed by foul-smelling, semisolid greenish vaginal discharge, abdominal pain, distension, absolute constipation and fever that were managed conservatively elsewhere. Her second and third pregnancies terminated in spontaneous second trimester abortions, and were also followed by similar greenish discharge per vaginum for a few days. Plain X-rays of the abdomen were unremarkable. Abdominal and trans-vaginal ultrasonography revealed a gas shadow in the uterine fundus, small intestinal loops adherent to the uterus and minimal fluid in the Pouch of Douglas. The charcoal challenge test was positive 2 h after oral administration of activated charcoal. Retrograde methods to demonstrate a fistulous tract were deferred because of a low probability of demonstrating the fistula, minimisation of preoperative delay, and to avoid ascending infection. Laparotomy revealed oedematous, inflamed jejunal loops adherent to the uterus which, when meticulously separated, exposed a 0.5 × 0.5 cm fistulous communication between the uterine fundus and an adherent jejunal loop (Fig. 1). After thorough irrigation of the uterine cavity with a 5% solution of povidone iodine, the rent in the uterus was closed in two layers with 2/0 Vicryl (Ethicon, Johnson and Johnson Co., NJ, USA). Resection anastomosis of the affected segment of jejunum was followed by thorough peritoneal lavage and insertion of a pelvic drain that was removed on the fifth day. Postoperative recovery was uneventful. Histopathology of fistulous tract revealed no specific cause. Discussion
