Abstract: The White Paper Working for Patients has produced dramatic changes in the organization of health care in the UK that will soon be followed by analogous reforms in other countries. The core of the reform is represented by the separation of the responsibility for purchasing health care from providing the services. The creation of this internal market is said to enhance efficiency, but some peculiar characteristics of health care prevent a Pareto optimal solution being reached. This paper describes the purchaser-provider relationship using an innovative principal agent model to assess the relative merits of the different forms of contracts. The model is also used to show how competition among providers allows the purchaser to extract this private information. From a theoretical point of view, the approach is innovative in the formulation of the principal's objective function; the interesting finding is that the presence of a stringent budget constraint alters both risk-sharing conditions and the First-best contract proposed by the literature. From a policy point of view, the paper explains why in the first wave over 75% of contracts between purchasers and providers were block contracts. It is also demonstrated why this contractual form should be avoided.

Abstract: Using a post structural lens, I make arguments against homogenizing people’s conditions and circumstances. In particular, I acknowledge that the post 1994-reform agenda intended to streamline the previously fragmented and segregated landscape. Black women, who are the main target of this article suffered triple marginalization- race, social class and sexism. The aim of the article is to show the tensions within White Paper (1997)- (A programme for the transformation of higher Education) that have not enabled gender gaps within post 1994 South African higher education addressed successfully- the gaps are still gapping. I state that we should not take for granted phrases such as “equal opportunities” and “equal access” in policies. Instead, we should seek their meaning and achievement inter alia in earnest for the targeted group. Therefore, I postulate that gender and gendering is complex and very fragmented. For this reason, formulating transformation interventions on the premise of equality for all does not necessarily guarantee gender equality or gender equity. With this in mind, a “one-size fits all” approach is implausible and does not suffice in addressing salient gender injustices. I propose a multifaceted approach, which encompasses a realistic and holistic outlook on the divergent needs of black women as a possible solution to the current challenges.

Abstract: The second 50 years of the NHS will see a very different relation between the service and the public it serves. One of the four key themes of the white paper is the introduction of NHS Direct, a 24 hour nurse led helpline. After piloting it will cover the whole country by 2000, as proposed in the chief medical officer's report on emergency services in the community.1 However, NHS Direct is not solely about telephone advice lines. It should also herald a fundamental shift in the NHS where more public participation in health care can happen closer to home and where more care can be delivered without face to face contact. It may well be the most important development this white paper has to offer. NHS Direct has profound implications for the shape and purpose of the developing health service in general and primary health care in particular (especially if the gateway to the NHS moves from the surgery to the sitting room).
