Modified Fontan operation for univentricular heart and complicated congenital lesions.

Aortic dissection and aortic aneurysm surgery: clinical observations, experimental investigations, and statistical analyses. Part II.

Sixteen consecutive patients (12 with tricuspid atresia) underwent a Fontan procedure, with no operative deaths. There were three late deaths. Eleven of the survivors were electively catheterized 4-25 months postoperatively. Rest and exercise hemodynamics were measured in five patients, and resting hemodynamics alone were measured in three. All exercised patients were New York Heart Association class I Cardiac index was low at rest (2.3 +/- 0.61/min/m2) and during exercise (4.9 +/- 1.11/min/m2) due to a low stroke index both at rest (28 ml) and exercise (35 ml). Accordingly, mixed venous oxygen saturations were decreased (66% at rest and 31% during exercise). These values are significantly lower than those at rest and during exercise from 23 control patients of similar age and size. Heart rates, pulmonary vascular resistances, and left ventricular filling pressures appeared normal both at rest and during exercise. High right atrial pressure at rest (15 mm Hg) was associated with minimal conduit gradient (2 mm Hg). However, exercise increased the mean conduit gradient to 8 mm Hg, demonstrating significant functional conduit obstruction. As expected, the cardiovascular response to exercise is abnormal after the Fontan procedure, even in asymptomatic patients. This abnormal response may be exacerbated by conduit obstruction, and conduit obstruction may not be apparent during resting studies.

Four patients who had had a Fontan type of procedure for tricuspid atresia 23, 6, 6, and 11 months previously were investigated by ambulatory electrocardiographic recording and simultaneous recording of the jugular venous pressure and echocardiogram of the conduit or pulmonary valve. All had been considerably improved by the operation. In 1 patient episodes of supraventricular tachycardia were recorded but no rhythm disturbance was detected in the other 3. Pulmonary blood flow was shown to be pulsatile and atrial systole is an important factor in this. The conduit valve showed delayed opening and slow closure suggesting that its presence in the pulmonary circuit may be unnecessary.

Persistence and effects of sinus rhythm after Fontan procedure for tricuspid atresia.

Two cases of perforation of the left ventricle during mitral valve replacement are described. In the first case there was perforation at the site of papillary muscle excision and this was recognized and successfully treated. However, a true ventricular aneurysm developed at the repair site. One month after operation rupture of the left ventricle occurred at a second and separate site on the posterior aspect of the atrioventricular ring. This resulted in a false aneurysm which produced a pansystolic murmur mimicking mitral regurgitation. Both the true and the false aneurysm were successfully repaired. In the second case perforation occurred on the posterior aspect of the atrioventricular ring and was successfully repaired. However, a false ventricular aneurysm developed and ruptured into the left atrium producing severe, but silent, mitral regurgitation. This was recognized and successfully repaired. The implications of these cases are discussed.

Endomyocardial fibrosis in a European woman and its successful surgical treatment

One hundred patients with left ventricular aneurysms were operated on between February 1973 and January 1983. The principal indications for operation were left ventricular failure in 58, angina in 23, both in 17, with arrhythmia and systemic emboli accounting for one case each. Eighty five had had anterior infarction causing 82 anteroapical and three lateral aneurysms, while the remainder had had inferior infarcts resulting in 14 inferior aneurysms and one lateral aneurysm. Coronary angiography detected a single coronary lesion in 46%. Three patients had aneurysmal plication and the remainder had aneurysmectomy. Eleven mitral valve replacements were performed. Forty patients underwent coronary artery bypass grafting with a mean number of grafts per patients of 1.4. The early mortality was 7% with no early deaths since 1978. The actuarial five year survival was 68%, and 82% of survivors are in New York Heart Association class I or II (mean follow up three years). Left ventricular aneurysmectomy may be performed with a low operative mortality and good long term results.
