Summary. Currently new indicator of arterial stiffness is researched – cardio-ankle vascular index (CAVI). This index has been studied extensively in patients with coronary heart disease and shows its relationship with the severity of coronary artery lesions. CAVI is proposed to use as a noninvasive marker of the severity of coronary atherosclerosis. The purpose of this study was to examine the relationship between CAVI and the prevalence of atherosclerosis in patients with coronary artery disease. 511 patients with coronary heart disease were surveyed prior to coronary bypass surgery. Normal values of CAVI (< 8,0) were identified in 46% of cases, borderline (≥ 8,0 and < 9,0) – in 29% of cases and pathological (≥ 9,0) – in 25% of cases. In patients with different values of CAVI there was no difference in the expression of coronary and carotid atherosclerosis. At the same time, atherosclerosis of the lower extremities was more common in patients with low values of CAVI (11,4-14,0%), less – in pathological values (6,3%) and even more rarely – in intermediate values of the index (1,3%).

Can we use cardio-ankle vascular index to estimate the prevalence of atherosclerosis in patients with coronary heart disease?

Aim.  To compare  the  novel arterial stiffness  index START and  the cardio-ankle vascular index (CAVI) in hypertensive (HTN) patients. Material and methods. A total of 709 people  with HTN aged  24 to 64 years  were  examined by volume sphygmography  using VaSera VS-1000 (Fukuda Denshi, Japan).  CAVI  and  START values obtained retrospectively  from pulse  wave velocity and  blood pressure  were studied using an online calculator.Results. The median CAVI score was within the normal range (<9,0): 7,5 (6,6; 8,4) on the right and 7,4 (6,5; 8,4) on the left. The median of the START index was 6,25 (5,0; 7,55) on the right and 6,4 (5,1; 7,8) on the left. For young people (21-30 years old), the median START score was 4,47, for the CAVI index — 6,25 (p>0,05). In the older age group (61-70 years), the median START index was 7,27, for the CAVI index — 8,4 (p>0,05).  The growth pattern of the START score with age was higher by 38,5% than the CAVI index by 25,6% (p=0,034). A high degree  of correlation between CAVI and START was noted (r=0,823, p<0,001).  Parameters were associated with age (r=0,412 for CAVI and r=0,355 for START; p<0,001), smoking duration (r=0,390 and r=0,361; p<0,001), glomerular filtration rate (r=-0,317 and r= -0,318; p<0,001), body mass index (r=-0,176 and r=-0,185; p=0,001). For the CAVI index, a relationship was found with the glucose level (r=0,192; p<0,001) and a sedentary  lifestyle (r=-0,157; p=0,04), while for the START index — with the creatinine level (r=0,143; p=0,01).Conclusion.  Patients  with hypertension  showed  a high correlation between   the  novel  START index  and  the  CAVI   score   (r=0,823, p<0,001).  The indicators  had similar associations  with risk factors, which is important for the  subsequent practical  application of the START index.

Comparative analysis of START and CAVI arterial stiffness scores in hypertensive patients

Background. There are many differences in chest pain symptoms between men and women in terms of location, nature, and additional symptoms. The issue of describing the differences in chest pain in men and women with acute coronary syndrome (ACS), as well as their correlation with changes in the electrocardiogram (ECG) and coronary angiography (CAG) remains relevant.Methods. The study included 588 patients of the cardiology department of the Novokuznetsk City Clinical Hospital No. 1 from 2013 to 2017 with a diagnosis of ACS. Depending on the gender, the subjects were divided into two groups: Group I – 330 men; Group II – 258 women.Results. ACS with ST elevation was more common in men (45.8 %) than in women (33.3 %; p = 0.002). There were no pathological ECG changes in women in 58.1 % of cases, in men – in 45.5 % (p < 0.001). ECG type Q/ST elevation was detected more often in men (45.8 %) than in women (33.3 %; p = 0.002). The absence of coronary artery lesions was observed in 27.9 % of men and 44.2 % of women (p < 0.001). Hemodynamically significant coronary artery stenosis was more common in men (57.6 %) than in women (38.7 %; p < 0.001). In a typical angina clinic, hemodynamically significant coronary artery disease in patients with Q/without ST elevation ACS was detected in 40.2 % of men and in 58.5 % of women (p = 0.002). In the atypical angina clinic, hemodynamically significant lesions of coronary artery were more common in men (40.6 %) than in women (34.1 %; p = 0.02).Conclusion. In women atypical chest pains and intact coronary arteries were detected more often than in men, and hemodynamically significant coronary artery stenosis were found less often than in men. In men, a more pronounced pathology of the coronary arteries in ACS was revealed, in women – great difficulties in diagnosing ACS.

Clinical symptoms and ECG data in women with acute coronary syndrome

Highlights. Patients with coronary artery disease undergoing coronary artery bypass grafting have a high prevalence of type 2 diabetes mellitus and prediabetes. The frequency of postoperative stroke and hospital stay is significantly higher in patients with impaired carbohydrate metabolism.Insulin resistance markers are associated with a variety of perioperative characteristics, but according to multivariate analysis, only free fatty acids and HOMA-IR were independent predictors of hospitalacquired complications and long-term hospital stayAim. To analyze insulin resistance markers and their association with the preoperative outcome and in-hospital complications of coronary bypass grafting (CABG) in patients with type 2 diabetes mellitus (DM 2), prediabetes and normoglycemia.Methods. The study included 383 consecutive patients undergoing CABG at the same center. Glycemic status, free fatty acids (FFA), fasting insulin, glucose, lipid profile of all patients were determined before surgery and the following insulin resistance indices (IR) were calculated: HOMA-IR (Homeostasis Model Assessment of Insulin Resistance), QUICKI (Quantitative Insulin Sensitivity Check Index), Revised QUICKI, McAuley. Patients were divided into 2 groups: the group that included patients with carbohydrate metabolism disorders (CMD), type 2 diabetes mellitus and prediabetes (n = 192), and the group of patients without CMD (n = 191). Perioperative characteristics of patients, postoperative complications and their association with insulin resistance markers were analyzed.Results. FFA and calculated indices of insulin resistance such as HOMA-IR, QUICKI, RevisedQUICKI, and McAuley correlated with the following perioperative characteristics: the duration of surgical intervention and cardiopulmonary bypass, lipid levels, coagulation index, left ventricular dimension and myocardial diastolic function, etc. The analysis of in-hospital complications revealed that the frequency of postoperative stroke (p = 0.044) and hospital stay after CABG >30 days (p = 0.014) was significantly higher in patients with CMD. According to the results of multivariate analysis, the predictors of the composite endpoint (hospital stay after CABG>10 days and/or significant perioperative complication) were as follows: female sex (odds ratio (OR) 2.862, 95% confidence interval (CI) 1.062-7.712, p = 0.036); age (OR 1.085, 95%CI 1.027–1.147, p = 0.003); duration of cardiopulmonary bypass (OR 1.146, 95%CI 1.008–1.301, p = 0.035); body mass index (OR 1.125, 95% CI 1.035–1.222, p = 0.005), left atrial dimension (OR 5.916 95% CI 2.188–15.996, p<0.001); any CMD (OR 1.436, 95%CI 1.029–2.003, p = 0.032), type 2 DM (OR 2.184, 95%CI 1.087–4.389, p = 0.027), FFA levels (OR 5.707, 95%CI 1.183–27.537, p = 0.029) and HOMA–IR index (OR 1.164, 95%CI 1.025–1.322, p = 0.019).Conclusion. FFA, HOMA-IR, QUICKI, Revised-QUICKI, and McAuley correlate with a variety of perioperative characteristics of patients undergoing CABG, but multivariate analysis revealed that only FFA levels and the HOMA-IR can be used as predictors of in-hospital complications and prolonged hospital stay.
