In Reply. —In reply to the letter of Dr Burke, all conclusions and figures were based on the prestated primary end point of the occurrence of either nonfatal recurrent infarction or cardiac death, whichever came first. Data on cardiac death were included for completeness. As we stated, the lack of independent risk for cardiac death may imply either a differential effect of psychosocial factors on recurrent cardiac events, or a reduced statistical power for cardiac death (123 vs 226 events). We certainly agree with Drs Frasure-Smith, Lesperance, Hojat, Morris, and Robinson that negative emotions such as depression, loneliness, anxiety, and emotional distress may bear an important relationship to the observed increase in major recurrent events. As of October 1983 only five generic drugs (quinidine, procainamide, lidocaine, disopyramide, and verapamil) have b h n approved by the Food and Drug Administration (FDA) as primary antiarrhythmic agents. Although beta blockers have some antiarrhythmic properties, these drugs exert their effects through antiadrenergic actions and thus should be considered secondary antiarrhythmic agents. It is well recognized by the pharmaceutical industry, clinical pharmacologists, cardiologists, and primary care physicians that an enormous number of patients are troubled with annoying palpitations and a smaller number with potentially life-threatening arrhythmias. Furthermore, the existing oral antiarrhythmic agents have limited efficacy and a high incidence of adverse side effects. Presently, the ideal antiarrhythmic drug puitable for chronic usage does not exist. Thus, the challenge exists for developing and scientifically substantiating the safety and efficacy of new antiarrhythmic agents. At the present time at least ten antiarrhythmic agents are undergoing active clinical testing in the United States; these are amiodarone, aprindine, encainide, ethmozin, flecainide, imipramine, lorcainide, mexiletine, propafenone, and tocainide. How should clinical trials with these agents be carried out, and what are the pitfalls in drug testing that are unique to this class of agents?
