Background and Purpose - Numerous imaging techniques have been developed and applied to evaluate brain hemodynamics. Among these are positron emission tomography, single photon emission computed tomography, Xenon-enhanced computed tomography, dynamic perfusion computed tomography, MRI dynamic susceptibility contrast, arterial spin labeling, and Doppler ultrasound. These techniques give similar information about brain hemodynamics in the form of parameters such as cerebral blood flow or cerebral blood volume. All of them are used to characterize the same types of pathological conditions. However, each technique has its own advantages and drawbacks. Summary of Review - This article addresses the main imaging techniques dedicated to brain hemodynamics. It represents a comparative overview established by consensus among specialists of the various techniques. Conclusions - For clinicians, this article should offer a clearer picture of the pros and cons of currently available brain perfusion imaging techniques and assist them in choosing the proper method for every specific clinical setting.

Background and Purpose— We sought to evaluate a new, angle-independent ultrasonic device for assessment of blood flow volume (BFV) in the internal carotid artery (ICA). Methods— Nineteen patients and 4 healthy volunteers were enrolled in a comparative study conducted in the Care Unit of the Division of Neurosurgery at UCLA Medical Center. All patients had been admitted because of severe brain injury: 15 patients with severe head trauma (Glasgow Coma Scale score≤8) and 4 patients with subarachnoid hemorrhage due to aneurysm rupture. In all patients and subjects, cerebral blood flow (CBF) values obtained with the 133xenon-clearance technique were compared with BFV measurements in the ipsilateral ICA. Results— Hemispheric CBF values showed a close and linear correlation with BFV measurements (r=0.76, P<0.0001). Assessment of cerebral blood flow by means of blood-flow-volume measurement in the internal carotid artery: comparative study with a 133xenon clearance technique.

Ultrasound instruments are used to evaluate blood flow velocities in the human body. Most clinical instruments perform velocity calculations based on the Doppler principle and measure the frequency shift of a reflected ultrasound beam. Doppler-only instruments use single-frequency, single-crystal transducers. Linear- and annular-array multiple-crystal transducers are used for duplex scanning (simultaneous B-mode image and Doppler). Clinical interpretation relies primarily on determination of peak velocities or frequency shifts as identified by the Doppler spectrum. Understanding of the validity of these measurements is important for instruments in clinical use. The present study examined the accuracy with which several ultrasound instruments could estimate velocities based on the identification of the peak of the Doppler spectrum, across a range of different angles of insonation, on a Doppler string phantom. The string was running in a water tank at constant speeds of 50, 100, and 150 cm/sec and also in a sine wave pattern at 100- or 150-cm/sec amplitude. Angles of insonation were 30, 45, 60, and 70 degrees. The single-frequency, single-crystal transducers (PC Dop 842, 2-MHz pulsed-wave, 4-MHz continuous-wave) provided acceptably accurate velocity estimates at all tested velocities independent of the angle of insonation. All duplex Doppler instruments with linear-array transducers (Philips P700, 5.0-MHz; Hewlett-Packard Sonos 1000, 7.5-MHz; ATL Ultramark 9 HDI, 7.5-MHz) exhibited a consistent overestimation of the true flow velocity due to increasing intrinsic spectral broadening with increasing angle of insonation.(ABSTRACT TRUNCATED AT 250 WORDS)
