TL;DR: Recommendations for expectantly-managed patients include close follow-up, serial US and UD, and prompt initiation of CIC upon recognition of new hydronephrosis, reflux, elevated storage pressures, or febrile UTIs, as this was associated with fewer DMSA abnormalities than delaying CIC.
