The National High Blood Pressure Education Program presents the complete Seventh Report of the Joint National Committee on Prevention, Detection, Evaluation, and Treatment of High Blood Pressure. Like its predecessors, the purpose is to provide an evidence-based approach to the prevention and management of hypertension. The key messages of this report are these: in those older than age 50, systolic blood pressure (BP) of greater than 140 mm Hg is a more important cardiovascular disease (CVD) risk factor than diastolic BP; beginning at 115/75 mm Hg, CVD risk doubles for each increment of 20/10 mm Hg; those who are normotensive at 55 years of age will have a 90% lifetime risk of developing hypertension; prehypertensive individuals (systolic BP 120-139 mm Hg or diastolic BP 80-89 mm Hg) require health-promoting lifestyle modifications to prevent the progressive rise in blood pressure and CVD; for uncomplicated hypertension, thiazide diuretic should be used in drug treatment for most, either alone or combined with drugs from other classes; this report delineates specific high-risk conditions that are compelling indications for the use of other antihypertensive drug classes (angiotensin-converting enzyme inhibitors, angiotensin-receptor blockers, beta-blockers, calcium channel blockers); two or more antihypertensive medications will be required to achieve goal BP (<140/90 mm Hg, or <130/80 mm Hg) for patients with diabetes and chronic kidney disease; for patients whose BP is more than 20 mm Hg above the systolic BP goal or more than 10 mm Hg above the diastolic BP goal, initiation of therapy using two agents, one of which usually will be a thiazide diuretic, should be considered; regardless of therapy or care, hypertension will be controlled only if patients are motivated to stay on their treatment plan. Positive experiences, trust in the clinician, and empathy improve patient motivation and satisfaction. This report serves as a guide, and the committee continues to recognize that the responsible physician's judgment remains paramount.

The metabolic syndrome has received increased attention in the past few years. This statement from the American Heart Association (AHA) and the National Heart, Lung, and Blood Institute (NHLBI) is intended to provide up-to-date guidance for professionals on the diagnosis and management of the metabolic syndrome in adults.

The metabolic syndrome is a constellation of interrelated risk factors of metabolic origin— metabolic risk factors —that appear to directly promote the development of atherosclerotic cardiovascular disease (ASCVD).1 Patients with the metabolic syndrome also are at increased risk for developing type 2 diabetes mellitus. Another set of conditions, the underlying risk factors , give rise to the metabolic risk factors. In the past few years, several expert groups have attempted to set forth simple diagnostic criteria to be used in clinical practice to identify patients who manifest the multiple components of the metabolic syndrome. These criteria have varied somewhat in specific elements, but in general they include a combination of both underlying and metabolic risk factors.

The most widely recognized of the metabolic risk factors are atherogenic dyslipidemia, elevated blood pressure, and elevated plasma glucose. Individuals with these characteristics commonly manifest a prothrombotic state and a pro-inflammatory state as well. Atherogenic dyslipidemia consists of an aggregation of lipoprotein abnormalities including elevated serum triglyceride and apolipoprotein B (apoB), increased small LDL particles, and a reduced level of HDL cholesterol (HDL-C). The metabolic syndrome is often referred to as if it were a discrete entity with a single cause. Available data suggest that it truly is a syndrome, ie, a grouping of ASCVD risk factors, but one that probably has more than one cause. Diagnosis and Management of the Metabolic Syndrome An American Heart Association/National Heart, Lung, and Blood Institute Scientific Statement

2007 Guidelines for the Management of Arterial Hypertension : The Task Force for the Management of Arterial Hypertension of the European Society of Hypertension (ESH) and of the European Society of Cardiology (ESC).

Results The prevalence of the metabolic syndrome ranged from 8.8% to 14.3%, depending on the definition. There were 109 deaths during the approximately 11.4year follow-up, of which 46 and 27 were due to CVD and CHD, respectively. Men with the metabolic syndrome as defined by the NCEP were 2.9 (95% confidence interval [CI], 1.2-7.2) to 4.2 (95% CI, 1.6-10.8) times more likely and, as defined by the WHO, 2.9 (95% CI, 1.2-6.8) to 3.3 (95% CI, 1.4-7.7) times more likely to die of CHD after adjustment for conventional cardiovascular risk factors. The metabolic syndrome as defined by the WHO was associated with 2.6 (95% CI, 1.4-5.1) to 3.0 (95% CI, 1.5-5.7) times higher CVD mortality and 1.9 (95% CI, 1.2-3.0) to 2.1 (95% CI, 1.3-3.3) times higher all-cause mortality. The NCEP definition less consistently predicted CVD and all-cause mortality. Factor analysis using 13 variables associated with metabolic or cardiovascular risk yielded a metabolic syndrome factor that explained 18% of total variance. Men with loadings on the metabolic factor in the highest quarter were 3.6 (95% CI, 1.7-7.9), 3.2 (95% CI, 1.7-5.8), and 2.3 (95% CI, 1.5-3.4) times more likely to die of CHD, CVD, and any cause, respectively. Conclusions Cardiovascular disease and all-cause mortality are increased in men with the metabolic syndrome, even in the absence of baseline CVD and diabetes. Early identification, treatment, and prevention of the metabolic syndrome present a major challenge for health care professionals facing an epidemic of overweight and sedentary lifestyle.

The metabolic syndrome and total and cardiovascular disease mortality in middle-aged men

This study aims to demonstrate the prevalence of pain as a reason for seeing a physician in primary care. We also performed an analysis of the localization, duration and frequency of pains, as well as the diagnoses of patients having pain. A total of 28 physicians at 25 health centers in Finland collected the data, comprising 5646 patient visits. Pain was identified as the reason for 2237 (40%) of the visits. The most common localizations were in the lower back, abdomen and head. One-fifth of the pain patients had experienced pain for over six months. Analysis of the diagnoses revealed half of the pains to be musculoskeletal. Patients experienced considerable limitations in various activities of life due to pain. A quarter of the pain patients of active working age received sick leave. Our results confirm that pain is a major primary health care problem, which has an enormous impact on public health.

ContextChronic pain is common in Western societies. Self-rated health is an
important indicator of morbidity and mortality, but little is known about
the relation between chronic pain and self-rated health in the general population.ObjectiveTo analyze the association between chronic pain and self-rated health.Design, Setting, and PopulationA questionnaire survey carried out during the spring of 2002 of an age-
and sex-stratified population sample of 6500 individuals in Finland aged 15
to 74 years, with a response rate of 71% (N = 4542) after exclusion of those
with unobtainable data (n = 38). Chronic pain was defined as pain with a duration
of at least 3 months and was graded by frequency: (1) at most once a week;
(2) several times a week; and (3) daily or continuously. On the basis of a
5-item questionnaire on self-rated health, individuals were classified as
having good, moderate, or poor health. Multinominal logistic regression analysis
was used to assess the determinants of health. Analysis included sex, age,
education, working status, chronic diseases, and mood.Main Outcome MeasuresPerceived chronic pain graded by frequency and self-rated health status.ResultsThe prevalence of any chronic pain was 35.1%; that of daily chronic
pain, 14.3%. The prevalence of moderate self-rated health was 26.6% and of
poor health, 7.6%. For moderate self-rated health among individuals having
chronic pain at most once a week compared with individuals having no chronic
pain, the adjusted odds were 1.36 (95% confidence interval [CI], 1.05-1.76);
several times a week, 2.41 (95% CI, 1.94-3.00); and daily, 3.69 (95% CI, 2.97-4.59).
Odds for poor self-rated health were as follows: having chronic pain at most
once a week, 1.16 (95% CI, 0.65-2.07); several times a week, 2.62 (95% CI,
1.76-3.90); and daily, 11.82 (95% CI, 8.67-16.10).ConclusionChronic pain is independently related to low self-rated health in the
general population.

Chronic Pain and Poor Self-rated Health

Most researchers agree that obesity is an important modulator of the metabolic syndrome, 1 2 which is a clustering of cardiovascular risk factors associated with insulin resistance —such as hypertension,hypertriglyceridaemia,a low concentration of high density lipoprotein cholesterol, abnormal glucose metabolism, and hyperinsulinaemia.3Little is known, however, about the association between relative weight change from childhood to adulthood and the development of metabolic syndrome in adulthood.

We recently published data of a population study for the metabolic syndrome, performed during 1993-4 in Pieksamaki, Finland. All subjects (n=1008) born in the years 1947, 1952, and 1957 were examined according to a protocol described elsewhere.4Data on both weight and height at age 7 years (the start of primary school) were also collected.

Relation between obesity from childhood to adulthood and the metabolic syndrome: population based study

Objective Previous cross-sectional studies have suggested that patients with depression have a high risk for metabolic syndrome. As there is a paucity of data concerning the temporal relationship of depression and metabolic syndrome, we decided to evaluate the risk for developing depressive symptoms in patients with metabolic syndrome in a population-based follow-up study. Method The prevalence of depressive symptoms and metabolic syndrome at baseline in 1998 and at 7-year follow-up in 2004/2005 was studied in a large, middle-aged, population-based sample collected from Central Finland. Depressive symptoms were measured with the Beck Depression Inventory, with a cutoff score of 10 points. Metabolic syndrome was assessed using the modified National Cholesterol Education Program Adult Treatment Panel III criteria. Results Nondepressed women and men with metabolic syndrome at baseline were twice as likely to have depressive symptoms at follow-up (OR = 2.2, 95% CI = 1.1 to 4.5 for women; OR = 2.2, 95% CI = 0.8 to 5.9 for men) as compared with the nondepressed cohort members without metabolic syndrome at baseline. Conclusions The higher rate of depressive symptoms in the subgroup with metabolic syndrome suggests that the metabolic syndrome may be an important predisposing factor for the development of depression. Effective prevention and treatment of metabolic syndrome could also be important for the prevention of depression.
