Fragile X syndrome (FXS) is the leading inherited form of intellectual disability and autism spectrum disorder, and patients can present with severe behavioural alterations, including hyperactivity, impulsivity and anxiety, in addition to poor language development and seizures. FXS is a trinucleotide repeat disorder, in which >200 repeats of the CGG motif in FMR1 leads to silencing of the gene and the consequent loss of its product, fragile X mental retardation 1 protein (FMRP). FMRP has a central role in gene expression and regulates the translation of potentially hundreds of mRNAs, many of which are involved in the development and maintenance of neuronal synaptic connections. Indeed, disturbances in neuroplasticity is a key finding in FXS animal models, and an imbalance in inhibitory and excitatory neuronal circuits is believed to underlie many of the clinical manifestations of this disorder. Our knowledge of the proteins that are regulated by FMRP is rapidly growing, and this has led to the identification of multiple targets for therapeutic intervention, some of which have already moved into clinical trials or clinical practice.

In this systematic review, we aimed to compare conical versus nonconical implant-abutment connection systems in terms of their in vitro and in vivo performances. An electronic search was performed using PubMed, Embase, and Medline databases with the logical operators: "dental implant" AND "dental abutment" AND ("conical" OR "taper" OR "cone"). Names of the most common conical implant-abutment connection systems were used as additional key words to detect further data. The search was limited to articles published up to November 2012. Recent publications were also searched manually in order to find any relevant studies that might have been missed using the search criteria noted above. Fifty-two studies met the inclusion criteria and were included in this systematic review. As the data and methods, as well as types of implants used was so heterogeneous, this mitigated against the performance of meta-analysis. In vitro studies indicated that conical and nonconical abutments showed sufficient resistance to maximal bending forces and fatigue loading. However, conical abutments showed superiority in terms of seal performance, microgap formation, torque maintenance, and abutment stability. In vivo studies (human and animal) indicated that conical and nonconical systems are comparable in terms of implant success and survival rates with less marginal bone loss around conical connection implants in most cases. This review indicates that implant systems using a conical implant-abutment connection, provides better results in terms of abutment fit, stability, and seal performance. These design features could lead to improvements over time versus nonconical connection systems.

Performance of conical abutment (Morse Taper) connection implants: a systematic review.

P>Aim The aim of the present systematic review of implant-supported maxillary overdentures was to assess the survival of implants, survival of maxillary overdentures and the condition of surrounding hard and soft tissues after a mean observation period of at least 1 year. Material and methods MEDLINE (1950-August 2009), EMBASE (1966-August 2009) and CENTRAL (1800-August 2009) were searched to identify eligible studies. Two reviewers independently assessed the articles. Results Out of 147 primarily selected articles, 31 studies fulfilled the inclusion criteria. A meta-analysis showed an implant survival rate (SR) of 98.2% per year in case of six implants and a bar anchorage. In case of four implants and a bar anchorage, the implant SR was 96.3% per person. In case of four implants and a ball anchorage, the implant SR was 95.2% per year. Conclusion In all three treatment options, the SR of the implants is more than 95%. The studies included reveal that a maxillary overdenture supported by six dental implants, which are connected with a bar, is the most successful treatment regarding survival of both the implants and overdenture. Second in line is the treatment option with four implants and a bar. The treatment option with four or less implants and a ball attachment system is the least successful.

A systematic review of implant‐supported maxillary overdentures after a mean observation period of at least 1 year

Polyamide as a Denture Base Material: A Literature Review.

Statement of problem Although new materials have emerged as options to fabricate removable dental prostheses, the development of Candida biofilms on these materials and the effectiveness of methods to control these pathogenic biofilms are poorly understood. Purpose The purpose of this study was to evaluate the efficacy of denture cleansers on Candida single- and dual-species biofilms formed on polyamide resin. Material and Methods Polymethyl methacrylate (PMMA) resin (Acron MC) and polyamide resin (Flexite M.P.) specimens (n=116) were prepared, and their surface roughness was standardized (0.34 ±0.02 μm). Surface free energy (SFE) was measured for some specimens (n=20 per resin), while the remainder were randomly divided by lottery into 24 groups (n=8) for biofilm assay. C. albicans and/or C. glabrata biofilm was formed for 72 hours, and then specimens were treated with an enzymatic cleanser solution (Polident 3 Minutes), a cleanser solution (Corega Tabs), or 0.5% sodium hypochlorite (NaOCl) solution. Water served as the negative control. Remaining adherent microorganisms were removed from the treated specimens by ultrasonic waves, and colony-forming units (CFU) of each microorganism were calculated. SFE data were analyzed by 1-way ANOVA, and Candida species data were analyzed by 3-way ANOVA followed by the Tukey-Kramer test ( P =.05). Results All tested biofilms displayed significantly higher growth on polyamide resin ( P Candida levels; however, the 0.5% NaOCl solution was the only effective cleanser. C. glabrata revealed significantly higher CFU counts under all experimental conditions ( P Conclusions The highest Candida spp. biofilm growth was shown to occur on polyamide resin when compared with PMMA. Denture cleansers were able to remove Candida spp. biofilm formed on both denture base resins.

Preload loss can favor the occurrence of implant-abutment interface misfit, and bacterial colonization at this interface may lead to implant failure. The aim of this study was to evaluate the preload loss and bacterial penetration through the implant-abutment interface of conical and external hexagon connection systems subjected to thermal cycling and mechanical fatigue (TM). Four different implant-abutment connection systems were evaluated (n=6): external hexagon with universal post, Morse taper with universal post, Morse taper with universal post through bolt, and locking taper with standard abutment. The assemblies (implant-abutment) were subjected to a thermal cycling regimen (1,000 cycles of 5 degrees C and 55 degrees C) and to mechanical fatigue (1.0 million cycles, 1.0 Hz, 120 N). The assemblies were immersed in Tryptic Soy + Yeast Extract broth containing Streptococcus sanguinis and incubated at 37 degrees C and 10% CO(2) for 72 h. Detorque values were recorded. The bacterial penetration was assessed and the abutments were observed by scanning electron microscopy. The preload data were analyzed statistically by two-way ANOVA and Tukey's test at 5% significance level. All screw abutment systems showed significantly higher (p<0.05) detorque values when subjected to TM and all conical systems presented bacterial penetration. The results show no relationship between the preload loss and the bacterial penetration.

Preload loss and bacterial penetration on different implant-abutment connection systems

The addition of different polymers, such as polytetrafluorethylene (PTFE), to denture base resins could be an option to modify acrylic resin mechanical properties. This study evaluated the surface hardness, impact and flexural strength, flexural modulus and peak load of 2 acrylic resins, one subjected to a long and another subjected to a short polymerization cycle, which were prepared with or without the addition of 2% PTFE. Four groups were formed according to the polymerization cycle and addition or not of PTFE. Forty specimens were prepared for each test (10 per group) with the following dimensions: hardness (30 mm diameter x 5 mm thick), impact strength (50 x 6 x 4 mm) and flexural strength (64 x 10 x 3.3 mm) test. The results of the flexural strength test allowed calculating flexural modulus and peak of load values. All tests were performed in accordance with the ISO 1567:1999 standard. Data were analyzed statistically by ANOVA and Tukey's test with the level of significance set at 5%. No statistically significant differences (p>0.05) were found for surface hardness. Flexural strength, impact strength and peak load were significantly higher (p<0.05) for resins without added PTFE. The flexural modulus of the acrylic resin with incorporated 2% PTFE polymerized by long cycle was significantly higher (p<0.05) than that of the other resins. Within the limits of this study, it may be concluded that the addition of PTFE did not improve the mechanical properties of the evaluated acrylic resins.

Objective: The aim was to analysis the importance of multidisciplinary members of Intensive Care Unit (ICU) and dentists assign to the integration of dentist into that group. Additionally, evaluate the oral hygiene protocol used in ICU patients. Methods: The questionnaire was applied in 58 staff who directly work on the ICU: 38 nursing technicians, 08 doctors, 08 nurses, 04 physiotherapists and 29 dentists that they was not members staff of ICU. After all volunteers completed questionnaire, then the results were analyzed and the percentages for each question were calculated. Results: 57% of the multidisciplinary member and 96% of dentists agree that it is important the presence of dentists in this group. Considering the positive influence of dentists on improvement of clinical case of the ICU patient, it was verified that 55% of multidisciplinary members staff agree that the dentist on ICU member staff will result an improvement of the patient clinical case. Additionally, 100% volunteers agree that efficient oral hygiene is important for patients on ICU care. Conclusion: Even though oral hygiene was considered important in ICU patients by all professionals. There was no agreement on recognizing the importance of the dentist on multidisciplinary group of ICU. Additionally, the protocol of biofilm control currently used on ICUs was not the most appropriate. Entretanto, a literatura é unânime em mostrar que a equipe não está completa, pois falta a presença do cirurgião-dentista para que ocorra de fato promoção da saúde integral de pacientes internados em UTIs1. Os Projetos de Lei (PL): no 2.776/2008 e PL 363/2011, ambos aprovados pela Comissão de Seguridade Social e Família em 2012, estabelecem a obrigatoriedade da presença de profissionais da Odontologia em hospitais públicos e privados em que existam pacientes internados em UTI ou enfermarias. Essa medida objetiva aprimorar os cuidados prestados aos pacientes, defender e apoiar a prestação de assistência integral à saúde, que na verdade consiste em um dos princípios do Sistema Único de Saúde (SUS), expresso na Constituição Federal2. A presença de um cirurgião-dentista torna-se importante para a concretização da saúde integral dos pacientes hospitalizados em Unidades de Terapia Intensiva (UTI), pois estes pacientes necessitam de cuidados rigorosos devido a um quadro clínico caracterizado por imunodeficiência, fato que os tornam mais susceptíveis à instalação de infecções bucais e/ou sistêmicas, agravando o seu estado de saúde geral3. Na maioria dos casos, pacientes hospitalizados em UTI encontram-se totalmente dependentes para a realização de higiene bucal adequada e eficiente, necessitando, assim, do suporte profissional devidamente capacitado ou bem orientado para adequação ou reestabelecimento da qualidade do meio bucal destes pacientes4. Algumas pesquisas documentaram1-5 que pacientes admitidos em UTI possuem higiene bucal comprometida, possivelmente pela ausência de supervisão e relacionamento interprofissional Odontologia e Enfermagem, visto que uma das funções do cirurgião-dentista em UTI é a de supervisionar e orientar adequadamente os técnicos de enfermagem para a realização de uma higiene bucal satisfatória e eficaz. A manutenção de um controle rigoroso da higiene bucal de pacientes admitidos em UTI visa controlar o desenvolvimento e a maturação de um biofilme de maior patogenicidade nos diversos sítios da cavidade bucal, não só dentes, mas também mucosa de recobrimento, língua e dispositivos protéticos fixos. Bactérias gram-positivas são comumente encontradas na cavidade bucal, mas, à medida que o biofilme desenvolve, podem ocorrer associações com bactérias anaeróbicas gram-negativas e fungos, tornando este biofilme mais patogênico e, consequentemente, aumentando o risco de complicações sistêmicas5. A adesão de microrganismos e o desenvolvimento do biofilme nas superfícies presentes na cavidade bucal ocorrem de maneira relativamente rápida. A quantidade e a patogenicidade deste biofilme formado aumenta de acordo com o aumento do tempo de internação, que, associada à falta da higiene bucal, pode acarretar vários problemas como: o aumento de patógenos respiratórios presentes no biofilme, instalação ou agravamento de doença periodontal, disseminação dessas bactérias e estabelecimento de infecções6,7. Estes microrganismos que colonizam as superfícies da cavidade bucal podem ser disseminados para outras partes do organismo por meio dos procedimentos hospitalares de rotina na UTI, como a entubação sob ventilação mecânica, que pode transportar bactérias presentes na cavidade bucal e da orofaringe até os pulmões8-11, favorecendo a instalação da pneumonia nosocomial, a qual está relacionada com aumento do tempo de hospitalização dos pacientes, onerando o tratamento ou, então, causando o óbito12-15. A higiene bucal efetiva de pacientes internados em UTI é primordial, uma vez que o biofilme bucal aumenta de volume de maneira rápida e intensa, pois é comum nestes pacientes a diminuição da autolimpeza natural da cavidade bucal. A autolimpeza bucal deficiente acontece por diversos motivos: redução da ingestão de alimentos duros e fibrosos, diminuição da movimentação da língua e das bochechas, redução do fluxo salivar devido ao uso de alguns medicamentos, sangramentos espontâneos da mucosa bucal e a presença de ressecamento e fissuras labiais16-20. Dentre os protocolos de higienização da cavidade bucal descritos na literatura é recomendada a remoção química e mecânica do biofilme, tanto em indivíduos dentados como desdentados e também em aparelhos protéticos, uma vez que o método mecânico associado ao químico é mais eficaz quando comparado com métodos apenas químicos (soluções bactericida e/ou bacteriostática)21-25. O conhecimento do papel da Odontologia em hospitais ainda é restrito. A orientação sobre a importância da higienização e qualidade de saúde bucal deve ser estendida a todos os diferentes segmentos da equipe multidisciplinar22,23. É importante que a Odontologia se integre ao atendimento dos pacientes hospitalizados nas UTIs com o objetivo de minimizar o risco de disseminação de patógenos da cavidade bucal que possam causar problemas sistêmicos, atuando na manutenção da higienização dos dentes, gengiva, bochecha e língua, e controlando a colonização intensa de patógenos. Além disso, a necessidade do cirurgião-dentista na equipe de atendimento das UTIs amplia o campo de ensino e atuação do profissional. Assim, os objetivos dessa pesquisa foram: verificar qual a importância que os profissionais que atuam na equipe multidisciplinar de UTI e os profissionais da Odontologia atribuem à integração de um cirurgião-dentista a essa equipe e, adicionalmente, investigar qual o protocolo de higienização bucal atual aplicado em pacientes internados em UTI. mAteRiAl e método Seleção dos voluntários O estudo foi submetido e aprovado pelos Comitês de Ética em Pesquisa (CEP) da Universidade do Oeste Paulista (Prot./N° 480/10) e Comitê de Ética do Hospital Regional de Base de Presidente Prudente. Os voluntários analisados nesta pesquisa foram profissionais da área da saúde de UTI: médicos, enfermeiros, fisioterapeutas e técnicos em enfermagem. A pesquisa foi realizada com os profissionais que compõem as equipes multidisciplinares das três UTIs do Hospital Regional de Base de Presidente Prudente. Também foram pesquisados cirurgiões-dentistas que, como critério de inclusão na pesquisa, não poderiam ter vínculo empregatício com nenhum hospital. Coleta de dados Foram convidados a participar da pesquisa 150 profissionais e houve o esclarecimento quanto a não obrigatoriedade da participação. Apenas 58 aceitaram o convite e assinaram o Termo de Consentimento Livre e Esclarecido, de acordo com os princípios éticos originados da Declaração de Helsinki. A pesquisa foi realizada no período de maio a agosto de 2012, por meio do preenchimento de um formulário quantitativo validado em um projeto piloto aplicado em uma porcentagem da amostra, para a concretização dos devidos ajustes das questões. O formulário validado foi aplicado sob a forma de questionário, e foi entregue aos participantes nos seus locais de trabalho após a aceitação de sua participação na pesquisa, sendo que os mesmos não precisaram se identificar e tiveram sete dias para respondê-lo antes que fossem recolhidos pelos pesquisadores. Primeiramente, foi aplicado o questionário para os profissionais da UTI e, posteriormente, para os profissionais da Odontologia. O questionário constava de duas partes distintas.
