Abstract: In the first five postoperative days following abdominoperineal resection, two groups of five patients were treated by infusion therapy following two schedules: 11.9 calories per kilogram per day and 0.042 gram of nitrogen per kilogram per day, and 53.8 calories per kilogram per day and 0.244 gram of nitrogen per kilogram per day. Nitrogen balance was determined, and the wound collagen content was evaluated by biopsies carried out on the second, third, fourth, fifth, sixth, ninth, 12th and 15th postoperative days. It was noted that in group 2 patients nitrogen balance was rather equilibrated and the quantity of collagen in the granulation tissue was higher than in group 1. Postoperative parenteral hyperalimentation spares a good quantity of nitrogen and can facilitate wound healing.

Abstract: From 1930 to 1972, 27 cases of melanoma of the vulva were treated at the National Cancer Institute of Milan. Prognosis was evaluated in relation to the therapy, the anatomical site, the lymphnodes metastases and the classification according to Clark. The median survival was 20 months and the 4-yr survival was 19%. The authors reached the following conclusions: site and T of melanoma have no prognostic value; survival is strictly related to the lymphnodes metastases and to Clark's classification: no N+ patients survived longer than two years while 62% of N-- patients are still alive after four years. No survival after 2 years was observed in IV-V class of Clark while a 54% survival after 4 years was observed in II-III class of Clark. Surgery gives the best results while radiotherapy as single treatment is useless and its role as adjuvant postoperative therapy is still uncertain.

Abstract: 420 consecutive patients having clean surgical operations were studied: 210 of them, selected at random, received 0.5-1 g of ampicillin into the wound before closure, the remainder being controls. Patients were examined for wound infection during the first postoperative week, in line with the opinion that topical antibiotic treatment can afford significant protection against wound infection caused by peroperative contamination. Wound infection has been defined as a local inflammation (redness and hyperthermia), usually with fever and/or discharge of pus. In some doubtful cases, a longer observation period was necessary to obtain a correct interpretation. Four out of 210 clean wounds (1.9%) became infected in the patients receiving ampicillin compared with 12 out of 210 (5.7%) in the control group. The difference could have arisen by chance less that one in a hundred times (p less than 0.01). It is our opinion therefore that after clean operations, the higher the risk of local infection, the more effective the topical use of antibiotics.

Abstract: A singular association of neurinoma of the abdominal vagus with relapsing peptic ulcer is reported. The singularity lies in the exceptional finding of a neurinoma of the left abdominal vagus -- not previously recorded in the literature -- and in the concomitant severe relapsing peptic ulcer, for which the nerve lesion may have been partly responsible.
