Background The authors evaluated the fracture resistance of endodontically treated teeth filled with either gutta-percha or a new resin-based obturation material. Methods The authors prepared and randomly divided 80 single-canal extracted teeth into five groups: lateral and vertical condensation with gutta-percha, lateral and vertical condensation with the new resin-based obturation material, and a control group with no filling material. The specimens were stored in 100 percent humidity for two weeks, mounted in polyester resin and loaded to failure. Results The authors found statistically significant differences among the experimental groups ( P Conclusions Filling the canals with the new resin-based obturation material increased the in vitro resistance to fracture of endodontically treated single-canal extracted teeth when compared with standard gutta-percha techniques. Clinical Implications If other properties of the new resin-based obturation material compare favorably with those of gutta-percha for filling the root canal, it should be considered as a replacement for gutta-percha, as the results of this study indicate that it could provide enhanced resistance to tooth fracture.

Fracture resistance of roots endodontically treated with a new resin filling material

When non-surgical attempts prove unsuccessful or are contraindicated, surgical endodontic therapy is needed to save the tooth. The procedure usually consists of exposure of the involved area, root end resection, root end preparation and insertion of a root end filling material. Numerous materials have been suggested as root end filling materials. This article is a review of the literature on the suitability of various root end filling materials based on their leakage assessment, marginal adaptation, cytotoxicity, and usage test in experimental animals and humans.

Root end filling materials: a review

Twenty cases of a rare tumor, chordoma involving the nasal cavity, paranasal sinuses, or nasopharynx, are reported Patients most often had localized headache, nasal obstruction, decreased hearing, or symptoms related to cranial nerve involvement, especially diplopia A mass bulging into the nasopharynx, posterior nasal cavity, or pharynx was found on physical examination in 13 of these 20 patients, and in another 6 patients on radiologic examination Paralysis of one or more cranial nerves was identified in the majority of patients who had tumors involving the upper nasopharynx On radiologic examination, bone destruction was frequently identified in the clivus and sphenoid areas, and less commonly in the cervical vertebrae Each patient had biopsy-proven tumor in the nasopharynx or sphenoid areas Patients usually were treated by combinations of surgery and radiotherapy, in some cases leading to prolonged periods of apparent tumor control Radiotherapy frequently resulted in regression of symptoms, sometimes lasting many months or years A few tumors that involved mainly the lower nasopharynx were more amenable to surgical resection, in one case leading to apparent tumor control In those cases with adequate follow-up information, most patients had either died of their disease or were living with recurrent or persistent inoperable tumor Some patients lived for many months or years between recurrences or with known tumor, indicating that chordomas often are slowly growing neoplasms The histologic features of these chordomas are described and illustrated, and the histopathologic differential diagnosis is discussed

Nonepithelial tumors of the nasal cavity, paranasal sinuses, and nasopharynx. A clinicopathologic study. XIII: Meningiomas.

Odontogenic myxoma: a clinicopathological study of 33 cases.

Ten cases of odontogenic myxoma were evaluated radiologically, histologically, and histochemically. Ultrastructural examination was performed in five cases. Stellate cells with branching processes were seen in an abundant acid mucopolysaccharide background. Occasionally macrophages were found in the tumors. In two cases islands of inactive odontogenic epithelium were identified. The ultrastructural features suggest that many lesional cells are very similar to a myofibroblast. With the avidin-biotin complex immunohistochemical technique, the lesions were tested with antibodies to S-100 protein, neuron-specific enolase, neurofilaments, glial fibrillary acid protein, keratin, desmin, muscle-specific actin, and vimentin. All the lesions were found to be positive for vimentin and muscle-specific actin, and negative for the rest. The results of our study seem to confirm the muscle-specific actin-positivity of odontogenic myxomas as previously reported.

Osseous hemangioma in the maxillofacial region (OHMF) was first reported in the literature as a clinical entity in the middle of last century. Several fatal cases of accidental hemorrhage following tooth extraction were attributed to OHMF. The discovery of X-rays and image diagnosis should have facilitated identification of the lesion and contributed to establishing differential diagnosis with other entities. However, our review of case-reports suggests that this lesion has been misdiagnosed and mistaken for other neoplastic and non-neoplastic hemorrhagic bone lesions. The aim of this study was to analyze clinical and radiographic data obtained from our review in order to determine the diagnostic criteria for OHMF over the last twenty years. Our case study showed that OHMF occurred at a mean age of 27.3 years, and appeared at a later age in women (30.4 years). OHMF was more frequent in women with a female to male ratio of 1.6:1, although a higher ratio has been reported in the literature. The site of predilection was the premolar and molar region of the mandible (65%). Clinical diagnosis of hemangioma was performed in 7 cases only (35%); diagnosis of the remaining cases was less accurate. The most frequent radiographic feature was mixed radiodensity (70%) and radiolucency (30%). A honeycombed appearance predominated (35%). In addition, 8 cases of OHMF involved the teeth which exhibited resorption and displacement. The present study shows that image diagnosis of OHMF must be based on the critical application of increasingly complex techniques in order to define the lesion.
