Background For many years, low back pain has been both the leading cause of days lost from work and the leading indication for medical rehabilitation. The goal of the German Disease Management Guideline (NDMG) on nonspecific low back pain is to improve the treatment of patients with this condition. Methods The current update of the NDMG on non-specific low back pain is based on articles retrieved by a systematic search of the literature for systematic reviews. Its recommendations for diagnosis and treatment were developed by a collaborative effort of 29 scientific medical societies and organizations and approved in a formal consensus process. Results If the history and physical examination do not arouse any suspicion of a dangerous underlying cause, no further diagnostic evaluation is indicated for the time being. Passive, reactive measures should be taken only in combination with activating measures, or not at all. When drugs are used for symptomatic treatment, patients should be treated with the most suitable drug in the lowest possible dose and for as short a time as possible. Conclusion A physician should be in charge of the overall care process. The patient should be kept well informed over the entire course of his or her illness and should be encouraged to adopt a healthful lifestyle, including regular physical exercise.

Early Time-Restricted Feeding Improves Insulin Sensitivity, Blood Pressure, and Oxidative Stress Even without Weight Loss in Men with Prediabetes.

It is known that family history to type 2 diabetes induces anthropometric changes in various populations. Regular physical activity can induce adaptations in these subjects regularizing body composition and anthropometric parameters. The aim of this study is therefore to understand if family history to type 2 diabetes affects anaerobic performance in young male elite athletes. Forty six young male elite athletes were tested. Thirty three without family history to type 2 diabetes (FH-) and thirteen with family history to type 2 diabetes (FH+). Anthropometric parameters, body composition, physiological parameters and athletic performance were assessed. Weight (p 0,0050), BMI (p 0,0019), waist circumference (p 0,0090), hips circumference (p 0,0490) and WHR (p 0,0339) were different between the two groups, showing greater values for the FH + subjects. Body composition showed lower FM and higher FFM percentages for the FH + group compared to the FH-. Anaerobic performance tests showed differences between the groups highlighting that the FH + group had higher anaerobic performance values (Wingate test for FH + 512,77 ± 107,93 W vs Wingate test for FH- 447,94 ± 56,95 W). The elite athletes with FH + showed better anaerobic performances and a higher body mass. At this stage we cannot generalise, but in a sample of athletes we tested, all who were with FH + showed both interesting and unexpected results; if confirmed, this evidence may represent a remarkable knowledge for fitness coaches and professionals who are daily dealing with track and field athletes and their performances.

The surprising influence of family history to type 2 diabetes on anaerobic performance of young male élite athletes

There is insufficient evidence in the literature about many correlations between motor and cognitive skills in 3-5 year old children [1]. The aim of this study was to explore the relationship between the development of the gross motor skills and the prerequisites of reading/writing following a ludic-motor program (LMP) in pre-school children. This study has involved 189 children (age: 4.62 ± 0.97 years; height: 107.83 ± 7.82 cm, body weight: 19.84 ± 4.95 kg) attending 8 kindergartens in Palermo. The children were randomly divided in a control group (C, n= 29), a 1-intervention group (I-1, n= 120) and a 2-intervention group (I-2, n= 40). I-1 and I-2 respectively performed 4 and 10 hours/week of a 16-week LMP carried out by outside experts; while C children do not perform any LMP. This program was planned in 21 learning modules aimed to develop bodily schemes, basic motor skills, fine motor control and coordination abilities. Before and after the LMP, locomotor and object control skills were evaluated with the Test of Gross Motor Development, while early reading/writing skills with the PRCR-2 test. Analyses of covariance were performed to compare outcomes for I1, I2 and C groups at post-test and the covariate was the participants’ measure of cognitive skills at pre-test. Statistical significance was defined at p<0.05. I-1 and I-2 groups showed a significant increase in both motor skills compared with C group after the LMP. A significant decrease in the number of errors concerning the serial work skills from left to right, the visual analysis and memory was found in I-1 and I-2 groups compared with C group following the LMP. Moreover, in I-2 group we observed a positive correlation between the pre-requisites of writing/reading and the Quotient of Gross-Motor Ability. The level of motor skills of children of 3-5 years is dependent on the amount of structured physical activity executed. More the level of motor skills reached is high more is correlates with a greater learning of cognitive skills. Therefore, this activity may be a tool to encourage academic achievement of all children and, even more, of those with specific learning disorders.This work was supported by a grant from the Municipality of Palermo

Effects of a ludic-motor program on motor development and early literacy skills in preschool children

It is known amateur female athletes show an altered redox status [1] and the consumption of Opuntia Ficus Indica (OFI) decreases oxidative stress (OS) in healthy humans [2]. Therefore, the aim of this study was to investigate whether the supplementation with Sicilian OFI juice affected plasma redox balance following a maximal effort test in young physically active women 			. This study was randomized, double blind, placebo controlled and crossover design. Eight women (23.25±2.95 years old, weight of 54.13±9.05 kg, height of 157.75±0.66 cm and BMI of 21.69±0.66 kg/m2) were randomly divided into 2 groups and each group was supplied with either 50 ml OFI, diluted to 170 ml with water, or 170 ml Placebo containing the same concentration of fruit juice ingredients except for Vitamin C and indicaxanthin. They consumed OFI or Placebo every day for 3 days before of effort test on cycle ergometer and continued to take it for 2 consecutive days after testing. Blood samples were taken before and after the effort test without supplementation (baseline), pre- and post-test, 24 h and 48 h post-test with OFI or Placebo supplementation. H2O2 levels and total antioxidant capacity (PAT) were measured with photometer and resonance Raman spectroscopy [1,2]. The differences within and between groups were calculated with ANOVA analysis and considered significant with P<0.05. OFI group showed a significant lower quantity of H2O2 than Placebo group after the effort test. PAT levels of OFI group were significantly higher than pre/post those of baseline and 48 h post-test of Placebo group. In conclusion, OFI supplementation might to be used to restore redox balance after intense exercises in moderately trained women.

Plasma redox response of Sicilian Opuntia Ficus Indica juice in young physically active women.

The purpose of this study was to assess the effects of a specific flexibility training program on thoracic spinal range of motion (T-ROM) in female older subjects. Thirty older women were randomized into either a trained group [TG] (n: 17; age: 68.35±6.04 years; height: 1.54±0.06 m; weight: 64.78±10.16, kg, BMI: 27.28±3.08) and a control group [CG] (n: 13; age: 69.69±7.94 years; height: 1.57±0.06 m; weight: 68.42±8.18 kg, BMI: 27.88±2.81). Trained subjects were trained for 8 weeks by two sessions/week. In particular, every trained session included: a warm up period (~15 min), a training period (~60 min) including specific exercises to train spinal flexibility, cool down period (~15 min). Control subjects did not perform any programmed physical activity during the experimental period. Data were obtained before and after the experimental phase. Spinal ranges of motion were measured using SpinalMouse® (Idiag, Volkerswill, Switzerland), which is an electronic computer-aided device that measures sagittal spinal ROM and inter-segmental angles non-invasively (Imagama et al. 2011). Each angle was measured three times in a neutral standing (nS) position, maximum extension (maxE) position and maximum bending (maxB) one, and average data were used. We found a significant increase in thoracic ROM from nS to maxB position (p 0.05). In conclusion, our findings indicate that used flexibility training protocol performed for eight weeks can improve the thoracic ROM from nS to maxB position in elderly people. In agreement with Imagama et al. (2011), we suggest that an increase in T-ROM could have positive consequences on quality of life and activities of daily living in older women.
