Despite the growing number of publications in the field of implant dentistry, there are limited studies to date investigating the biology and metabolism of bone healing around dental implants and their implications in peri-implant marginal bone loss. The aim of this review article is to provide a thorough understanding of the biological events taking place during osseointegration and the subsequent early and late phases of bone remodeling around dental implants. An update on the coupling mechanism occurring during bone resorption-bone remodeling is provided, focused on the relevance of the osteocytes, bone lining cells and immune cells during bone maintenance. An electronic and manual literature search was conducted by three independent reviewers in several databases, including MEDLINE, EMBASE, Cochrane Central Register of Controlled Trials, and Cochrane Oral Health Group Trials Register databases for articles up to September 2016 with no language restriction. Local bone metabolism is subject to signals from systemic calcium-phosphate homeostasis and bone remodeling. Three areas of interest were reviewed due to recent reported compromises in bone healing including the putative effects of (1) cholesterol, (2) hyperlipidemia, and (3) low vitamin D intake. Moreover, the prominent influence of osteocytes and immune cells is discussed as being key regulators during dental implant osseointegration and maintenance. These cells are of crucial importance in the presence of biofilm accumulation and their associated byproducts that leads to hard and soft tissue breakdown; the so called peri-implantitis. Factors that could negatively impact osteoclastogenesis or osteal macrophage activation should be monitored in future research including implant placement/torque protocols, bone characteristics, as well as meticulous maintenance programs to favor osseointegration and future long-term stability and success of dental implants. © 2016 Wiley Periodicals, Inc. Basis of bone metabolism around dental implants during osseointegration and peri-implant bone loss.

Objective
To evaluate the bacterial microflora present inside the implant connection and in the peri-implant sulcus fluid of healthy implants, and to analyze the relationships between these harboring sites for four different implant systems after at least 5 years of functional loading.

Materials and methods
A cross-sectional study was performed involving 40 patients treated with metal-ceramic cemented bridges supported by at least two healthy implants functionally loaded for 5 years. Four different implant-abutment connections were studied: external hexagon (control group), double internal hexagon (test group 1), internal hexagon with external collar (test group 2), and conical connection (test group 3). Samples for microbiological analysis were obtained from three types of sites: peri-implant sulci, connections' inside and abutments surface and, as control, gingival sulci of neighboring teeth. Quantitative real-time PCR was carried out for Total Bacterial Count and for 10 microorganisms: Aggregatibacter actinomycetemcomitans, Porphyromonas gingivalis, Tannerella forsythensis, Treponema denticola, Prevotella intermedia, Peptostreptococcus micros, Fusobacterium nucleatum, Campylobacter rectus, Eikenella corrodens, and Candida albicans. The response variables were percentage of positive sites and absolute bacterial load. The relations of the response variables with the type of connection and of sampling site were assessed using generalized estimating equations.

Results
Regarding the analysis of positivity to bacteria in the peri-implant sulcus no significant differences were observed. Analyzing the connection's inside, none of the connection designs had the capacity to prevent microbiological leakage through the implant/abutment microgap. Test group 3 presented the lowest mean values for red complex bacteria and control group the highest, although differences were non-significant. Statistical significance was only reached for Treponema denticola in the bacterial load analysis inside the connection. Test groups 1 and 2 yielded lower values for orange complex bacteria but only for Peptostreptococos micros the differences resulted significant. Test groups 2 and 3 had significantly lower total bacterial counts in the peri-implant sulcus and inside the connection.

Conclusions
Outcomes suggested that all the analyzed connections resulted contaminated after 5 years of functional loading. However, the connection design might influence bacterial activity levels qualitatively and quantitatively, especially inside the implant connection.

Microbiological assessment of the implant‐abutment interface in different connections: cross‐sectional study after 5 years of functional loading

Purpose Different implant-abutment connections are available and it has been claimed they could have an effect on marginal bone loss. The aim of this review is to establish if implant connection configuration influences peri-implant bone loss (PBL) after functional loading. Methods A specific question was formulated according to the Population, Intervention, Control, and Outcome (PICO): Does the type of implant-abutment connection (external, internal, or conical) have an influence on peri-implant bone loss? A PubMed/MEDLINE electronic search was conducted to identify English language publications published in international journals during the last decade (from 2006 to 2016). The search was conducted by using the Medical Subject Headings (MeSH) keywords "dental implants OR dental abutment AND external connection OR internal connection OR conical connection OR Morse Taper." Selected studies were randomized clinical trials and prospective studies; in vitro studies, case reports and retrospective studies were excluded. Titles and abstracts and, in the second phase, full texts, were evaluated autonomously and in duplicate by two reviewers. Results A total of 1649 articles were found, but only 14 studies met the pre-established inclusion criteria and were considered suitable for meta-analytic analysis. The network meta-analysis (NMA) suggested a significant difference between the external and the conical connections; this was less evident for the internal and conical ones. Platform-switching (PS) seemed to positively affect bone levels, non-regarding the implant-connection it was applied to. Conclusions Within the limitations of this systematic review, it can be concluded that crestal bone levels are better maintained in the short-medium term when internal kinds of interface are adopted. In particular, conical connections seem to be more advantageous, showing lower peri-implant bone loss, but further studies are necessary to investigate the efficacy of implant-abutment connection on stability of crestal bone levels.

The influence of implant-abutment connection to peri-implant bone loss: A systematic review and meta-analysis.

Two-piece implant systems are mainly used in oral implantology involving an osseointegrated implant connected to an abutment, which supports prosthetic structures. It is well documented that the presence of microgaps, biofilms and oral fluids at the implant-abutment connection can cause mechanical and biological complications. The aim of this review paper was to report the degradation at the implant-abutment connection by wear and corrosion processes taking place in the oral cavity. Most of the retrieved studies evaluated the wear and corrosion (tribocorrosion) of titanium-based materials used for implants and abutments in artificial saliva. Electrochemical and wear tests together with microscopic techniques were applied to validate the tribocorrosion behavior of the surfaces. A few studies inspected the wear on the inner surfaces of the implant connection as a result of fatigue or removal of abutments. The studies reported increased microgaps after fatigue tests. In addition, data suggest that micromovements occurring at the contacting surfaces can increase the wear of the inner surfaces of the connection. Biofilms and/or glycoproteins act as lubricants, although they can also amplify the corrosion of the surfaces. Consequently, loosening of the implant-abutment connection can take place during mastication. In addition, wear and corrosion debris such as ions and micro- and nanoparticles released into the surrounding tissues can stimulate peri-implant inflammation that can lead to pathologic bone resorption.

Synergistic interactions between corrosion and wear at titanium‐based dental implant connections: A scoping review

Objectives
Rehabilitation with implant-supported fixed prostheses is a predictable modality to restore lost function and esthetics; however, fixed restorations are subject to biological and prosthetic complications, which may represent a problem in the long-term. The aim of this study was to evaluate the long-term survival and complication rates of fixed restorations supported by Morse-taper connection implants.

Materials and Methods
Between January 1992 and December 2002, 49 patients (age range 22–70 years), were included in this study. The restorations involved 58 fixed reconstructions (15 single crowns [SCS], 29 partial prostheses, 14 full-arches), supported by 178 Morse-taper connection implants with a follow-up ranging from 10 to 20 years. Outcomes such as implant survival, marginal bone loss, frequency of biological and prosthetic complications as well as “complication-free” survival of restorations were investigated.

Results
The 20-year overall cumulative implant survival was 97.2%. A few biological (3.4%) and prosthetic (10.3%) complications were reported. The “complication-free” survival rate of restorations was 85.5%. No statistically significant differences were observed among patients' gender, age, smoking or parafunctional habits, prosthesis site and type.

Conclusions
Satisfactory “complication-free” survival rates can be achieved after 20 years for fixed restorations supported by Morse-taper connection implants, with minimal marginal bone loss and complications.

Fixed restorations supported by Morse‐taper connection implants: a retrospective clinical study with 10–20 years of follow‐up

In this systematic review, we aimed to compare conical versus nonconical implant-abutment connection systems in terms of their in vitro and in vivo performances. An electronic search was performed using PubMed, Embase, and Medline databases with the logical operators: "dental implant" AND "dental abutment" AND ("conical" OR "taper" OR "cone"). Names of the most common conical implant-abutment connection systems were used as additional key words to detect further data. The search was limited to articles published up to November 2012. Recent publications were also searched manually in order to find any relevant studies that might have been missed using the search criteria noted above. Fifty-two studies met the inclusion criteria and were included in this systematic review. As the data and methods, as well as types of implants used was so heterogeneous, this mitigated against the performance of meta-analysis. In vitro studies indicated that conical and nonconical abutments showed sufficient resistance to maximal bending forces and fatigue loading. However, conical abutments showed superiority in terms of seal performance, microgap formation, torque maintenance, and abutment stability. In vivo studies (human and animal) indicated that conical and nonconical systems are comparable in terms of implant success and survival rates with less marginal bone loss around conical connection implants in most cases. This review indicates that implant systems using a conical implant-abutment connection, provides better results in terms of abutment fit, stability, and seal performance. These design features could lead to improvements over time versus nonconical connection systems.
