TL;DR: S. pneumoniae is the most frequent pathogen to cause AOM in infants younger than three months that were not vaccinated against Streptococcus pneumoniae and empirical treatment with amoxicillin or ceftriaxone should be considered depending on clinical suspicion of severe invasive infection.
