Emotional Labour and Wellbeing: What Protects Nurses?

In this 14-month ethnographic study, I examined the emotional labor and coping strategies of 114, level-4, neonatal intensive care unit (NICU) nurses. Emotional labor was an underrecognized component in the care of vulnerable infants and families. The nature of this labor was contextualized within complex personal, professional, and organizational layers of demand on the emotions of NICU nurses. Coping strategies included talking with the sisterhood of nurses, being a super nurse, using social talk and humor, taking breaks, offering flexible aid, withdrawing from emotional pain, transferring out of the NICU, attending memorial services, and reframing loss to find meaning in work. The organization had strong staffing, but emotional labor was not recognized, supported, or rewarded. The findings can contribute to the development of interventions to nurse the nurse, and to ultimately facilitate NICU nurses' nurturance of stressed families. These have implications for staff retention, job satisfaction, and delivery of care.

The Need to Nurse the Nurse: Emotional Labor in Neonatal Intensive Care

The shortage of nurses is known. We explored nurses' reasons for leaving the profession based on responses from an e-newsletter of the Nurses and Midwives e-cohort Study. Qualitative content analysis of data from email responses (n=66) showed 'Lack of support' as a social work value describing their manager's lack of support, unsupportive relationships within their work group, and a health-care system putting business principles before care resulting in job dissatisfaction and nurse turnover. These findings are examples of a 'complex'/'wicked' problem and as such will resist simple solutions to the presenting issues. The dialogic process such as Open Space Technology and Talking Circles should be considered as potentially suitable to the needs of nurses.

Aim: 

The aim of the present study was to investigate the effects of job demands and
resources as well as emotional labour on job satisfaction and emotional exhaustion
among nurses. Background: 

While emotional labour is a construct that has considerable significance
in health care as nurses often need to express organizationally desired
emotions, little research has investigated the relationships between emotional
labour, job demands and resources in the prediction of nurses well-being.
Methods The questionnaire was distributed to 450 registered nurses (RN) working
in a teaching hospital in Taiwan during February 2007, of which 240 valid
questionnaires were returned and analysed (53.33% response rate). In addition to
descriptive statistics and correlation, structural equation modelling (LISREL 8.8)
was conducted. Results: 

The findings showed that the frequency of interacting with difficult patients
positively related to surface acting. Perceived organizational support (POS) positively
related to deep acting and negatively to surface acting. The results also showed that
surface acting related negatively, and deep acting related positively, to job satisfaction.
The frequency of interactions with difficult patients related positively to emotional
exhaustion, and negatively to job satisfaction. Perceived organizational support
related negatively to emotional exhaustion and positively to job satisfaction. Conclusions: 

The results suggest that job demands, resources and emotional labour
can predict nurses well-being. Implications for Nursing Management: 

The results of the present study indicate that
nurses well-being can be predicted by job demands, resources and emotional
labour. There is a need to address organizational support and training programmes
to enhance job satisfaction and reduce emotional exhaustion among nurses.
