Oxytalan connective tissue fibers are a separate and distinct fiber type. Although current histochemical methods cannot distinguish pre-elastic from oxytalan fibers, the two fiber types are readily distinguished by electron microscopy. Oxytalan fibers are found in periodontal membranes of all teeth of man, monkeys, rats, guinea pigs and mice. Increased numbers and size of oxytalan fibers are observed in periodontal membranes of teeth subjected to increased stress, such as those used for bridge abutments. Edwards (1968) observed increased size and number of oxytalan fibers in periodontal membranes of dog incisors subjected to orthodontic forces. Some oxytalan fibers serve to support the blood and lymphatic vessels leading to the teeth. Oxytalan fibers appear to have a protein component and a stainable component digestible with beta-glucuronidase after peracetic acid digestion. Oxytalan fibers develop in repair tissues of the periodontal membrane. Although oxytalan fibers probably develop in relation to tumors developed from dental tissues, electron microscopy must be employed to distinguish oxytalan from developing elastic tissues inasmuch as histochemical methods are inadequate.

Oxytalan connective tissue fibers: a review.

A prospective study was conducted of the effects of adenoidectomy and adenotonsillectomy on established otitis media with effusion unresponsive to medical treatment. The operations were performed at random with a controlled no surgery group on a cohort of 103 children with the condition and the results assessed six weeks, three months, six months, nine months, and one year later. After adenoidectomy the rate of resolution of the condition increased from 39% at six weeks to 72% at one year; and after adenotonsillectomy the rate increased from 59% at six weeks to 62% at one year. In the no surgery group the rate increased from 16% at six weeks to 26% at one year. Compared with the no surgery group the effect of adenoidectomy alone at one year was highly significant (p less than 0.001), and similarly the effect of adenotonsillectomy was significant (p less than 0.01). There was, however, no increased benefit from the addition of tonsillectomy compared with adenoidectomy alone. Thus there was resolution of 36-46% of chronic effusions as a result of adenoidectomy.

Chronic otitis media with effusion (glue ear) and adenotonsillectomy: prospective randomised controlled study.

The "gusher" is a dramatic complication of stapes surgery. The smallest control hole in the foot plate can produce a flow of perilymph (cerebrospinal fluid) so profuse as to fill the middle ear and external auditory canal within seconds. What is the origin of this abnormal communication between the perilymphatic and subarachnoid spaces? The purpose of this paper is to: (1) Review the experience of others with spontaneous cerebrospinal otorrhea in general and the stapes "gusher" in particular. (2) Present a clinical investigation in which some new evidence can be set forth as to the origin of the abnormal communication. (3) Propose a plan for investigating the entity on a large scale basis.

Seven cases of ocular complications arising from primary disease in the paranasal sinuses are presented and discussed. The compartmentalization of medical knowledge and the alteration in the natural course of disease by widespread antibiotic usage are factors contributing to delayed recognition of these conditions. Prompt diagnosis, massive doses of antibiotics, and timely surgical drainage are the important therapeutic features.

The results of ninety-five revision operations for post-stapedectomy conductive deafness are reported and discussed. They show that closure, or near-closure, of the air-bone gap can be obtained in about 60 per cent of patients. This figure falls to about 40 per cent for second and subsequent revision operations. There is little danger of causing a further deterioration in hearing.
