Automatic pediatric congenital heart disease classification based on heart sound signal

The phonocardiogram (PCG) signal deciphers the mechanical activity of the heart, and it consists of the fundamental heart sounds (FHSs) (S1 and S2), murmurs, and other associated sounds (S3 and S4). Detection of FHS activity (FHSA) is vital for the automated analysis of PCG signals to diagnose various heart valve diseases. This article proposes a time–frequency-domain (TFD) deep neural network (DNN) approach for automated FHSA detection using PCG signals. The modified Gaussian window-based Stockwell transform (MGWST) is used to obtain the time–frequency representation (TFR) of PCG signals. The Shannon–Teager–Kaiser energy (STKE), smoothing, and thresholding techniques are then employed to evaluate the segmented heart sound components. The TFD Shannon entropy (TFDSE) features are computed from the segmented heart sound components of the PCG signal. The DNN developed based on the stacked autoencoders (SAEs) is used for the automated identification of FHSA components. The performance of the proposed approach is evaluated using two publicly available standard databases (Database 1: Michigan heart sound and murmur database and Database 2: PhysioNet Computing in Cardiology Challenge 2016). The results demonstrate that the proposed approach has achieved the accuracy, sensitivity, specificity, and precision values of 99.55%, 99.93%, 99.26%, and 99.02% for Database 1 and 95.43%, 97.92%, 98.32%, and 97.60% for Database 2, respectively. It is shown that the proposed FHSA detection approach has obtained better accuracy than existing methods.

Automated Heart Sound Activity Detection From PCG Signal Using Time–Frequency-Domain Deep Neural Network

This research was aimed at exploring the diagnostic and screening effect of composite echocardiography based on the artificial intelligence (AI) segmentation algorithm on fetal congenital heart disease (CHD) during pregnancy, so as to reduce the birth rate of newborns with CHD. A total of 204 fetuses with abnormal heart conditions were divided into group II, group C (optimized with the AI algorithm), and group W (not optimized with the AI algorithm). In addition, 9,453 fetuses with normal heart conditions were included in group I. The abnormal distribution of fetal heart and the difference of cardiac Z score between group II and group I were analyzed, and the diagnostic value of group C and group W for CHD was compared. The results showed that the segmentation details of the proposed algorithm were better than those of the convolutional neural network (CNN), and the Dice coefficient, precision, and recall values were higher than those of the CNN. In fetal CHD, the incidence of abnormal ultrasonic manifestations was ventricular septal defect (98/48.04%), abnormal right subclavian artery (29/14.22%), and persistent left superior vena cava (25/12.25%). The diagnostic sensitivity (75.0% vs. 51.5%), specificity (99.6% vs. 99.2%), accuracy (99.0% vs. 98.2%), negative predictive value (88.5% vs. 78.5%), and positive predictive value (99% vs. 57.7%) of echocardiography segmentation in group C were significantly higher than those in group W. To sum up, echocardiography segmented by the AI algorithm could obviously improve the diagnostic efficiency of fetal CHD during gestation. Cardiac ultrasound parameters of children with CHD changed greatly.

Adoption of Compound Echocardiography under Artificial Intelligence Algorithm in Fetal Congenial Heart Disease Screening during Gestation

This study aimed to evaluate the capability of a piezoelectric sensor to detect a heart murmur in patients with congenital heart defects. Heart sounds and murmurs were recorded using a piezoelectric sensor and an electronic stethoscope in healthy neonates (n = 9) and in neonates with systolic murmurs caused by congenital heart defects (n = 9) who were born at a hospital. Signal data were digitally filtered by high-pass filtering, and the envelope of the processed signals was calculated. The amplitudes of systolic murmurs were evaluated using the signal-to-noise ratio and compared between healthy neonates and those with congenital heart defects. In addition, the correlation between the amplitudes of systolic murmurs recorded by the piezoelectric sensor and electronic stethoscope was determined. The amplitudes of systolic murmurs detected by the piezoelectric sensor were significantly higher in neonates with congenital heart defects than in healthy neonates (p < 0.01). Systolic murmurs recorded by the piezoelectric sensor had a strong correlation with those recorded by the electronic stethoscope (ρ = 0.899 and p < 0.01, respectively). The piezoelectric sensor can detect heart murmurs objectively. Mechanical improvement and automatic analysis algorithms are expected to improve recording in the future.

Detection of Pathologic Heart Murmurs Using a Piezoelectric Sensor.

The machine learning (ML)-based classification models are widely utilized for the automated detection of heart diseases (HDs) using various physiological signals such as electrocardiogram (ECG), magnetocardiography (MCG), heart sound (HS), and impedance cardiography (ICG) signals. However, ECG-based HD identification is the most common one used by clinicians. In the current investigation, the ECG records or subjects have been sampled and are used as inputs to the classification model to distinguish between normal and abnormal patients. The study has employed an imbalanced number of ECG samples for training the various classification models. Few ML methods such as support vector machine (SVM), logistic regression (LR), and adaptive boosting (AdaBoost) which have been rarely used for HD detection have been selected. The performance of the developed model has been evaluated in terms of accuracy, F1-score, and area under curve (AUC) values using ECG signals of subjects given in publicly available (PTB-ECG, MIT-BIH) datasets. Ranking of the models has been assigned based on these performance metrics and it is found that the AdaBoost and LR classifiers stand in first and second positions. These two models have been ensembled based on the majority voting principle and the performance measure of this ensemble model has also been determined. It is, in general, observed that the proposed ensemble model demonstrates the best HD detection performance of 0.946, 0.949, and 0.951 for the PTB-ECG dataset and 0.921, 0.926, and 0.950 for the MIT-BIH dataset in terms of accuracy, F1-score, and AUC, respectively. The proposed methodology can also be employed for the classification of HD using ICG, MCG, and HS signals as inputs. Further, the proposed methodology can also be applied to the detection of other diseases.

Imbalanced ECG signal-based heart disease classification using ensemble machine learning technique

Heart auscultation is a convenient tool for early diagnosis of heart diseases and is being developed to be an intelligent tool used in online medicine. Currently, there are few studies on intelligent diagnosis of pediatric murmurs due to congenital heart disease (CHD). The purpose of the study was to develop a method of intelligent diagnosis of pediatric CHD murmurs. Phonocardiogram (PCG) signals of 86 children were recorded with 24 children having normal heart sounds and 62 children having CHD murmurs. A segmentation method based on the discrete wavelet transform combined with Hadamard product was implemented to locate the first and the second heart sounds from the PCG signal. Ten features specific to CHD murmurs were extracted as the input of classifier after segmentation. Eighty-six artificial neural network classifiers were composed into a classification system to identify CHD murmurs. The accuracy, sensitivity, and specificity of diagnosis for heart murmurs were 93%, 93.5%, and 91.7%, respectively. In conclusion, a method of intelligent diagnosis of pediatric CHD murmurs is developed successfully and can be used for online screening of CHD in children.

Intelligent Diagnosis of Heart Murmurs in Children with Congenital Heart Disease.

Purpose Children's heart sounds were denoised to improve the performance of the intelligent diagnosis. Methods A combined noise reduction method based on variational modal decomposition (VMD) and wavelet soft threshold algorithm (WST) was proposed, and used to denoise 103 phonocardiogram samples. Features were extracted after denoising and employed for an intelligent diagnosis model to verify the effect of the denoising method. Results The noise in children's phonocardiograms, especially crying noise, was suppressed. The signal-to-noise ratio obtained by the method for normal heart sounds was 14.69 dB at 5 dB Gaussian noise, which was higher than that obtained by WST only and the other VMD denoising method. Intelligent classification showed that the accuracy, sensitivity and specificity of the classification system for congenital heart diseases were 92.23, 92.42, and 91.89%, respectively and better than those with WST only. Conclusion The proposed noise reduction method effectively eliminates noise in children's phonocardiograms and improves the performance of intelligent screening for the children with congenital heart diseases.

Classification of Children's Heart Sounds With Noise Reduction Based on Variational Modal Decomposition

In modern medicine, doctors can use ultrasonic equipment to detect hand tendon slippage. However, due to the lack of obvious characteristics of the tendon on the ultrasound image, it is difficult to analyze the amount of tendon slippage when the hand is moving with eyes. In order to solve the shortage of medical image tracking, this paper proposes a real-time tracking algorithm based on target tracking by using control theory, image analysis and image processing technology. In this paper, ultrasonic equipment is used to collect the motion image data of the hand tendon, and the target tracking algorithm is used to realize the tracking of the hand tendon. Three feature points of eight image sequences are tracked, and the tracking rate is 70%. However, some tracking frames disappear and cannot be tracked. Through the image processing and improvement, the contrast is increased, the tracking effect is enhanced to a certain extent, the tracking rate remains above 75%, and the loss situation has been greatly improved.
