The aim of this study was to assess the associations of overweight and obesity with lumbar radicular pain and sciatica using a meta-analysis. We searched the PubMed, Embase, Scopus, and Web of Science databases from 1966 to July 2013. We performed a random-effects meta-analysis and assessed publication bias. We included 26 (8 cross-sectional, 7 case-control, and 11 cohort) studies. Both overweight (pooled odds ratio (OR) = 1.23, 95% confidence interval (CI): 1.14, 1.33; n = 19,165) and obesity (OR = 1.40, 95% CI: 1.27, 1.55; n = 19,165) were associated with lumbar radicular pain. The pooled odds ratio for physician-diagnosed sciatica was 1.12 (95% CI: 1.04, 1.20; n = 109,724) for overweight and 1.31 (95% CI: 1.07, 1.62; n = 115,661) for obesity. Overweight (OR = 1.16, 95% CI: 1.09, 1.24; n = 358,328) and obesity (OR = 1.38, 95% CI: 1.23, 1.54; n = 358,328) were associated with increased risk of hospitalization for sciatica, and overweight/obesity was associated with increased risk of surgery for lumbar disc herniation (OR = 1.89, 95% CI: 1.25, 2.86; n = 73,982). Associations were similar for men and women and were independent of the design and quality of included studies. There was no evidence of publication bias. Our findings consistently showed that both overweight and obesity are risk factors for lumbar radicular pain and sciatica in men and women, with a dose-response relationship.

Obesity as a Risk Factor for Sciatica: A Meta-Analysis

Intervertebral disc degeneration (IDD) is associated with lower back pain and is a global burden with severe healthcare and socioeconomic consequences. However, the underlying mechanisms of IDD remain largely unelucidated. Accumulating evidence has indicasted that newly defined gene regulators, microRNAs (miRNAs), play a vital role in neurodegenerative, pathophysiological and certain reproductive disorders. To characterize the differential miRNA expression profiles between IDD and spinal cord injury, specimens from 3 patients with IDD and 3 with spinal cord injury were selected for microarray analysis. Total RNA from these 6 specimens was extracted and subjected to global miRNA expression analysis using the Exiqon miRCURY™ LNA Array (v.16.0). The microarray data were then validated by quantitative reverse transcription polymerase chain reaction (qRT-PCR). In addition, bioinformatics analysis was performed to investigate the dysregulated miRNA target genes and signaling pathways involved. Among the miRNAs analyzed, 25 miRNAs were found to be upregulated and 26 were found to be downregulated in the IDD group compared with the spinal cord injury group. The qRT-PCR results validated the microarray data. Bioinformatics analysis indicated that the signaling pathways most likely to be controlled by these miRNAs were the phosphoinositide 3-kinase (PI3K)-Akt, mitogen-activated protein kinase (MAPK), epidermal growth factor receptor (EGFR; ErbB) and Wnt pathways. Our results demonstrated that the miRNA expression in patients with IDD differed significantly from that in patients who sustained injury to the intervertebral disc. Our data indicate that the dysregulated miRNAs control the signaling pathways important for the maintenance of IDD. Further studies on miRNA target gene identification and biological functions may address the specific regulatory mechanisms of miRNAs in IDD, and may provide valuable insight into the diagnosis and treatment of IDD.

Characterization of microRNA expression profiles in patients with intervertebral disc degeneration.

The epidemiology and pathophysiology of lumbar disc herniations

Background Lumbar disc degeneration (LDD) is a process that begins early in life, contributing to the development of low back pain. LDD is a consequence of a variety of factors, and its etiology remains poorly understood. Objectives to investigate occupational and genetic risk factors inducing lumbar disc degeneration, and to evaluate the possible association of genetic polymorphisms of matrix metalloproteinase 3 (MMP-3) and vitamin D receptor (VDR) with the severity of LDD in an Egyptian population. Subjects and Methods A case control study involving 84 LDD and 60 controls was carried out. Five types of work related factors were investigated by questionnaire, complete neurological examination for all subjects and MRI for the cases. Polymerase chain reaction and restriction fragment length polymorphism methods were applied to detect polymorphisms in MMP-3 Promoter (−1,171 6A/5A) (rs 731236) and VDR-Apa (rs 35068180). Results We found that family history, back injury, smoking, high level of sitting, bending/twisting, physical workload, lifting, whole body vibration, mutant allele 5A of MMP-3 and mutant allele T of VDR were significantly associated with LDD (OR = 2.9, 3.1, 2.1, 11.1, 15.9, 11.7, 8.2, 12.6, 2.5 and 3.1 respectively, p < 0.05). Cases that carry allele 5A and/or allele T were associated with LDD severity. Conclusion LDD is closely associated in occurrence and severity with occupational, environmental risk factors and susceptibility genes namely MMP-3, and VDR (ApaI). This study throws light on the importance of screening for early detection of susceptible individuals and disease prevention.

Matrix Metalloproteinase-3, Vitamin D Receptor Gene Polymorphisms, and Occupational Risk Factors in Lumbar Disc Degeneration

The aim of this research was to identify the differences in the incidence of symptomatic cervical and lumbar disc herniation according to age, sex, and national health insurance eligibility. We evaluated the hospital documents of patients who received medical treatment for symptomatic cervical and lumbar disc herniation between 2004 and 2010 and excluded those who claimed to have expenses at oriental medical clinics or pharmacies. Furthermore, any duplicate documents from the labor force population aged 20–69 years were excluded from the analysis. The results showed that the number of individuals diagnosed with symptomatic cervical and lumbar disc herniation increased with age, and the incidence of these diseases was higher in women than in men. Additionally, the incidence differed depending on the subject’s qualification for health insurance. The incidence of lumbar disc herniation showed differences depending on the degree of the lumbar burden. The present study findings may help determine whether lumbar disc herniation is associated with tasks performed at the patient’s workplace. Further research is needed to classify the risk of lumbar disk herniation in the workplace into detailed categories such as types of business, types of occupation, and lumbar compression force.

Differences in the Incidence of Symptomatic Cervical and Lumbar Disc Herniation According to Age, Sex and National Health Insurance Eligibility: A Pilot Study on the Disease's Association with Work.

Study design A case-control study of 4180 subjects was carried. Objective To explore the risk factors of lumbar disc herniation in China. Summary of background data In China, along with the economic development, people's living environment and working conditions have undergone some tremendous changes, such as the accelerating life pace and competition. In this situation, it is important to know whether the risk factors of lumbar disc herniation have changed or not. This case-control study, including the possible social and psychological factors based on the literature, attempted to search the new risk factor, therefore provide better prevention measures for lumbar disc herniation. Methods A total of 2010 hospitalized patients, diagnosed with lumbar disc protrusion by CT and/or MRI were selected as cases. A total of 2070 people from communities and hospitals, without history of low back pain and sciatica, were selected as controls. All patients and controls were investigated for their family history, occupational characters' smoking status, working psychosocial factors, etc. The risk factors were analyzed by multiple nonconditional logistic regression method. Results Family history (OR = 3.6) was the most important risk factor for lumbar disc protrusion in this study, followed by lumbar load (OR = 2.1), hard-working (OR = 1.8), and time urgency (OR = 1.1). Additionally, physical exercises (OR = 0.5) and bed characteristics (OR = 0.4) appeared to be the protective factors for lumbar disc protrusion. After stratified by age, family history (OR = 14.5), occupational character (OR = 5.2), and physical exercises (OR = 0.2) stronger association with lumbar disc protrusion was seen in subjects younger than 30 years. In subjects from 30 to 55 years, family history (OR = 5.1), lumbar load (OR = 1.91), hard-working (1.9), physical exercises (OR = 0.5), time urgency (OR = 1. 3), bed characteristics (OR = 0.4) were significantly important. In subjects older than 55 years, lumbar load (OR = 2.9) and bed characteristics (OR = 0.4) were closely related to lumbar disc protrusion. Conclusion Family history, lumbar load, hard-working, and time urgency are the major risk factors for lumbar disc herniation, and physical exercises and sleeping on the hard bed might be the protective factors.

Risk factors for lumbar intervertebral disc herniation in Chinese population: a case-control study.

To explore the etiologic role of apoptosis-related genes, environmental risk factors, and their interaction in the occurrence of lumbar disk herniation (LDH), a controlled case study was performed with 128 LDH patients and 132 age- and sex-matched controls. Matrix-assisted laser desorption/ionization, time-of-flight mass spectrometry assay was used to analyze the genotype of nine polymorphism sites in three genes, including Fas –1377G/A rs2234767, Fas –670G/A rs1800682, Fas rs2147420, Fas rs2296603, Fas rs7901656, Fas rs1571019, Fas ligand ( FasL ) –844C/T rs763110, caspase 9 ( CASP9 ) –1263A>G rs4645978, and CASP9 –712C>T rs4645981. The patients and controls showed similar age and sex, but had significant differences in lumbar load, bed type, amateur sports, and leisure activities ( P FasL −844C/T (rs763110) and CASP9 –1263A>G (rs4645978) had a significant correlation with LDH, indicating that the genotypes of FasL −844C/T TT and CASP9 –1263A>G GG are probably high-risk genotypes for LDH. The results of environment-gene interaction analysis revealed that, in LDH, the interaction of the FasL −844TT genotype and level III to IV lumbar load was consistent with the ultramultiplying model, and the interaction of the CASP9  rs4645978 GG genotype and level III to IV lumbar load was consistent with the submultiplicative model. Therefore, the risk of LDH was determined by both environmental and genetic risk factors, and the mechanisms of interactions between different genotypes and environmental factors also differed.

A Controlled Case Study of the Relationship between Environmental Risk Factors and Apoptotic Gene Polymorphism and Lumbar Disc Herniation

BACKGROUND
Recurrence of lumbar disc herniation (LDH) is an adverse event after percutaneous endoscopic transforaminal discectomy (PETD). Accurate prediction of the risk of recurrent LDH (rLDH) after surgery remains a major challenge for spine surgeons.


OBJECTIVES
To develop and validate a prognostic model based on risk factors for rLDH after PETD.


STUDY DESIGN
Retrospective study.


SETTING
Inpatient surgery center.


METHODS
Clinical data were retrospectively collected from 645 patients with LDH who underwent PETD at the Affiliated Hospital of Xuzhou Medical University from January 1, 2017 to January 1, 2021. Predictors significantly associated with rLBH were screened according to least absolute shrinkage and selection operator (LASSO) regression, and a prognostic model was established, followed by internal model validation using the enhanced bootstrap method. The performance of the model was assessed using receiver operating characteristic (ROC) curves and calibration curves. Finally, the clinical usefulness of the model was analyzed using decision curve analysis (DCA) and clinical impact curves (CICs).


RESULTS
Among the 645 patients included in this study, 56 experienced recurrence of LDH after PETD (8.7%). Seven factors significantly associated with rLDH were selected by LASSO regression, including age, type of herniation, level of herniation, Modic changes, Pfirrmann classification, smoking, and history of high-intensity physical work. The bias-corrected curve of the model fit well with the apparent curve, and the area under the ROC curve was 0.822 (95% confidence interval, 0.76-0.88). The DCA and CIC confirmed that the prognostic model had good clinical utility.


LIMITATIONS
This is a single-center study, and we used internal validation only.


CONCLUSIONS
The prognostic model developed in this study had excellent comprehensive performance and could well predict the risk of rLDH after PETD. This model could be used to identify patients at high risk for rLDH at an early stage to individualize the patient's treatment modality and postoperative rehabilitation plan.

Development and Validation of a Prognostic Model for the Risk of Recurrent Lumbar Disc Herniation After Percutaneous Endoscopic Transforaminal Discectomy.

Image-defined risk factors (IDRF) in neuroblastoma have been developed to predict tumor resectability and surgical complications; however, the potential prognostic value of IDRF in neuroblastoma has been variably reported. Previous studies did not report the IDRF status separately from the International Neuroblastoma Risk Group (INRG) stage. Moreover, the association between IDRF and clinical and pathological factors has not been discussed further. In this retrospective study, we investigated the clinical and biological features of neuroblastoma at different INRG stages based on IDRF. Event-free survival (EFS) and overall survival (OS) related to the INRG stage were analyzed using log-rank tests, and the prognostic value of the IDRF number and type was also evaluated. Among 72 patients, 182 IDRF at diagnosis were found in 79.2%. The distribution of the INRG stages was 10 L1 (13.9.0%), 25 L2 (34.7%), and 37 M/MS (51.4%). Patients with stage M/Ms had a larger tumor volume, a higher percentage of age ≥ 18 months, elevated lactate dehydrogenase (LDH) level, elevated ferritin level, and a higher percentage of COG high-risk compared with stage L1 and L2 patients. EFS and OS were similar for stage L1 and L2 tumors but were significantly poorer for metastatic disease. However, EFS (P = 0.06) and OS (P = 0.07) were similar for IDRF-negative and positive neuroblastomas. Patients with stage M/Ms with IDRF-positive had poorer EFS (P = 0.001) and OS (P < 0.001) compared with patients in stage L2. An IDRF ≥ 4, vascular IDRF, and infiltrative IDRF of the tumor were significant indicators of poor prognosis. Conclusion: Our study indicates that increasing the INRG stages based on IDRF is associated with various unfavorable clinical features of neuroblastoma. The principal determinant of survival in neuroblastoma is the presence of metastatic disease more than IDRF alone at diagnosis. Both the number and type of IDRF have important clinical significance in the protocol planning of neuroblastoma, rather than just considering the absence or presence of IDRF.
