Evidence-based guidelines for the management of patients with methicillin-resistant Staphylococcus aureus (MRSA) infections were prepared by an Expert Panel of the Infectious Diseases Society of America (IDSA). The guidelines are intended for use by health care providers who care for adult and pediatric patients with MRSA infections. The guidelines discuss the management of a variety of clinical syndromes associated with MRSA disease, including skin and soft tissue infections (SSTI), bacteremia and endocarditis, pneumonia, bone and joint infections, and central nervous system (CNS) infections. Recommendations are provided regarding vancomycin dosing and monitoring, management of infections due to MRSA strains with reduced susceptibility to vancomycin, and vancomycin treatment failures.

Central venous catheters permit the measurement of hemodynamic variables that cannot be measured accurately by noninvasive means. They also allow delivery of medications and nutritional support. More than 15 percent of patients, however, have a serious mechanical, infectious, or thrombotic complication related to the use of a central venous catheter. Clinical practice guidelines for antimicrobial prophylaxis in surgery

After follow-ups ranging from two to five years on all but four (five hips) of 2,694 patients who had 3,215 total hip arthroplasties, deep wound infection had been demonstrated in forty-two hips (1.3 per cent). The infections among the 3,210 hips appeared during the immediate postoperative period or as long as five years after surgery. All operations were performed in conventional operating rooms. Previous operations, prolonged operating time, positive culture at operation, and unrecognized preoperative sepsis were related to the development of deep infection. In only eight of the forty-one patients (forty-two hips) was salvage of the prosthetic arthroplasty possible. The deaths of tree patients were directly attributable to the infection or its treatment.

Prosthetic Valve Endocarditis

Blood-culture procedures must be designed to overcome the intermittency and low order of magnitude of most bacteremias and fungemias and to inhibit any antimicrobial properties or components of the blood. Among the several variables affecting yields, the volume of blood cultured appears to be most important. It is recommended that at least 10 ml, and preferably 20-30 ml, of blood be obtained for each of two to three separate cultures. More than three separate blood cultures per septic episode is rarely necessary. Other issues involve the systems used for blood culture and the procedures used for their examination.

The minimal inhibitory concentrations of 601 clinical isolates of anaerobic bacteria to 10 different antimicrobial agents were determined by an agar-dilution technique. Nearly all strains were resistant to kanamycin and gentamicin, although moderate activity to both drugs was noted with Fusobacterium sp., anaerobic cocci, some strains of Bacteroides melaninogenicus , and nonsporeforming gram-positive bacilli. Chloramphenicol at 12.5 μg/ml inhibited all but three of the strains tested. Tetracycline at 6.25 μg/ml had high activity against all groups tested, with the exception that only 39% of strains of Bacteroides fragilis were inhibited at this concentration. Excluding certain species of Bacteroides , the majority of anaerobes were inhibited by penicillin at 3.1 μg/ml or less and by cephalothin at 12.5 μg/ml or less. Lincomycin at 6.2 μg/ml or less was active against nearly all strains. Erythromycin at a concentration of 3.1 μg/ml was active against B. fragilis ; however, erythromycin was less active against the other groups. Most of the minimal inhibitory concentrations of lincomycin exceeded those of clindamycin by fourfold. Rifampin inhibited virtually all strains at 3.1 μg/ml.

In Vitro Antimicrobial Susceptibility of Anaerobic Bacteria Isolated from Clinical Specimens

Of 83 patients who underwent 100 orthotopic liver transplantations, 53 had a single transplant procedure and at least 6 months of follow-up. In this main study group of 53 patients, major infections developed in 28 (53%) (a mean of 1.8 major episodes per infected patient). Of 51 major infections, 27 were bacterial, 19 were viral, 3 were protozoan, and 2 were fungal. Of the 27 bacterial infections, 22 (81%) occurred in the first 2 months after transplantation. Of the 40 bacterial isolates in the 27 bacterial infections, gram-positive aerobic bacteria were isolated in 26 (65%), anaerobic bacteria in 8 (20%), and aerobic gram-negative bacteria in 6 (15%). Only 1 of 16 bacteremic episodes was due to a gram-negative aerobic bacterium. Cytomegalovirus (CMV) infection occurred in 30 of the 53 patients (57%) and was symptomatic and invasive in 18. CMV infection was diagnosed a mean of 26 days after transplantation. Infections due to Pneumocystis carinii occurred later (2 to 3 months after transplantation). Death from infection occurred in 4 of the 53 patients (8%). In the group of 16 patients with two or more liver transplantations, fungal infection occurred in 2 and CMV infection in 13. In all 16 patients who underwent more than one liver transplantation, a major infection developed. The observations made in the main study group were consistent with findings in 13 patients with one liver transplantation but less than 6 months of follow-up. Infection is a major complication after liver transplantation, generally occurring in the first 2 months. Our observations suggest that the use of selective bowel decontamination may be associated with a relatively lower incidence of gram-negative aerobic bacterial infections.
