Rhythms of the brain

• Recent research on schizophrenia has demonstrated that in this disorder the brain is not, strictly speaking, normal. The findings suggest that nonspecific histopathology exists in the limbic system, diencephalon, and prefrontal cortex, that the pathology occurs early in development, and that the causative process is inactive long before the diagnosis is made. If these findings are valid and not epiphenomena, then the pathogenesis of schizophrenia does not appear to fit either traditional metabolic, posttraumatic, or neurodegenerative models of adult mental illness. The data are more consistent with a neurodevelopmental model in which a fixed "lesion" from early in life interacts with normal brain maturational events that occur much later. Based on neuro-ontological principles and insights from animal research about normal brain development, it is proposed that the appearance of diagnostic symptoms is linked to the normal maturation of brain areas affected by the early developmental pathology, particularly the dorsolateral prefrontal cortex. The course of the illness and the importance of stress may be related to normal maturational aspects of dopaminergic neural systems, particularly those innervating prefrontal cortex. Some implications for future research and treatment are considered.

Implications of normal brain development for the pathogenesis of schizophrenia

To understand the endogenous components of the event-related brain potential (ERP), we must use data about the components' antecedent conditions to form hypotheses about the information-processing function of the underlying brain activity These hypotheses, in turn, generate testable predictions about the consequences of the component We review the application of this approach to the analysis of the P300 component The amplitude of the P300 is controlled multiplicatively by the subjective probability and the task relevance of the eliciting events, whereas its latency depends on the duration of stimulus evaluation These and other factors suggest that the P300 is a manifestation of activity occurring whenever one's model of the environment must be revised Tests of three predictions based on this “context updating” model are reviewed Verleger's critique is based on a misconstrual of the model as well as a partial and misleading reading of the relevant literature

The average evoked-potential waveforms to sound and light stimuli recorded from scalp in awake human subjects show differences as a function of the subject's degree of uncertainty with respect to the sensory modality of the stimulus to be presented. Differences are also found in the evoked potential as a function of whether or not the sensorymodality of the stimulus was anticipated correctly. The major waveform alteration is in the amplitude of a positive-going component which reaches peak amplitude at about 300 milliseconds.

Evoked-Potential Correlates of Stimulus Uncertainty

Although descriptive and etiological approaches to psychopathology have made notable advances, they seem to have reached a plateau. After reviewing the six approaches to etiology that now preempt the field—ecological, developmental, learning, genetic, internal environment, and neurophysiological models—a second-order model, vulnerability, is proposed as the common denominator, and methods for finding markers of vulnerability are suggested in the hope of revitalizing the field. It is assumed that exogenous and/or endogenous challengers elicit a crisis in all humans, but depending on the intensity of the elicited stress and the threshold for tolerating it, that is, one's vulnerability, the crisis will either be contained homeostatically or lead to an episode of disorder. Vulnerability and episode stand in a trait-state relation, and markers for each must be provided to distinguish between them.

The waveform of evoked responses recorded from human scalp is not determined solely by the physical eliciting stimulus, but also varies as a function of the effective information provided by the stimulus. There is a positive component whose latency is determined by the point in time at which ambiguity is reduced, and whose shape and amplitude are influenced by whether it is the presence or absence of an external event which delivers the information.

A late positive-going component (P3) of the average evoked potential recorded from human scalp was shown to be quantitatively related to a priori stimulus probability both when the S was told the identity of the stimulus before it was presented and when the S was not told, and was instructed to guess In the guessing situation, the amplitude of P3 was much larger and was influenced not only by the a priori probability of events determined by the experimenter but also by the interaction of these probabilities with the S's guessing behavior The amplitude of the late positive component was inversely related to the proportion of trials in which a particular event was associated with a particular guess, ie, the proportion of hits and misses It was larger the more unexpected the outcome of the guess This relationship held for different methods of manipulating the probability of two events
