The paper draws together a large and scattered body of empirical evidence concerning the prevalence of snail infection with schistosome parasites in field situations, the duration of the latent period of infection in snails (and its dependence on temperature), and the mortality rates of infected and uninfected snails in field and laboratory conditions. A review and synthesis of quantitative data on the population biology of schistosome infections within the molluscan host is attempted and observed patterns of infection are compared with predictions of a schistosomiasis model developed by May (1977) which incorporates differential snail mortality (between infected and uninfected snails) and latent periods of infection. It is suggested that the low levels of prevalence within snail populations in endemic areas of schistosomiasis are closely associated with high rates of infected snail mortality and the duration of the latent period of infection within the mollusc. In certain instances, the expected life-span of an infected snail may be less than the duration of the latent period of infection. Such patterns generate very low levels of parasite prevalence. A new age prevalence model for schistosome infections within snail populations is developed and its predictions compared with observed patterns.The implications of this study of observed and predicted patterns of snail infection within molluscan populations are discussed in relation to the overall transmission dynamics of schistosomiasis.

Prevalence of schistosome infections within molluscan populations: observed patterns and theoretical predictions

Background 
Programs for schistosomiasis control are advancing worldwide, with many benefits noted in terms of disease reduction. Yet risk of reinfection and recurrent disease remain, even in areas with high treatment coverage. In the search for means to better prevent new Schistosoma infections, attention has returned to an older strategy for transmission control, i.e., chemical mollusciciding, to suppress intermediate host snail species responsible for S. mansoni and S. haematobium transmission. The objective of this systematic review and meta-analysis was to summarize prior experience in molluscicide-based control of Bulinus and Biomphalaria spp. snails, and estimate its impact on local human Schistosoma infection.


Methodology/Principal Findings 
The review was registered at inception with PROSPERO (CRD42013006869). Studies were identified by online database searches and hand searches of private archives. Eligible studies included published or unpublished mollusciciding field trials performed before January 2014 involving host snails for S. mansoni or S. haematobium, with a primary focus on the use of niclosamide. Among 63 included papers, there was large variability in terms of molluscicide dosing, and treatment intervals varied from 3–52 weeks depending on location, water source, and type of application. Among 35 studies reporting on prevalence, random effects meta-analysis indicated that, on average, odds of infection were reduced 77% (OR 0.23, CI95% 0.17, 0.31) during the course of mollusciciding, with increased impact if combined with drug therapy, and progressively greater impact over time. In 17 studies reporting local incidence, risk of new infection was reduced 64% (RR 0.36 CI95% 0.25, 0.5), but additional drug treatment did not appear to influence incidence effects.


Conclusion/Significance 
While there are hurdles to implementing molluscicide control, its impact on local transmission is typically strong, albeit incomplete. Based on past experience, regular focal mollusciciding is likely to contribute significantly to the move toward elimination of schistosomiasis in high risk areas.

Systematic Review and Meta-analysis of the Impact of Chemical-Based Mollusciciding for Control of Schistosoma mansoni and S. haematobium Transmission

The increased occurrence of pollutants in ecosystems is a continuing area of concern. It is known that numerous diseases of wild aquatic animals can occur with decreased or increased prevalences in areas associated with high or chronic levels of pollution. This may have serious implications for environmental health. There has consequently been an increasing number of laboratory and field studies on disease transmission under polluted conditions, especially focusing on digeneans of medical or economic importance. The effect of pollutants to the transmission of larval digeneans (miracidia, cercariae, metacercariae) and snail-digenean interactions is therefore considered. An overview and interpretation of the published literature on laboratory and field studies is provided. It is apparent from these studies that the influence of pollutants on digenean transmission is highly complex with much of the observed effects in the laboratory often masked by a complexity of other factors in the field. Future studies would benefit from a standardisation of experimental procedures, increasing the number of combined laboratory and field studies, and increasing the complexity of the experiments undertaken.

Pollution toxicity to the transmission of larval digeneans through their molluscan hosts.

The results of a 5-year study on the distribution and ecology of Bulinus truncatus in Khuzestan, south-western Iran, are described. Live snails were found in 4 foci, namely, the main focus, the upper Karun River, the lower Karkheh River, and the Bala Rud watershed; in addition, dead snails were found in 3 other foci. The patchy distribution of snails was related to the presence of surface waters and to their chemical composition.Monthly observations on the population dynamics of the snails and on the role of the snails in the transmission of Schistosoma haematobium and S. bovis were carried out in 14 ponds, 9 canals, 5 swamps, 2 field drains, 1 spring and 1 stream-pool. In standing waters, the peak of snail population could potentially occur in 2 seasons-May-July and November-January-but the main seasons for the transmission of mammalian schistosome cercariae were April-May and October-November. In flowing waters, a higher number of snails might be found in summer in some canals and in winter in another group of canals. Canals were found to be more important sites of transmission of human schistosomes than were village ponds. Transmission took place mainly in hot months when man-water contacts were greatest. Village ponds accessible to livestock were found to be important in the transmission of bovine schistosomiasis. As new irrigation systems have been expanding in this area of Khuzestan, more attention should be paid to flowing waters than to standing waters in future schistosomiasis control programmes.
