Accessible online at: http://BioMedNet.com/karger Dear Sir, It is recognized that the maintenance of blood pressure (BP) through vascular constriction is regulated to a large extent by the combined effects of neurohormonal pressure mechanisms, such as renin angiotensin (RA), sympathetic nervous system (SNS) and arginine vasopressin (AVP) [1]. In normal subjects, RA and SNS work predominantly, thus the pressor effect of AVP is blurred. However, in subjects with disturbed RA and SNS, it has been supposed that the pressor activity of AVP plays an important role in the maintenance of BP [2, 3]. Hypotension during a hemodialysis (HD) session is one of the major complications that interrupt a HD session. Autonomic dysfunctions have been postulated as representing one of the pathogeneses of HD hypotension [4]. In addition, it is shown that the renin response to hypotension is occasionally blunted in dialysis patients [5]. The secretion of AVP is stimulated by a hypotensive episode in normal subjects. Accordingly, if AVP secretion in response to a fall in BP is impaired in patients with HD hypothension, this could be one of the pathophysiological factors exaggerating the develop-
