Abstract: Ivalon sponge has proved to be a useful embolic material, particularly when long-term or permanent occlusions are desirable. It has been stated that Ivalon is well tolerated and that it produces little if any damage to the wall or periadventitial tissues of the embolized artery. Our study demonstrates definite evidence of inflammatory reaction in the wall of the embolized arteries; these changes were seen only in the acute phase and seem to be secondary to damage of the vessel wall by the plastic material. Permanent occlusion results with disappearance of the inflammatory changes at nine months postembolization.

Abstract: Splenic-artery aneurysms are commonly found incidentally during abdominal angiography. Because approximately 10% of these aneurysms may rupture and lead to fatal hemorrhage, surgical treatment is indicated. A new, nonsurgical approach is described in which the splenic-artery aneurysm is packed with foreign matter introduced through a catheter to induce thrombosis. The procedure can be carried out under local anesthesia and is the preferred technique in high-risk patients. As more experience is gained with this procedure, it can be performed on an elective basis under local anesthesia and the patients can be spared the disadvantages of laparotomy.

Abstract: Splenoportography, a useful preoperative diagnostic procedure in patients with portal hypertension, has been largely discontinued because of the risk of hemorrhage. We have used both Ivalon and Gelfoam in particle and plug form to plug the needle tract after splenoportography. Initial studies were conducted on six dogs. Plugging of the needle tract by injection of suspensions of small Gelfoam or Ivalon particles resulted in intravasation of these particles into the splenic vein. This was demonstrated by radioactive tagging of particles and the direct collection of splenic blood during the intrasplenic injection. Plugging of the splenic needle tract with compressed Gelfoam caused immediate, complete cessation of bleeding in fully heparinized animals. Because of these experimental results, five humans with decreased platelet counts and severe portal hypertension underwent successful splenoportography without significant hemorrhage. At surgery, successful plugging of the needle tract by Gelfoam was confirmed.

Small amounts of contrast material should be administered. Hepatic angiography may result in in an area of tumor blush at the site of a previous wedge injection.

Abstract: Small nylon brushes with a stainless steel core have been found safe and effective for transcatheter arterial occlusion. Studies performed in 14 dogs demonstrate the brushes result in rapid occlusion of the renal artery. Follow-up studies for periods up to ten months document the safety and permanence of the occlusion. The animal studies reported herein suggest that these devices can be safely used in humans in cases where large artery occlusion is desired.

Abstract: Right ventricular outflow obstructive lesions are among the most prevalent congenital cardiac anomalies with a frequency ranging from 6–10% when considered as isolated anomalies. As part of a more complicated developmental complex, their frequency is undoubtedly higher. The diagnosis can be suspected by the traditional clinical and hemodynamic findings, but a firm diagnosis can only be made by angiography. An exact diagnosis is most important since most of these lesions can be surgically corrected.

Abstract: Transcatheter gastric embolization using Ivalon shavings was performed on three groups of dogs. The left gastric artery and splenic branches (the short gastric and left gastroepiploic arteries) were embolized in Group I, the left gastric artery in Group II, and the splenic branches in Group III. Infarctions of the gastric mucosa occurred only in Group I. Ischemic ulcers developed in three of the five dogs in Group I and in one of the three dogs in Group II. The study suggests that embolization can only be performed safely if particles of 0.3 mm or larger are used.

Abstract: Percutaneous decompression of the biliary system is a useful and well established procedure which can be carried out with negligible risk and may be of great benefit to the patient. The main indications are pre-operative decompression and primarily decompression of patients with inoperable obstructive neoplastic disease. In order to provide drainage, the catheter should be advanced under fluoroscopic control into one of the major radicles ore preferably into the distal common duct beyond the point of obstruction. This ideal internal drainage into the duodenum was accomplished in the reported patient with sclerosing cholangitis who was markedly improved by this procedure.

Abstract: One hundred and six patients with angina pectoris who underwent routine coronary angiography and left ventriculography were given intracoronary injection of thallium 201. The myocardial images had excellent resolution because of lack of interference from the background activity. The myocardial images and the left ventriculograms showed 94 % correlation. Correlation between scans and EKG was lower. There was no correlation between the degree of coronary artery stenosis and 201TI scan. Currently, the intracoronary injection of 201TI at rest does not yield additional information to coronary angiography and left ventriculography.

Abstract: Various embolization cases are described, and these require certain embolization materials. In our experience Gelfoam particles are very well suited for gastrointestinal tract lesions. Ivalon shavings gave excellent results in congenital arteriovenous malformations with permanent occlusions. Bristle brushes are a new device for larger artery occlusion. Stainless steel coils in combination with silk tufts proved thrombosis of large AV fistuals or of aneurysms particularly of the splenic artery.

Abstract: Variations of the scimitar syndrome have been reported previously, the most common being the absence of systemic arterial supply to the right lung and varying degrees of lung hypoplasia. Anomalous scimitar-like veins draining the entire right lung and connecting normally to the left atrium have been reported. The case presented is unique in that there is marked hypoplasia of the right bronchial tree and a large bronchogenic pulmonary cyst without dextroposition of the heart and anomalous systemic arterial supply to the right lung.

Abstract: In 8 of 18 patients with suspected diagnosis of renovascular hypertension, the resting renin values were inconclusive. However, following tourniquet stress, the values became positive. Sixteen of 18 patients showed significant decrease in blood pressure following surgery. The procedure was well tolerated, and its routine use is recommended.

Abstract: A large fusiform atherosclerotic aneurysm of the aorta with a long, thick, laminated, calcified thrombus mimicked the angiographic appearance of a dissection of the descending aorta with a 20-mm wide thrombosed false channel. Therefore, an increase in the width of the aortic wall exceeding 10 mm may not be a uniformly reliable sign for dissecting aneurysms of the descending aorta.

The method's advantages include: (a) the use of modular hardware and software, which can be extended and upgraded; (b) a minimal expenditure of time and money; (c) uniformly calculated results; and (d) a graphic and legible report.

Abstract: A new technique converts a standard biased x-ray tube into a microfocal spot tube with a clinically useful loading capacity if the tube is pivoted exactly through the focal spot, the film can be scanned with a 5 10 cm wide beam and cover a 17-inch field. Then the target angle can be decreased to a few degrees. The width of the focal spot can be decreased using a grid bias.
