Background: AUB is one of the most common problem in life of an adult female. Evaluation of histopathological pattern of endometrium is essential for appropriate management of patient with abnormal uterine bleeding. Objective –The study was carried out to evaluate different clinical presentation and to determine the histopathological pattern of Endometrium in woman with AUB in the age group of 40-60 years. Material & Methods: Analysis of 500 patients was done during a period of one year from May 2013 to April 2014. Routine paraffin sections and Haematoxylin and eosin stain has been used. Histology is gold standard for diagnosis of AUB.

Study Of “Histopathological Pattern Of Endometrium In Abnormal Uterine Bleeding In The Age Group 40-60 Years” –A Study Of 500 Cases.

Background: Current study designed to identify the causes of abnormal uterine bleeding( AUB) in perimenopausal women and also to correlate with their histopathological examination. Methods: A prospective study of 65 perimenopausal women with complaint of AUB with age group ranging from 40 to 1 year within menopause those are attending to department of gynaecology of Narayana Medical College & Hospital, Nellore. Clinical examinations between the parameters were analysed. Histopathological examination report was also correlated with clinical examination and ultrasonography. Results: Out of 65 women, 11 (16.9%) were primiparas, 36 (55.38%) were para 2, 18 (27.69%) were para 3 and above. The frequent menstrual abnormality seen in perimenopausal women is heavy menstrual bleeding i.e., 41 (63%) women out of 65 women, 14 (21.53%) had polymenorrhoea, 9 (13.84%) had oligomenorrhea, 6 (9.23%) had postmenopausal bleeding, 4 (6.15%) had intermenstrual spotting. Most common type of endometrial pattern is proliferative endometrium. Out of 65 perimenopausal women, 23 (35.38%) had proliferative type of endometrium, 20 (30.76%) women had secretory pattern, 9 (13.84%) had typical simple endometrial hyperplasia, 4 (6.15%) had atypical simple endometrial hyperplasia, 3 (4.61%) had adenomyomatous polyp, and 6 (9.2%) women had irregular ripening. Conclusions: The present study shows that Histopathological examination is mandatory in all cases of AUB in perimenopausal age. 66.14% had benign pathology which can be managed by hormonal therapy or conservative surgical modalities which alleviates need for unnecessary hysterectomy.

Histopathological correlation of abnormal uterine bleeding in perimenopausal women

Background :
Abnormal uterine bleeding (AUB) is the most common presenting complaint of patients in Gynaecology Outpatient department. Early diagnosis of endometrial hyperplasia through ultrasongraphic measurement of endometrial thickness and definite diagnosis by histopathologic examination will help the clinicians to give appropriate treatment to the patients. The objective of this study was to correlate the histopathology findings in abnormal uterine bleeding with endometrial thickness by ultrasonography.
Methods :
 This was a prospective study done from Nov 2016 to June 2018.A total of 382 cases of AUB presenting above 40 years of age were included. Endometrial thickness was measured by ultrasonography. Histopathological findings were studied and compared with the endometrial thickness. Statistical analysis were done using chi square test.
Results :
 A total of 382 cases were analysed. Maximum number of cases were from peri menopausal age group (68.32%). Majority of the cases were due to non organic causes(85.34%). 46.86% of patients had endometrial thickness of 5-10mm followed by
endometrial thickness of 11-15mm in 39.01%. Disordered proliferative endometrium was the most common pattern (38.34%) in our study followed by secretory endometrium (35.58%) and menstrual endometrium (18.71%). These findings were found to be statistically significant.
Conclusion : 
A thorough histopathological work up and clinical correlation is mandatory in cases of abnormal uterine bleeding above 40 years to find out organic lesions. Careful screening can detect precursor lesions like endometrial hyperplasia and early cases of endometrial carcinoma which has excellent prognosis and it will help in further management.

Correlation Between Endometrial Thickness By Ultrasonography and Histopathology in Abnormal Uterine Bleeding

Background: Abnormal uterine bleeding (AUB) is considered one of the most common and challenging problems presenting to the gynecologist. Histopathological examination of endometrial biopsy is gold standard diagnostic tool in evaluation of AUB. Our study is aimed at determining the spectrum of endometrial pathologies in different age group patients presenting with AUB at our hospital which caters largely to women living in rural area.
Methods: This was a retrospective study done on patients presenting with AUB over the period Jan 2013- Dec.2015, in Department of Pathology, Ashwini rural medical college and research centre, Kumbhari, Tq. South Solapur, in Western Maharashtra(India). The histopathological findings of AUB were categorized into functional and organic causes. A detailed histological study was carried out and the findings were noted. A statistical analysis between age of presentation and a specific endometrial cause was done using chi-square test.
Results: The most common age group presenting with AUB was 41- 50 yrs (42%). Mean age for women presented with AUB was 44.2 years. The commonest pattern in these patients was normal cyclical endometrium (33.9%): Proliferative and secretory phase. The commonest pathology was endometrial hyperplasia (22.2%). Other causes identified were disordered proliferative endometrium(13.7%), atrophic endometrium(13.2%), benign endometrial polyp(8%), deficient secretory phase(3.3%), chronic endometritis(2.4%), and endometrial carcinoma(2.3%). Histopathological patterns of endometrial lesions in patients with abnormal uterine Bleeding in rural area of Western Maharashtra

Background: Excessive and abnormal menstrual bleeding is the commonest symptom which brings a perimenopausal woman to the hospital. This age group is more amenable to serious conditions like genital cancer; hence this bleeding should be seriously evaluated to exclude the life threatening conditions by easily available investigating modalities like ultrasonography (USG) and biopsy before deciding their line of treatment. Objective: Analysing the histomorphological pattern of endometrium in abnormal uterine bleeding, Correlation b/w histopathology and bleeding pattern in AUB and to analyse the various risk factors associated with AUB. at a tertiary care centre. Materials and Methods: The material for the study was collected from the endometrial biopsy samples sent for histopathological examination from clinically diagnosed cases of Abnormal uterine bleeding from OPD, J.K.Lon Women hospital, GMC Kota. This was a prospective study done on patients presenting with AUB from Jan 2015 to Dec 2015 in the Department of Obstetrics & Gynaecology in collaboration with the department of Pathology of Govt. Medical College Kota, (Rajasthan). The study material included a total number of 100 specimens consisting hysterectomy specimens. Results: Abnormal uterine bleeding was high in parous women, Grand multipara 38%, multipara 55% (P3= 34%, P2= 21%). The majority of the women (49%) presented with menorrhagia, 26% with metrorhagia, polymenorrhagia was present in 20% of cases and Postmenopausal bleeding were seen in 5% cases. There were 29% fibroid uterus, Adenomyosis (26%) and 21% were diagnosed as DUB, fibroid with adenomyosis in (13%). Postmenopausal bleeding (5%) of cases. Endometrial polyp in 3% Endometrial Carcinoma were seen in 2% cases and Monckerbergs scleroma were seen in 1 case. Endometrium was Secretory in 66%, Proliferative (10%). Atrophic endometrium (9%). Endometrial hyperplasia (7%), endometrial malignancy (2%), Endometrial polyp (3%) ,Chronic endometritis (2%) and Monckerbergs scleroma was seen in 1% cases. Multiparity (55%) and grand multiparity (38%) was the major risk factors for AUB followed by hypertension (21%), PID (19%) Diabetes (12%), Obesity (4%) and tuberculosis is found in 1% case. There is significant correlation between Parity and Bleeding pattern (Coefficient of Correlation (r) = +0.896). There is significant correlation between Endometrial histopathological finding and Bleeding pattern (Coefficient of Correlation (r) = +0.98) Conclusion: Abnormal uterine bleeding predominantly affects women of perimenopuasal age group which needs thorough evaluation as it could be the only clinical manifestation of endometrial cancer. In our study, Organic causes were found to be the commonest cause of abnormal uterine bleeding followed by dysfunctional uterine bleeding. All these women underwent dilatation & curettage/ endometrial biopsy or hysterectomy depending on the diagnosis.

Correlation between histology and bleeding pattern in a case of AUB and risk factors analysis of AUB in reproductive age group

This study was conducted in 400 cases of endometrium in women of all age groups. The mean age of women presenting with abnormal uterine bleeding was 41 years. normal physiological changes of menstrual cycle, seen as proliferative and secretory phases of endometrium, were the most common histological findings present in 287 cases (71.75%). benign lesions were found in 19% cases followed by precursor lesions in 4.75% cases and malignant lesions in 4.5% cases. Histopathological examination of endometrial biopsy is a major diagnostic tool in evaluation of abnormal uterine bleeding (abnormal uterine bleeding) and a specific diagnosis could help the gynaecologist to plan therapy for successful management of abnormal uterine bleeding.
