This edition reflects the renewed interest in the immunological theories of multiple sclerosis. More than half the text is devoted to Professor Lumsden's analysis of the pathoIogy and, in particular, the chemical pathology of the immune response. There is a great deal of original work devoted to the chemistry and behaviour of the immunoglobulins. Much of this appears in specialist journals and one must be grateful for the critical summary provided here. Professor Lumsden unequivocally sees the key to the problem of multiple sclerosis in the study of its immunochemistry, relegating infection by a virus or a slow virus to a quite subsidiary role. The clinical studies drawing on wide practical experience help to get one's prejudices about the illness onto a more reasoned footing. The section on treatment is still sadly limited. Dr. McAlpine found little to add to the regime which he described in 1955. McAlpine, Lumsden, and Acheson's reappraisal is an essential reference for the practising neurologist and the new edition makes important modification of and changes in emphasis from the edition of 1965.

Introduction:Pathological gambling disorder (PG) has been associated with fronto-temporal dysfunction and maladaptive personality traits, such as impulsivity and novelty seeking. The purpose of this study was to examine the predictive variance of neuropsychological and personality characteristics in PG.Methods:Persons with PG (n=25) and a comparison group (n=34) were administered a battery of neuropsychological tests, the Temperament and Character Inventory, and the Barratt Impulsiveness Scale. Subjects with PG had evidence of fronto-temporal dysfunction as assessed by the Stroop, Wisconsin Card Sorting Test-64, Wechsler Adult Intelligence Scale Letter-Number Sequencing, Controlled Oral Word Association Test, and Boston Diagnostic Aphasia Examination Animal Naming Test.Results:Subjects with PG also had impaired decision making on the Iowa Gambling Task. PG subjects had elevated levels of impulsivity, novelty seeking, and harm avoidance, and lower levels of self-directedness and cooperativeness. Logistic regression analyses indicated that neuropsychological variables did not add significant incremental variance over personality traits in predicting PG (Block χ2=5.19, P=.074), while personality variables added significant incremental variance over neuropsychological traits in predicting PG (Block χ2=25.13, P<.001).Conclusion:These results suggest that personality traits are better predictors than neuropsychological characteristics of whether someone has PG.

Borderline personality disorder (BPD) has been associated both with frontolimbic dysfunction and maladaptive personality traits such as impulsivity and neuroticism. The purpose of this study was to examine whether there are differences in predictive variance between neuropsychological variables and selected personality traits in persons with BPD. We compared persons with DSM-IV BPD (n = 25) to individuals without BPD or substance abuse (n = 20). All subjects completed a battery of neuropsychological tests, Cloninger's Temperament and Character Inventory, and the Barratt Impulsiveness Scale. BPD subjects had evidence of cognitive inhibition, and deficits in working memory, perseveration, and decision-making not accounted for by IQ differences between groups. In particular, Stroop Interference remained significant after controlling for IQ, depression and alcohol use (p = 0.043). They also had higher levels of impulsivity, novelty seeking and harm avoidance, but lower levels of self-directedness and cooperativeness than the comparison group. Logistic regression analyses indicated that both personality traits (Nagelkerke R2 = 0.74, Model χ2 = 36.21, p < 0.001) and neuropsychological tests (Nagelkerke R2 = 0. 47, Model χ2 = 19.22, p < 0.001) were significant predictors of BPD. However, neuropsychological tests did not add significant incremental variance over selected personality trait dimensions in predicting BPD (Block χ2 = 6.45, p = 0.168), whereas personality traits added significant incremental variance over neuropsychological tests in predicting BPD (Block χ2 = 3.44, p < 0.001). This study suggests that persons with BPD have impaired neuropsychological performance, but that impulsivity and other selected personality traits have a primary role in predicting BPD over and above neuropsychological test abnormalities.
