For individuals with Down syndrome (DS), obstructive sleep apnea (OSA) is a complex disorder with significant clinical consequences. OSA is seen frequently in DS, and when present, it tends to be more severe. This increased prevalence is likely related to common anatomic abnormalities and a greater risk of additional comorbidities such as hypotonia and obesity. Because signs and symptoms do not often correlate with disease, all children and adults with DS should receive routine screening for OSA. Similar to the general population, polysomnography remains the gold standard for diagnosis. Because individuals with DS may be more susceptible to cardiovascular and neurocognitive sequelae, early diagnosis and treatment of OSA is becoming increasingly important. Treatment options generally involve upper airway surgery (primarily adenotonsillectomy) and continuous positive airway pressure (CPAP); however, various adjunctive therapies including intranasal steroids, palatal expansion, and oropharyngeal exercises are also available. Residual disease status post adenotonsillectomy is common, and further evaluation (eg, drug-induced sleep endoscopy [DISE]) is often needed. More advanced and directed airway surgery can be performed if additional sites of obstruction are observed. Novel therapies including hypoglossal nerve stimulation are emerging as effective treatments for refractory OSA. Due to the diversity among individuals with DS, personalized treatment plans should be developed. Within this arena, opportunities for research remain abundant and should include areas involving patient risk factors, alternative diagnostic methods, and outcome analysis.

Obstructive sleep apnea in patients with Down syndrome: current perspectives

Diagnosis, management and pathophysiology of central sleep apnea in children

BACKGROUND AND OBJECTIVES: New US Down syndrome (DS) BMI growth charts were recently published, but their utility in identifying children with excess adiposity or increased cardiometabolic risk (CMR) remains unknown. We sought to compare the ability of the Centers for Disease Control and Prevention (CDC) BMI 85th percentile and DS-specific BMI 85th percentile to identify excess adiposity in children with DS. METHODS: Participants with DS aged 10 to 20 years were enrolled in a cross-sectional CMR study. Data from typically developing children enrolled in the Bone Mineral Density in Childhood Study (BMDCS) were used for comparison. Sensitivity and specificity were calculated to assess the CDC BMI 85th percentile in the BMDCS and DS groups, and the DS-specific BMI 85th percentile in the DS group, relative to fat mass index (FMI) ≥80th percentile, a threshold associated with increased CMR. RESULTS: Included were 121 DS participants (age 14.8 ± 3.3 years, 57% girls) and 7978 BMDCS reference data points (age 15.0 ± 3.0 years, 51.3% girls). The CDC BMI 85th percentile identified FMI ≥80th percentile with 96.9% sensitivity and 87.4% specificity in typically developing children. Similarly, the CDC BMI 85th percentile identified FMI ≥80th percentile with 100% sensitivity and 78.3% specificity in children with DS. In contrast, the sensitivity of the DS-specific BMI 85th percentile was only 62.3% (P CONCLUSIONS: For children with DS ≥10 years, the CDC BMI growth chart 85th percentile is a better indicator of excess adiposity, than the new DS-specific BMI charts. Additional studies are needed to clarify the relationships of BMI and FMI with CMR in DS.

Body Composition and BMI Growth Charts in Children With Down Syndrome.

STUDY OBJECTIVES Obstructive sleep apnoea (OSAS) in children with Down syndrome (DS) is now well recognized, but other forms of sleep disordered breathing (SDB) in this population are less well described. Anecdotally, respiratory support for SDB treatment in this population is not easily tolerated. We aimed to characterize the types of SDB in children with DS referred to a tertiary respiratory center and to assess the effectiveness and adherence to respiratory support. METHODS Retrospective study of DS patients 4 h/night for >50% nights. RESULTS Sixty patients were included, median age 1.5 (0.7-5.3) years; 49 (82%) had congenital heart disease, 16 (27%) pulmonary hypertension, 28 (47%) gastroesophageal reflux, 38 (63%) swallowing impairment; 16/17 who underwent CT scanning had evidence of aspiration. Forty-two had SDB: 27 (61%) OSAS (10 mild, 5 moderate, 12 severe), 11 (25%) central apnoeas, 19 (32%) nocturnal hypoventilation. Twenty-six had baseline saturations <95%. Lower SpO2 correlated with pulmonary hypertension (r2  = 0.1, P = 0.04). Thirty-nine (65%) patients started respiratory support (14 O2 , 18 CPAP, 7 NIV) and 22 (56%) have regularly used it. After a 1.9 years follow up 11/24 had satisfactory adherence to CPAP/NIV (average use 8 h/night). CONCLUSIONS Our results confirm the high prevalence of OSAS in children with DS. A significant number also have low baseline saturations, central apnoeas, and nocturnal hypoventilation. Contrary to popular belief, more than half of children with DS had satisfactory adherence to respiratory support.

Neurodevelopmental Disorders Commonly Presenting with Sleep Disturbances

Patients with Down syndrome (DS) are at risk for both obstructive sleep apnea (OSA) and central sleep apnea (CSA); however, it is unclear how these components evolve as patients age and whether patients are also at risk for hypoventilation. A retrospective review of 144 diagnostic polysomnograms (PSG) in a tertiary care facility over 10 years was conducted. Descriptive data and exploratory correlation analyses were performed. Sleep disordered breathing was common (seen in 78% of patients) with an average apnea-hypopnea index (AHI) = 10. The relative amount of obstructive apnea was positively correlated with age and body mass index (BMI). The relative amount of central sleep apnea was associated with younger age in the very youngest group (0–3 years). Hypoventilation was common occurring in more than 22% of patients and there was a positive correlation between the maximum CO2 and BMI. Sleep disordered breathing, including hypoventilation, was common in patients with DS. The obstructive component increased significantly with age and BMI, while the central component occurred most in the very young age group. Due to the high risk of hypoventilation, which has not been previously highlighted, it may be helpful to consider therapies to target both apnea and hypoventilation in this population.
