These guidelines for the treatment of persons who have or are at risk for sexually transmitted diseases (STDs) were updated by CDC after consultation with a group of professionals knowledgeable in the field of STDs who met in Atlanta on April 18-30, 2009. The information in this report updates the 2006 Guidelines for Treatment of Sexually Transmitted Diseases (MMWR 2006;55[No. RR-11]). Included in these updated guidelines is new information regarding 1) the expanded diagnostic evaluation for cervicitis and trichomoniasis; 2) new treatment recommendations for bacterial vaginosis and genital warts; 3) the clinical efficacy of azithromycin for chlamydial infections in pregnancy; 4) the role of Mycoplasma genitalium and trichomoniasis in urethritis/cervicitis and treatment-related implications; 5) lymphogranuloma venereum proctocolitis among men who have sex with men; 6) the criteria for spinal fluid examination to evaluate for neurosyphilis; 7) the emergence of azithromycin-resistant Treponema pallidum; 8) the increasing prevalence of antimicrobial-resistant Neisseria gonorrhoeae; 9) the sexual transmission of hepatitis C; 10) diagnostic evaluation after sexual assault; and 11) STD prevention approaches.

Social network theory has made major contributions to our understanding of human social organisation but has found relatively little application in the field of animal behaviour. In this review, we identify several broad research areas where the networks approach could greatly enhance our understanding of social patterns and processes in animals. The network theory provides a quantitative framework that can be used to characterise social structure both at the level of the individual and the population. These novel quantitative variables may provide a new tool in addressing key questions in behavioural ecology particularly in relation to the evolution of social organisation and the impact of social structure on evolutionary processes. For example, network measures could be used to compare social networks of different species or populations making full use of the comparative approach. However, the networks approach can in principle go beyond identifying structural patterns and also can help with the understanding of processes within animal populations such as disease transmission and information transfer. Finally, understanding the pattern of interactions in the network (i.e. who is connected to whom) can also shed some light on the evolution of behavioural strategies.

Sexual networks: implications for the transmission of sexually transmitted infections

Summary
Social network analysis (SNA) and graph theory have been used widely in sociology, psychology, anthropology, biology and medicine. Social network analysis and graph theory provide a conceptual framework to study contact patterns and to identify units of analysis that are frequently or intensely connected within the network. Social network analysis has been used in human epidemiology as a tool to explore the potential transmission of infectious agents such as HIV, tuberculosis, hepatitis B and syphilis. In preventive veterinary medicine, SNA is an approach that offers benefits for exploring the nature and extent of the contacts between animals or farms, which ultimately leads to a better understanding of the potential risk for disease spread in a susceptible population. Social network analysis, however, has been applied only recently in preventive veterinary medicine, therefore the characteristics of the technique and the potential benefits of its use remain unknown for an important section of the international veterinary medicine community. The objectives of this paper were to review the concepts and theoretical aspects underlying the use of SNA and graph theory, with particular emphasis on their application to the study of infectious diseases of animals. The paper includes a review of recent applications of SNA in preventive veterinary medicine and a discussion of the potential uses and limitations of this methodology for the study of animal diseases.

Screening for Chlamydial Infection: U.S. Preventive Services Task Force Recommendation Statement

BACKGROUND: Social network methods have improved our understanding of sexually transmitted disease transmission dynamics, and may be of use in routine field operations for partner notification. GOAL: To augment traditional syphilis-control activities with social network methods in an Atlanta area with high syphilis morbidity. STUDY DESIGN: Disease investigators conducted interviews, used network diagrams to prioritize their work, and relied on network connections for finding hard-to-reach persons. RESULTS: A total of 396 contacts were elicited from 48 infected and 50 uninfected persons. The cumulative prevalence of syphilis was 12.6%, and 24 persons infected with HIV were identified. Network methods disclosed a large, interconnected group (276 persons) characterized by high network centrality and the substantial presence of small, interactive subgroups (microstructures). CONCLUSION: The network approach is a feasible field technique, and can identify core groups involved in the intense transmission of syphilis. The targeted, network-based approach may be useful in attempts to eliminate syphilis.
