Foot ulceration results in substantial morbidity amongst diabetic patients. We have studied prospectively the relationship between high foot pressures and foot ulceration using an optical pedobarograph. A series of 86 diabetic patients, mean age 53.3 (range 17–77) years, mean duration of diabetes 17.1 (range 1–36) years, were followed-up for a mean period of 30 (range 15–34) months. Clinical neuropathy was present in 58 (67%) patients at baseline examination. Mean peak foot pressure was higher at the follow-up compared to baseline (13.5 kg·cm−2±7.1 SD vs 11.2±5.4, p 12.3) being present in 55 patients at follow-up and 43 at the baseline visit (p=NS). Plantar foot ulcers developed in 21 feet of 15 patients (17%), all of whom had abnormally high pressures at baseline; neuropathy was present in 14 patients at baseline. Non-plantar ulcers occurred in 8 (9%) patients. Thus, plantar ulceration occurred in 35% of diabetic patients with high foot pressures but in none of those with normal pressures. We have shown for the first time in a prospective study that high plantar foot pressures in diabetic patients are strongly predictive of subsequent plantar ulceration, especially in the presence of neuropathy.

The risk of foot ulceration in diabetic patients with high foot pressure: a prospective study.

Studies related to the treatment of pressure sores have appeared in the literature. Questions remain regarding both the etiology and treatment. This article presents a conceptual schema within which current knowledge can be organized and examined and further study facilitated. The critical determinants of pressure sores are primarily the intensity and duration of pressure. A second critical concept, the tolerance of the tissue for pressure, is also discussed. Conditions contributing to prolonged and intense pressure are included within the concepts of mobility, activity, and sensory perception. Tissue tolerance for pressure is influenced by both extrinsic and intrinsic factors. Extrinsic factors such as moisture, friction, and shear impinge on the skin and underlying tissues, while the multiple influences of nutrition, the physiologic effects of stress, aging, and alterations in cellular respiration are intrinsic factors which influence the architecture and integrity of the skin and supporting structures. The schema provides broad categories capable of organizing existing knowledge and new findings.

A conceptual schema for the study of the etiology of pressure sores.

A cross-sectional survey was done to determine the prevalence of pressure sores in hospitalized patients and the factors associated with having a pressure sore in the hospital. Among 634 adult patients, 30 (4.7%; 3.1% to 6.3%, 95% confidence interval) had a pressure sore and 78 (12.3%; 9.8% to 14.8%) were at risk for a pressure sore because they had been confined to a bed or chair for at least 1 week. Comparing these two groups of patients, we found that fecal incontinence, diarrhea, fractures, urinary catheter use, decreased weight, dementia, and hypoalbuminemia were associated with having pressure sores (p less than or equal to 0.05). Using logistic regression analysis, hypoalbuminemia, fecal incontinence, and fractures remained significantly and independently associated with having a pressure sore (odds ratios = 3.0, 3.1, and 5.2, respectively; p less than or equal to 0.05). Our findings suggest that 17% (14% to 20%) of hospitalized patients have pressure sores or are at risk for them, and that hypoalbuminemia, fecal incontinence, and fractures may identify bedridden patients at greatest risk.
