Assessment of intake from the diet

Abstract Following a request from the European Commission, the EFSA Panel on Dietetic Products, Nutrition and Allergies (NDA) derives dietary reference values (DRVs) for vitamin K. In this Opinion, the Panel considers vitamin K to comprise both phylloquinone and menaquinones. The Panel considers that none of the biomarkers of vitamin K intake or status is suitable by itself to derive DRVs for vitamin K. Several health outcomes possibly associated with vitamin K intake were also considered but data could not be used to establish DRVs. The Panel considers that average requirements and population reference intakes for vitamin K cannot be derived for adults, infants and children, and therefore sets adequate intakes (AIs). The Panel considers that available evidence on occurrence, absorption, function and content in the body or organs of menaquinones is insufficient, and, therefore, sets AIs for phylloquinone only. Having assessed additional evidence available since 1993 in particular related to biomarkers, intake data and the factorial approach, which all are associated with considerable uncertainties, the Panel maintains the reference value proposed by the Scientific Committee for Food (SCF) in 1993. An AI of 1 μg phylloquinone/kg body weight per day is set for all age and sex population groups. Considering the respective reference body weights, AIs for phylloquinone are set at 70 μg/day for all adults including pregnant and lactating women, at 10 μg/day for infants aged 7–11 months, and between 12 μg/day for children aged 1–3 years and 65 μg/day for children aged 15–17 years.

Impact of Trace Elements and Vitamin Supplementation on Immunity and Infections in Institutionalized Elderly Patients

This article provides an overview of the longitudinal Survey in Europe on Nutrition and the Elderly: a Concerted Action (SENECA) study, which was designed to assess differences in dietary and lifestyle factors among elderly Europeans, and to identify the factors that contribute to healthy aging. Elderly people from Belgium, Denmark, France, Italy, Portugal, Spain, Switzerland, and The Netherlands participated in the SENECA study. Standardized measurements were conducted at baseline in 1988-1989 and were repeated in 1993 and 1999. Diet, physical activity, and smoking, as well as maintenance of health and survival, were assessed. At baseline, considerable differences in lifestyle factors existed among elderly people. Mealtime patterns as well as dietary intake varied across Europe, and geographical patterns were apparent. Similar results were found for engagement in sport or professional activities. The smoking prevalence among women was generally low. Distinct geographical differences were also observed in percentages of deaths during the SENECA study and in overall survival time. A healthy lifestyle was related to stable self-perceived health, a delay in functional dependence, and mortality. Inactivity and smoking, and to a lesser extent a low-quality diet, increased mortality risk. A combined effect of multiple unhealthy lifestyle factors was also observed. The SENECA study showed that a healthy lifestyle at older ages is related to a delay in the deterioration of health status and a reduced mortality risk. Improving and maintaining a healthy lifestyle in elderly people across Europe is a great challenge for the European Community.

Lifestyle, Nutritional Status, Health, and Mortality in Elderly People Across Europe: A Review of the Longitudinal Results of the SENECA Study

Background: Vitamin B 6 is thought to be a most versatile coenzyme that participates in more than 100 biochemical reactions. It is involved in amino acid and homocysteine metabolism, glucose and lipid metabolism, neurotransmitter production and DNA/RNA synthesis. Vitamin B 6 can also be a modulator of gene expression. Nowadays, clinically evident vitamin B 6 deficiency is not a common disorder, at least in the general population. Nevertheless, a subclinical, undiagnosed deficiency may be present in some subjects, particularly in the elderly. Objective: This review gives a complete overview over the metabolism and interactions of vitamin B 6 . Further, we show which complications and deficiency symptoms can occur due to a lack of vitamin B 6 and possibilities for public health and supplemental interventions. Methods: The database Medline (www.ncvi.nlm.nih.gov) was searched for terms like “vitamin B 6 ”, “pyridoxal”, “cancer”, “homocysteine”, etc. For a complete understanding, we included studies with early findings from the forties as well as recent results from 2006. These studies were summarised and compared in different chapters. Results and conclusion: In fact, it has been proposed that suboptimal vitamin B 6 status is associated with certain diseases that particularly afflict the elderly population: impaired cognitive function, Alzheimer’s disease, cardiovascular disease, and different types of cancer. Some of these problems may be related to the elevated homocysteine concentrations associated to vitamin B 6 deficiency, but there is also evidence for other mechanisms independent of homocysteine by which a suboptimal vitamin B 6 status could increase the risk for these chronic diseases. Vitamin B6 status, deficiency and its consequences--an overview.

This chapter describes the vitamin plasma concentration data collection, preliminary analysis and results of the Euronut SENECA study. Blood plasma was collected from approximately 2500 elderly subjects born in 1913-1918 living in 17 small towns in 11 European countries, and the plasma levels of carotene, retinol, alpha-tocopherol, vitamin B12, folic acid and pyridoxal 5'-phosphate were determined. There were very large within- and between-centre differences in vitamin levels with no definite geographical pattern emerging. The vitamin status for retinol and folic acid was adequate in all centres. The prevalence of biochemical vitamin B6 deficiency was widespread and reached over 50% in some centres. Vitamin B12 biochemical deficiency was limited to ten centres and its prevalence was 1.6%-10%. Vitamin E biochemical deficiency was found in seven centres and varied from 0.5% to 25%.
