PURPOSE: There is a lack of homogeneity in the terminology used in the context of patient safety related to medication. The aim of this review was to identify the terms and definitions used in patient safety related to medication within the scientific literature. METHODS: Original and review articles that were indexed between 1998 and 2008 in MEDLINE and EMBASE and contained terms used in patient safety related to medication were included. Terms and definitions were extracted and categorised according to whether its definition referred to the process of medication use, or to the clinical outcome of medication use, or both. RESULTS: Of 2564 articles, 147 were included. Sixty terms used in patient safety related to medication with 189 different definitions were identified. Among terms that referred only to the process of medication use (n = 23), medication error provided the greatest number of definitions (n = 29). Among terms that referred only to the clinical outcome of medication use (n = 31), adverse drug event provided the greatest number of definitions (n = 15). Finally, among terms that referred both to the process of use and to the clinical outcome of medication use (n = 13), drug-related problem provided the greatest number of definitions (n = 7). CONCLUSIONS: A multitude of terms and definitions are used in patient safety related to medication. This heterogeneity makes it difficult to compare the results among studies and to appreciate the true magnitude of the problem. Background:Drug-related problems (DRPs) in the general population account for 15% of all hospital admissions, of which approximately 30% are preventable. Cardiac transplant patients may be at increased risk for DRPs because of their complicated medication regimens that include drugs with a narrow therapeutic index.Objective:To determine the incidence and praventability of DRPs causing hospital admission in cardiac transplant patients at a single institution.Methods:Between November 2009 and January 2010, a prospective longitudinal study investigated the incidence and preventability of DRPs in a single cardiac transplant center. Three independent reviewers used validated scoring systems to determine the incidence and preventability of drug-related hospital admissions. Drug-Related Problems and Hospital Admissions in Cardiac Transplant Recipients

Background The role of the pharmacist has evolved greatly over the last decades, expanding to patient-oriented activities, administrative tasks and public health functions. However, considerable differences emerge across regions. Aim of the review To gather evidence in order to describe and highlight the different characteristics of the pharmacists’ role and the impact of their activities in Spain. Method A review of the existing literature was conducted. The literature search was undertaken in PubMed between 01/01/2006 and 15/08/2017. Results were screened and reviewed to extract previously established criteria such as author(s), publication year, language, study design, setting, pharmaceutical activity, patient care programs, targeted diseases and intervention description using DEPICT2 tool. Pharmaceutical intervention were classified into eight outcome measures and categorized by types of outcomes reported: descriptive or impact evaluation regarding the effect of the service (positive, neutral or negative). Results The search strategy resulted in 473 articles and 108 articles met the inclusion criteria. The most common design was observational (n = 76, 70%). Most articles were published after 2011 (75%), in English (69%). Studies were conducted in hospitals (60%) and community pharmacies (30%). Of the 24 pharmaceutical activities identified, medication review was the activity most frequently studied (n = 42), followed by patient education (n = 29), risk and prevention (n = 27) and medication reconciliation (n = 19). Only 39 articles (36%) had outcome measures with impact evaluation. Of the 223 impact outcome measures, 48% (107/223) had a positive effect. Conclusion This review shows the substantial scientific production focusing on pharmacy practice in Spain over the last years. The evidence reviewed reflects the pharmacist role at various professional settings, providing a wide variety of activities on diverse targeted diseases and patient care programs, in line with the increasing specialization of clinical pharmacists over the last years.

Role and impact of pharmacists in Spain: a scoping review.

Resumen Objetivo Realizar una revision bibliografica para identificar las diferentes metodologias empleadas en el proceso de conciliacion de los tratamientos farmacologicos que sean aplicables a pacientes pluripatologicos. Diseno Revision sistematica. Fuentes de datos Se realizo una revision bibliografica (febrero de 2012) en las bases de datos Pubmed, EMBASE, CINAHL, PsycINFO e Indice Medico Espanol de metodos de conciliacion del tratamiento en pacientes pluripatologicos, o en su defecto, ancianos o polimedicados. Seleccion de estudios Se recuperaron 273 articulos, de los que se seleccionaron 25. Extraccion de datos Se extrajo informacion relativa a la metodologia empleada: nivel asistencial en el que se realiza, fuentes de informacion, uso de formulario, tiempo establecido, profesional responsable, informacion recogida y variables registradas como errores de conciliacion. Resultados La mayoria de estudios fueron al ingreso y al alta hospitalarios Como principales fuentes de informacion destacan la entrevista y la historia clinica. En la mayoria de trabajos no se especifica un tiempo preestablecido, ni se usa formulario, y el principal responsable es el farmaceutico clinico. Ademas de la medicacion domiciliaria, los habitos de automedicacion y la fitoterapia tambien son registrados. Se recogen como errores de conciliacion desde omisiones de farmacos hasta interacciones medicamentosas. Conclusiones Existe gran heterogeneidad en la metodologia empleada para la actividad de la conciliacion. No existe ningun trabajo realizado especificamente en el paciente pluripatologico, que por su complejidad y susceptibilidad a errores de conciliacion requiere una metodologia estandarizada.
