Introduction
A body of basic science and clinical research has been generated on the manual muscle test (MMT) since its first peer-reviewed publication in 1915. The aim of this report is to provide an historical overview, literature review, description, synthesis and critique of the reliability and validity of MMT in the evaluation of the musculoskeletal and nervous systems.

On the reliability and validity of manual muscle testing: a literature review

This prospective study was conducted to determine whether hip muscle strength and flexibility play a role in the incidence of adductor and hip flexor strains in National Hockey League ice hockey team players. Hip flexion, abduction, and adduction strength were measured in 81 players before two consecutive seasons. Thirty-four players were cut, traded, or sent to the minor league before the beginning of the season. Injury and individual exposure data were recorded for the remaining 47 players. Eight players experienced 11 adductor muscle strains, and there were 4 hip flexor strains. Preseason hip adduction strength was 18% lower in the players who subsequently sustained an adductor muscle strain compared with that of uninjured players. Adduction strength was 95% of abduction strength in the uninjured players but only 78% of abduction strength in the injured players. Preseason hip adductor flexibility was not different between players who sustained adductor muscle strains and those who did not. These results indicate that preseason hip strength testing of professional ice hockey players can identify players at risk of developing adductor muscle strains. A player was 17 times more likely to sustain an adductor muscle strain if his adductor strength was less than 80% of his abductor strength.

The association of hip strength and flexibility with the incidence of adductor muscle strains in professional ice hockey players.

OBJECTIVE Recent survey evidence suggests that sensory ataxia due to diabetic neuropathy may be a more frequent and serious problem than is commonly recognized. This view is further supported by research that confirms the major contribution of the somatosensory system to the control of posture. We therefore sought to determine the effects of significant diabetic distal symmetrical polyneuropathy on the control of posture.

RESEARCH DESIGN AND METHODS Fifty-one subjects, divided into three groups, participated in this study. Seventeen had diabetes and significant sensory neuropathy, 17 had diabetes and no neuropathy, and 17 had neither diabetes nor neuropathy. The subjects were matched across groups, and stringent exclusion criteria were applied. Postural stability during quiet standing was measured using a force platform. In addition to electrophysiological and quantitative sensory tests of neuropathy, a number of physical and functional characteristics were measured for all subjects.

RESULTS Postural instability was found to be significantly associated with sensory neuropathy, but not with diabetes per se. Patients with sensory neuropathy demonstrated between 66 and 117% more instability than did control subjects (depending on the testing condition). Based on multiple linear regression analyses, the most significant correlates of instability were the quantitative sensory measures of neuropathy and age.

CONCLUSIONS The loss of sensory perception secondary to diabetic distal symmetrical sensory neuropathy has a markedly detrimental effect on postural stability. The deficit is greatest when visual or vestibular cues are absent or degraded. Patients with neuropathy need to be informed of the postural consequences of this condition to limit the potential morbidity caused by falls.

Postural instability in patients with diabetic sensory neuropathy.

Objective To determine the relationship of previous lower extremity (LE) injury and/or low back pain (LBP) on hip abduction and extension strength. Design Cohort study of college athletes at time of preparticipation screening physical. Setting An NCAA Division I college. Participants Two hundred ten college athletes (140 males and 70 females) from an NCAA Division I school. Main outcome measures Mean and maximal hip abduction and extension strengths were recorded using a specially designed dynamometer anchoring station. Previous injury to the LE or LBP in the past year was recorded via personal interview at the time of screening and verified by review of previous injury records. Results A significant difference in side-to-side symmetry of maximum hip extension strength was observed in female subjects who reported LE injury or LBP as compared to those who did not. Side-to-side difference in hip strength, however, did not differ between male athletes, regardless of reported LE injury or LBP status. Conclusion Female athletes appear to have a differing response of the proximal hip musculature to LE injury or LBP, as compared with their male counterparts. Research is under way to further validate these findings. Clinical relevance This study provides some reasoning to support the screening of hip strength during the preparticipation physical, as it may be important in the prevention of LE injury and LBP in collegiate athletes.

Test-retest strength reliability: Hand-held dynamometry in community-dwelling elderly fallers

Physical therapists require an accurate, reliable method for measuring muscle strength. They often use manual muscle testing or hand-held dynametric muscle testing (DMT), but few studies document the reliability of MMT or compare the reliability of the two types of testing. We designed this study to determine the intrarater reliability of MMT and DMT. A physical therapist performed manual and dynametric strength tests of the same five muscle groups on 11 patients and then repeated the tests two days later. The correlation coefficients were high and significantly different from zero for four muscle groups tested dynametrically and for two muscle groups tested manually. The test-retest reliability coefficients for two muscle groups tested manually could not be calculated because the values between subjects were identical. We concluded that both MMT and DMT are reliable testing methods, given the conditions described in this study. Both testing methods have specific applications and limitations, which we discuss.
