The psychological impact of quarantine and how to reduce it: rapid review of the evidence.

Interest in meaning and meaning making in the context of stressful life events continues to grow, but research is hampered by conceptual and methodological limitations. Drawing on current theories, the author first presents an integrated model of meaning making. This model distinguishes between the constructs of global and situational meaning and between "meaning-making efforts" and "meaning made," and it elaborates subconstructs within these constructs. Using this model, the author reviews the empirical research regarding meaning in the context of adjustment to stressful events, outlining what has been established to date and evaluating the strengths and weaknesses of current empirical work. Results suggest that theory on meaning and meaning making has developed apace, but empirical research has failed to keep up with these developments, creating a significant gap between the rich but abstract theories and empirical tests of them. Given current empirical findings, some aspects of the meaning-making model appear to be well supported but others are not, and the quality of meaning-making efforts and meanings made may be at least as important as their quantity. This article concludes with specific suggestions for future research.

Making Sense of the Meaning Literature: An Integrative Review of Meaning Making and Its Effects on Adjustment to Stressful Life Events.

The present meta-analysis investigated the effectiveness of strategies derived from the process model of emotion regulation in modifying emotional outcomes as indexed by experiential, behavioral, and physiological measures. A systematic search of the literature identified 306 experimental comparisons of different emotion regulation (ER) strategies. ER instructions were coded according to a new taxonomy, and meta-analysis was used to evaluate the effectiveness of each strategy across studies. The findings revealed differences in effectiveness between ER processes: Attentional deployment had no effect on emotional outcomes (d+ = 0.00), response modulation had a small effect (d+ = 0.16), and cognitive change had a small-to-medium effect (d+ = 0.36). There were also important within-process differences. We identified 7 types of attentional deployment, 4 types of cognitive change, and 4 types of response modulation, and these distinctions had a substantial influence on effectiveness. Whereas distraction was an effective way to regulate emotions (d+ = 0.27), concentration was not (d+ = −0.26). Similarly, suppressing the expression of emotion proved effective (d+ = 0.32), but suppressing the experience of emotion or suppressing thoughts of the emotion-eliciting event did not (d+ = −0.04 and −0.12, respectively). Finally, reappraising the emotional response proved less effective (d+ = 0.23) than reappraising the emotional stimulus (d+ = 0.36) or using perspective taking (d+ = 0.45). The review also identified several moderators of strategy effectiveness including factors related to the (a) to-be-regulated emotion, (b) frequency of use and intended purpose of the ER strategy, (c) study design, and (d) study characteristics. However, research and treatment in this field typically adopts a DSM driven 'disorder-focused' approach - researchers and clinicians target a specific disorder, try to understand its aetiology and maintenance, and try to develop more effective strategies to treat the disorder. This book proposes an insightful and original approach to understanding these disorders, one that focuses on what they have in common. Instead of examining in isolation, for example, obsessive compulsive disorders, insomnia, schizophrenia, it asks - what do patients with these disorders have in common? It takes each cognitive and behavioural process - attention, memory, reasoning, thought, behaviour, and examines whether it is a transdiagnostic process - i.e., serves to maintain a broad range of psychological disorders. Having shown how these disorders share several important processes, it then describes the practical implications of such an approach to diagnosis and treatment. Importantly it explores why the different psychological disorders can present so differently, despite being maintained by the same cognitive and behavioural processes. Cognitive Behavioural Processes across Psychological Disorders: A Transdiagnostic Approach to Research and Treatment

Importance Irritable bowel syndrome (IBS) affects 7% to 21% of the general population. It is a chronic condition that can substantially reduce quality of life and work productivity. Objectives To summarize the existing evidence on epidemiology, pathophysiology, and diagnosis of IBS and to provide practical treatment recommendations for generalists and specialists according to the best available evidence. Evidence Review A search of Ovid (MEDLINE) and Cochrane Database of Systematic Reviews was performed for literature from 2000 to December 2014 for the terms pathophysiology , etiology , pathogenesis , diagnosis , irritable bowel syndrome , and IBS . The range was expanded from 1946 to December 2014 for IBS , irritable bowel syndrome , diet , treatment , and therapy . Findings The database search yielded 1303 articles, of which 139 were selected for inclusion. IBS is not a single disease but rather a symptom cluster resulting from diverse pathologies. Factors important to the development of IBS include alterations in the gut microbiome, intestinal permeability, gut immune function, motility, visceral sensation, brain-gut interactions, and psychosocial status. The diagnosis of IBS relies on symptom-based criteria, exclusion of concerning features (symptom onset after age 50 years, unexplained weight loss, family history of selected organic gastrointestinal diseases, evidence of gastrointestinal blood loss, and unexplained iron-deficiency anemia), and the performance of selected tests (complete blood cell count, C-reactive protein or fecal calprotectin, serologic testing for celiac disease, and age-appropriate colorectal cancer screening) to exclude organic diseases that can mimic IBS. Determining the predominant symptom (IBS with diarrhea, IBS with constipation, or mixed IBS) plays an important role in selection of diagnostic tests and treatments. Various dietary, lifestyle, medical, and behavioral interventions have proven effective in randomized clinical trials. Conclusions and Relevance The diagnosis of IBS relies on the identification of characteristic symptoms and the exclusion of other organic diseases. Management of patients with IBS is optimized by an individualized, holistic approach that embraces dietary, lifestyle, medical, and behavioral interventions.

Irritable Bowel Syndrome: A Clinical Review

Introduction: Given the breadth of correlational research linking social media use to worse well-being, we undertook an experimental study to investigate the potential causal role that social media plays in this relationship. Method: After a week of baseline monitoring, 143 undergraduates at the University of Pennsylvania were randomly assigned to either limit Facebook, Instagram and Snapchat use to 10 minutes, per platform, per day, or to use social media as usual for three weeks. Results: The limited use group showed significant reductions in loneliness and depression over three weeks compared to the control group. Both groups showed significant decreases in anxiety and fear of missing out over baseline, suggesting a benefit of increased self-monitoring. Discussion: Our findings strongly suggest that limiting social media use to approximately 30 minutes per day may lead to significant improvement in well-being.

One problem in identifying and treating depression is underreporting of symptoms by individuals. Previous research suggests that there may be systematic sex differences in self-report bias, with men tending to minimize their depressive symptoms more than women. This study used an experimental design with a sample of 238 community members to test whether disguising the purpose of the Beck Depression Inventory-II would significantly reduce self-report bias, especially in men. We found a main effect of condition such that both men and women reported significantly more core depressive symptoms in the covert condition, suggesting that surveys of community samples may underestimate the prevalence of depression.

One of the many impacts of natural disasters on the well-being of the humans who experience them is enforced abandonment and loss of companion animals. Hurricane Katrina, which struck the gulf coast of the United States in late August, 2005, was such a disaster. This study assessed the psychological effects of pet loss on survivors of Hurricane Katrina. Sixty-five predominantly white, female, middle-aged pet owners who lived in affected regions of the country completed online questionnaires, assessing symptoms of depression, acute stress, peri-traumatic dissociation, and posttraumatic stress disorder (PTSD). Almost all companion animals were cats or dogs. Pet loss was strongly associated with psychopathology across all measures, even when controlling for displacement from the home (Wilks' Lambda F(4,57) = 5.22, p = 0.001).
