Modern network science has revealed fundamental aspects of normal brain-network organization, such as small-world and scale-free patterns, hierarchical modularity, hubs and rich clubs. The next challenge is to use this knowledge to gain a better understanding of brain disease. Recent developments in the application of network science to conditions such as Alzheimer's disease, multiple sclerosis, traumatic brain injury and epilepsy have challenged the classical concept of neurological disorders being either 'local' or 'global', and have pointed to the overload and failure of hubs as a possible final common pathway in neurological disorders.

Modern network science of neurological disorders

In recent years, numerous electrophysiological signatures of consciousness have been proposed. Here, we perform a systematic analysis of these electroencephalography markers by quantifying their efficiency in differentiating patients in a vegetative state from those in a minimally conscious or conscious state. Capitalizing on a review of previous experiments and current theories, we identify a series of measures that can be organized into four dimensions: (i) event-related potentials versus ongoing electroencephalography activity; (ii) local dynamics versus inter-electrode information exchange; (iii) spectral patterns versus information complexity; and (iv) average versus fluctuations over the recording session. We analysed a large set of 181 high-density electroencephalography recordings acquired in a 30 minutes protocol. We show that low-frequency power, electroencephalography complexity, and information exchange constitute the most reliable signatures of the conscious state. When combined, these measures synergize to allow an automatic classification of patients’ state of consciousness.

Large scale screening of neural signatures of consciousness in patients in a vegetative or minimally conscious state.

Theoretical advances in the science of consciousness have proposed that it is concomitant with balanced cortical integration and differentiation, enabled by efficient networks of information transfer across multiple scales. Here, we apply graph theory to compare key signatures of such networks in high-density electroencephalographic data from 32 patients with chronic disorders of consciousness, against normative data from healthy controls. Based on connectivity within canonical frequency bands, we found that patient networks had reduced local and global efficiency, and fewer hubs in the alpha band. We devised a novel topographical metric, termed modular span, which showed that the alpha network modules in patients were also spatially circumscribed, lacking the structured long-distance interactions commonly observed in the healthy controls. Importantly however, these differences between graph-theoretic metrics were partially reversed in delta and theta band networks, which were also significantly more similar to each other in patients than controls. Going further, we found that metrics of alpha network efficiency also correlated with the degree of behavioural awareness. Intriguingly, some patients in behaviourally unresponsive vegetative states who demonstrated evidence of covert awareness with functional neuroimaging stood out from this trend: they had alpha networks that were remarkably well preserved and similar to those observed in the controls. Taken together, our findings inform current understanding of disorders of consciousness by highlighting the distinctive brain networks that characterise them. In the significant minority of vegetative patients who follow commands in neuroimaging tests, they point to putative network mechanisms that could support cognitive function and consciousness despite profound behavioural impairment.

Spectral signatures of reorganised brain networks in disorders of consciousness.

The aim of this study was to look for differences in the power spectra and in EEG connectivity measures between patients in the vegetative state (VS/UWS) and patients in the minimally conscious state (MCS). The EEG of 31 patients was recorded and analyzed. Power spectra were obtained using modern multitaper methods. Three connectivity measures (coherence, the imaginary part of coherency and the phase lag index) were computed. Of the 31 patients, 21 were diagnosed as MCS and 10 as VS/UWS using the Coma Recovery Scale-Revised (CRS-R). EEG power spectra revealed differences between the two conditions. The VS/UWS patients showed increased delta power but decreased alpha power compared with the MCS patients. Connectivity measures were correlated with the CRS-R diagnosis; patients in the VS/UWS had significantly lower connectivity than MCS patients in the theta and alpha bands. Standard EEG recorded in clinical conditions could be used as a tool to help the clinician in the diagnosis of disorders of consciousness.

Behaviour of Granger causality under filtering: Theoretical invariance and practical application

In this study we explored the use of coherence and Granger causality (GC) to separate patients in minimally conscious state (MCS) from patients with severe neurocognitive disorders (SND) that show signs of awareness. We studied 16 patients, 7 MCS and 9 SND with age between 18 and 49 years. Three minutes of ongoing electroencephalographic (EEG) activity was obtained at rest from 19 standard scalp locations, while subjects were alert but kept their eyes closed. GC was formulated in terms of linear autoregressive models that predict the evolution of several EEG time series, each representing the activity of one channel. The entire network of causally connected brain areas can be summarized as a graph of incompletely connected nodes. The 19 channels were grouped into five gross anatomical regions, frontal, left and right temporal, central, and parieto-occipital, while data analysis was performed separately in each of the five classical EEG frequency bands, namely delta, theta, alpha, beta, and gamma. Our results showed that the SND group consistently formed a larger number of connections compared to the MCS group in all frequency bands. Additionally, the number of connections in the delta band (0.1-4 Hz) between the left temporal and parieto-occipital areas was significantly different (P < 0.1%) in the two groups. Furthermore, in the beta band (12-18 Hz), the input to the frontal areas from all other cortical areas was also significantly different (P < 0.1%) in the two groups. Finally, classification of the subjects into distinct groups using as features the number of connections within and between regions in all frequency bands resulted in 100% classification accuracy of all subjects. The results of this study suggest that analysis of brain connectivity networks based on GC can be a highly accurate approach for classifying subjects affected by severe traumatic brain injury.

This presentation provides a demonstration of how Granger causality (GC) can be applied to MEG data to visualize dynamic functional connectivity and causality between cortical regions on a millisecond time scale. GC is derived from autoregressive models and provides directionality information. We apply the GC technique to dynamic statistical parameter map source space to demonstrate that the dynamics of neural networks can visualized during a perceptual task. The results from this demonstration coincide with models of speech perception and suggest that Dynamic Autoregressive Neuromagnetic Causality Imaging (DANCI) can be used to investigate and verify theoretical neural network models of brain function.
