Constructing Grounded Theory: A Practical Guide through Qualitative Analysis

The self-medication hypothesis of addictive disorders derives primarily from clinical observations of patients with substance use disorders. Individuals discover that the specific actions or effects of each class of drugs relieve or change a range of painful affect states. Self-medication factors occur in a context of self-regulation vulnerabilities--primarily difficulties in regulating affects, self-esteem, relationships, and self-care. Persons with substance use disorders suffer in the extreme with their feelings, either being overwhelmed with painful affects or seeming not to feel their emotions at all. Substances of abuse help such individuals to relieve painful affects or to experience or control emotions when they are absent or confusing. Diagnostic studies provide evidence that variously supports and fails to support a self-medication hypothesis of addictive disorders. The cause-consequence controversy involving psychopathology and substance use/abuse is reviewed and critiqued. In contrast, clinical observations and empirical studies that focus on painful affects and subjective states of distress more consistently suggest that such states of suffering are important psychological determinants in using, becoming dependent upon, and relapsing to addictive substances. Subjective states of distress and suffering involved in motives to self-medicate with substances of abuse are considered with respect to nicotine dependence and to schizophrenia and posttraumatic stress disorder comorbid with a substance use disorder.

An Occupational Perspective of Health

The purpose of this article is to understand a current status of sociolinguistics by reviewing previous studies and attempting to see the future of the discipline as linguistics. As Milroy and Milroy (1990:485) have defined, sociolinguistics is "the study of language as it is used by real speakers in social and situational contexts of use." It has four characteristics. (1)Those who study sociolinguistics are linguists but they have great interests in adding social variables to pure linguistics. Sociolinguists believe that criteria of correct language usage be based upon not only pure grammatical standards but also societal norms in terms of its relevance and general acceptance. (2)The goal of sociolinguistics is to identify a co-variance between language and society and to establish a theory of language performance. (3)Sociolinguistics regards synchronical and diachronical traits as an identical frame. (4)Sociolingustics pays attention to language usage in societal contexts and extends language competence, which is the main subject of pure linguistics, to communicative competence. D. Hymes predicts that the core areas of linguistics is actually sociolinguistics and, thus, the prefix 'socio' will not be necessary. Although we still have that prefix, it is true that sociolingusitics has already had its own identity and is growing rapidly as an independent discipline. In conclusion, this paper argues that sociolinguistics will receive more attention from linguists and play a key role in linguistics by explaining variation in language more systematically, and by interpreting and eliminating language conflict in everyday life.

Patient-related violence at triage: A qualitative descriptive study

In a finding that reflects international experiences, nurses in Australia have been identified as the occupation at most risk of patient-related violence in the health-care sector. A search of the literature was undertaken to explore this concept, with a focus on the emergency department and triage nurses. Significant findings included the fact that nurses are subjected to verbal and physical abuse so frequently that, in many instances, it has become an accepted part of the job. This attitude, combined with the chronic under-reporting of violent incidents, perpetuates the normalization of violence, which then becomes embedded in the workplace culture and inhibits the development of preventative strategies and the provision of a safe working environment. Nurses are entitled to a safe workplace that is free from violence under both the occupational health and safety legislation and the zero-tolerance policies that have been adopted in many countries including Australia, the UK, Europe, and the USA. Therefore, policy-makers and administrators should recognize this issue as a priority for preventative action.

Patient-related violence against emergency department nurses.

This paper presents some of the key findings and recommendations of the report The Tasmanian Children's Project (TCP): The Needs of Children with a Parent/Carer with a Mental Illness, October, 1999. The TCP, a collaborative venture between the University of Tasmania's School of Nursing and the Mental Health Services - South, Tasmania (Department of Health and Human Services), is the first study in Tasmania to formally examine the needs of children where the parent/carer has a mental illness. The study is a modified replication and extension of the 1993/94 Victorian Children's Project. Extension aspects of the TCP included interviews with children (in addition to parents and service providers), the inclusion of data on both maternal and paternal mental illness and a broad definition of mental illness (beyond psychotic illness and major affective disorder). The report highlights the need to provide a range of programs that encourage the development of personal competency among children, parents, and other family members and those that emphasize interagency collaboration. Implications of this research for mental health nursing education and practice are also addressed.

The Tasmanian children's project: the needs of children with a parent/carer with a mental illness.

In late 1999 the National Mental Health Working Group of the Australian Health Ministers Advisory Council commissioned the Australian and New Zealand College of Mental Health Nurses to undertake a scoping study of mental health nursing. A final report will be submitted to the National Mental Health Working Group in February 2000. The purpose of this article is to draw attention to some of the systemic problems that confront the education of mental health nurses in Australia. Shortcomings in the preparation of undergraduate students of nursing for commencing practice in mental health nursing are described and comments are given on issues affecting the quality of postgraduate mental health nursing education.
